In conclusion, targeting the mGiu4 receptor has shown some beneficial effects in relation to Parkinson's disease but much remains to be discovered about the actions of these agents both within and outside of the basal ganglia before any clear potential is revealed.

Book excerpt: MicroPET imaging studies were conducted to investigate the role of metabotropic glutamate subtype-5 receptors (mGluR5) in Parkinson's disease (PD). Four analogical PET ligands were used to characterize modulation of mGluR5 function in a 6hydroxydopamine (6-OHDA) induced rat model of PD. Unilateral 6-OHDA lesions were made in the right medial forebrain bundle, and severity of these lesions was determined with [11]CFT. The binding characteristics of the PET ligands were analyzed using a modified distribution volume method of the Logan reference region model. Binding potential values were calculated on the striatum, hippocampus, and cortex, using the cerebellum as a reference tissue. On the right (with lesion) side of the striatum, ["C]CFT binding decreased. Three of the four investigated mGluR5 ligands ([11C]MPEP, [11C]M-PEPy, and [11C]MMPEP) also showed enhanced binding characteristics on the same side of the brain. The right hippocampus and cortex showed similar results. The mGluR5s' enhanced binding characteristics on the right side of the brain suggest a complementary and compensatory role of metabotropic glutamate receptors in the dopaminergic neurodegeneration of Parkinson's disease.

However, chronic administration of L-DOPA leads to the emergence of motor complications such as dyskinesia in the vast majority of patients. Serotonin 2A (5-HT2A) receptor blockade is a validated approach to alleviate dyskinesia, but its effectiveness appears to be of limited magnitude. Recently, we have demonstrated that activation of metabotropic glutamate 2 (mGlu2) receptors reduce dyskinesia in the 6-hydroxydopamine (6-OHDA)-lesioned rat. Based on the fact that 5-HT2A and mGlu2 receptors form a functional hetero-complex, we hypothesised that combining EMD-281,014, the most selective 5-HT2A antagonist commercially available with LY-354,740, a selective mGlu2 orthosteric agonist (OA) and LY-487,379, the most selective mGlu2 positive allosteric modulator (PAM) commercially available, would be more effective at alleviating dyskinesia than modulating 5-HT2A and mGlu2 receptors separately.Rats were rendered hemi-parkinsonian by stereotaxic injection of 6-OHDA into the medial forebrain bundle. Following a recovery period, degree of parkinsonism was assessed using the cylinder test. Rats were then primed with daily administration of L-DOPA to induce stable axial, limb and oro-lingual (ALO) abnormal involuntary movements (AIMs). On experimental days, rats were administered L-DOPA in combination with previously determined effective doses of each of EMD-281,014 (vehicle, 0.03 and 0.1 mg/kg s.c), LY-354,740 (vehicle and 0.1 mg/kg s.c.), and LY-487,379 (vehicle and 0.1 mg/kg s.c.) after which ALO AIMs were assessed for 2 min, every 20 min, for 180 min. After a 3-day washout period, an acute low-dose of L-DOPA was administered with a combination of EMD-281,014/LY-354,740/LY-487,379, and the effect on L-DOPA anti-parkinsonian action was determined by the cylinder test.Combining EMD-281,014 0.1 mg/kg and LY-487,379 0.1mg/kg to L-DOPA significantly reduced ALO AIMs duration, by 10% (P ˂ 0.05), when compared to the vehicle. Other combinations, notably EMD-281,014 and LY-354,740, or EMD-281,014 with both LY-487,379 and LY-354,740, did not lead to an enhanced anti-dyskinetic effect. Combining EMD-281,014 with LY-354,740 and LY-487,379 did not have any effect on L-DOPA anti-parkinsonian action.Our results suggest that an additional anti-dyskinetic benefit was achieved when combining EMD-281,014 and LY-487,379, but not when combining EMD-281,014 with an mGlu2 OA or with an OA and a PAM, in the 6-OHDA-lesioned rat model of PD. Overactivity of the glutamatergic subthalamonigral pathway may underlie this continual degeneration of the nigrostriatal system. With this in mind, this thesis examined whether selective activation of group III metabotropic glutamate receptor subtypes may offer a novel strategy to halt persistent degeneration in PD. -- Initial distribution studies revealed mGlu4 and 7 group III mGlu receptor subtypes, demonstrated particularly intense immunoreactivity in the SNpc, suggesting these receptors may be ideally positioned to provide neuroprotective effects. Therefore, the first objective was to confirm this neuroprotective possibility using a broad spectrum agonist, L-AP4. Sub-chronic supranigral L-AP4 treatment mediated functional neuroprotection against a unilateral 6-OHDA lesion of the SN, confirmed by behavioural assessment and post-mortem analyses. -- Secondly, the pharmacological identity of the group III mGlu receptor mediating this protective effect was examined. To investigate mGlu4 receptors, the novel mGlu4 selective PAM VU0155041, was also shown to provide functional neuroprotection in the 6-OHDA rat model to an almost comparable level reached with L-AP4. Whilst these neuroprotective effects are likely mediated by an inhibition of glutamate to protect from glutamate-mediated excitotoxicity, VUO15504 also led to a significant reduction in levels of GFAP and IBA-1 suggesting an additional anti-inflammatory action. Further studies revealed little evidence for co-localisation of mGlu4 receptors with GFAP in the SN suggesting this anti-inflammatory component likely reflects an indirect effect via stimulation of neuronal mGlu4 receptors.

We found that group III mGluRs are presynaptic on striatal-pallidal terminals and that they mediate a reduction in IPSC amplitude in the SNr. Group III mGluRs also presynaptically inhibit EPSCs at the STN-SNr synapse. Group II mGluRs were found to mediate a presynaptic reduction of EPSCs in the SNr and group II agonist LY354740 reverses catalepsy in an animal model of PD. Furthermore, as these studies progress, the distribution and physiological roles of mGluRs in the BG raises the possibility that these receptors may provide targets for novel therapeutic agents for treatment of PD and related disorders.

Furthermore, comprehensive characterization of the roles of mGluRs in the basal ganglia raises the possibility that they may provide targets for novel therapeutic agents for treatment of PD.
