However, fluoride can also enter public water systems from natural sources, including runoff from the weathering of fluoride-containing rocks and soils and leaching from soil into groundwater. Fluoride pollution from various industrial emissions can also contaminate water supplies. In a few areas of the United States fluoride concentrations in water are much higher than normal, mostly from natural sources. Fluoride is one of the drinking water contaminants regulated by the U.S. Environmental Protection Agency (EPA) because it can occur at these toxic levels. In 1986, the EPA established a maximum allowable concentration for fluoride in drinking water of 4 milligrams per liter, a guideline designed to prevent the public from being exposed to harmful levels of fluoride. Fluoride in Drinking Water reviews research on various health effects from exposure to fluoride, including studies conducted in the last 10 years.

The first volume of Dietary Reference Intakes includes calcium, phosphorus, magnesium, vitamin D, and fluoride. The second book in the series presents information about thiamin, riboflavin, niacin, vitamin B6, folate, vitamin B12, pantothenic acid, biotin, and choline. Based on analysis of nutrient metabolism in humans and data on intakes in the U.S. population, the committee recommends intakes for each age groupâ€"from the first days of life through childhood, sexual maturity, midlife, and the later years. Recommendations for pregnancy and lactation also are made, and the book identifies when intake of a nutrient may be too much. Representing a new paradigm for the nutrition community, Dietary Reference Intakes encompasses: Estimated Average Requirements (EARs). These are used to set Recommended Dietary Allowances. Recommended Dietary Allowances (RDAs). Intakes that meet the RDA are likely to meet the nutrient requirement of nearly all individuals in a life-stage and gender group. Adequate Intakes (AIs). These are used instead of RDAs when an EAR cannot be calculated. Both the RDA and the AI may be used as goals for individual intake. Tolerable Upper Intake Levels (ULs). Intakes below the UL are unlikely to pose risks of adverse health effects in healthy people. This new framework encompasses both essential nutrients and other food components thought to pay a role in health, such as dietary fiber. It incorporates functional endpoints and examines the relationship between dose and response in determining adequacy and the hazards of excess intake for each nutrient.

In response to these findings, community water fluoridation programs were developed to add fluoride to drinking water to reach an optimal level for preventing tooth decay, while limiting the chance of developing dental fluorosis. By the 1980s, studies in selected U.S. communities reported an increase in dental fluorosis, paralleling the expansion of water fluoridation and the increased availability of other sources of ingested fluoride, such as fluoride toothpaste (if swallowed) and fluoride supplements. This report describes the prevalence of dental fluorosis in the United States and changes in the prevalence and severity of dental fluorosis among adolescents between 1986-1987 and 1999-2004. Prior toW orld War II this question simply did not arise: there was not sufficient interest or literature in the field to warrant anything beyond the classical monograph pub 1 lished by KAJ RoHOLM in 1937 • RoHOLM's work was directed chiefly toward a better understanding of the effects of fluorides on the general health of workers in the cyrolite industry. However, with the demonstration that water-borne fluoride was a causative agent of both mottled enamel and increased resistance to dental caries, the ground work was laid in the 1930's and early 1940's for a greatly increased interest in the biological effects of fluorides in human beings. During this time and earlier for that matter, work also had been going steadily ahead in the less spectacular area of effects produced in poultry and livestock when fluorine-containing rock phosphate was incorporated in the ration, and when pasture land was contami nated with fluorides released during the large-scale conversion of rock phosphate to fertilizer and phosphoric acid. These latter aspects of the problem had led to the development of a respectable literature in plant physiology, dealing with the effects of fluoride on vegetation.

Also referred to as tooth decay, this disease is one of the most common disorders throughout the world, second only to the common cold. Dental caries is the most common chronic childhood disease in the United States and is 5 to 7 times more common than asthma. According to the World Oral Health Report in 2003, dental caries affect 60-80% of school children and a vast majority of adults. Dental caries is a chronic bacterial infection of the hard tissue of the tooth that is characterized by alternating phases of demineralization and remineralization. Dental decay can lead to significant pain and dysfunction that can interfere with basic functions such as eating, sleeping, and speaking. If left untreated, dental caries can result in cavities forming and, eventually, tooth loss. Although the prevalence and severity of dental caries has decreased over the years, this disease can be controlled better with proper fluoride exposure. Fluoride dentifrices have been shown in numerous clinical trials to be effective anticaries agents and have been recognized as a major cause of the remarkable decline in caries prevalence in many developed countries. Dentifrices have been widely adopted around the world as the principle means of delivering topical fluoride and obtaining caries preventive benefits. Although there is documented literature on the use of topical fluorides, the issue needs to be further researched based on the recent documented literature and guidelines regarding use of topical fluorides. Hence, the present review was conducted with the aim to review the available literature on the use and effectiveness of different topical fluorides used in dentistry.
