Book excerpt: The purpose of this qualitative research project is to determine whether student athletic trainers will reduce the impact of injuries in high school sports. Existing literature shows that high school sports injuries are a critical problem; therefore practical and economical solutions are needed. A rural high school setting was studied using a variety of research tools including questionnaires, interviews, and direct observations. Questionnaires were administered to studetns in Health Careers classes and to high school coaches. Interviews were administered to students interested in becoming student athletic trainers, physical therapists in the community, and parents. Direct observation was conducted at various high school athletic events. Findings indicate that student athletic trainers may help to reduce high school sports injuries and help to provide coaches more time to coach. Additionally, most subjects agreed that programs would allow student athletic trainers to take their education and experiences to a higher level in fitness careers. Future studies are needed to implement and assess student athletic trainer programs in order to measure their success.

From being captain for the varsity team of the trainers, to staying in the program, trainers work extremely hard to ensure that athletes receive maximum support and care while playing and performing their best. Trainers are the group of people that come to the field before the players arrive and are the last ones to leave the ground. During any game or practice session, student athletic trainers [AM1] are the first people who rush to the field to provide medical aid to the injured athlete. The role and responsibilities of the trainers demands a lot of hard work, dedication, and sacrifice: rain or shine, they are there. It requires lots of mental preparedness and physical strength. Being a student athletic trainer is not an easy job. As someone who has the experience of being a student athletic trainer in High School, I understand the incredible importance that this role plays and the challenges that come with it. What started as a high school program, student athletic training has become a passion within me that keeps growing with each passing day and, with every step that I take on the field. Through this book, I want to capture and share my innate thoughts and my hands-on experience as a student trainer. I want to provide support and guidance to those who choose to pursue this rewarding and fulfilling program that offers multiple opportunities to make a positive impact on the lives of student -athletes, both on and off the field. Factors which play into this inequity in the general population, such as low socioeconomic status and being located in a rural area, also affect access to athletic training services in the high school and secondary school setting. An athletic trainer can provide timely and appropriate medical care to an athlete after experiencing a sports-related injury or illness at no additional cost to the athlete or their family. Having access to athletic trainers generally indicates that a school is better prepared to handle an emergency injury situation before such an injury occurs and that injuries are recognized and treated in a timely and appropriate manner. Methods: A systematic literature review was performed using keywords "athletic trainer", "high school or secondary school", and "disparity or access". Inclusion criteria: peer-reviewed articles, published from 2005-2020 in the English language with a setting of high schools or secondary schools. Exclusion criteria: articles not pertaining to the target population, conference proceedings, duplicate articles. Articles categorized by level of socioecological model to which they pertained. Results: 25 articles were found. Major themes of the articles included general access to athletic trainers (n=11), sports-related concussions (n=9), exertional heat stroke (n=2), and emergency action plans (n=3). Discussion: Access to athletic training services has positive impacts on the implementation of emergency action plans, preparedness for heat illnesses, and recognition and management of sports-related concussions. This set of articles also supports that athletic training services are less likely to be available in lower socioeconomic areas, rural areas, and in private schools. All levels of the socioeconomic model were represented in this search, though with policy level research the most lacking.

While injury prevention strategies (IPSs) are identified and studied, they often focus on specific types of injuries, a given injury prevention activity or set of activities, and implemented in controlled settings by health professionals or sport coaches. Studies involving the implementation of various IPSs in real-world sports settings are lacking. Organized strength and conditioning (S&C) programs offered by certified strength and conditioning specialists (CSCS) provide the perfect opportunity to explore the implementation of IPSs in a real-world setting. A gap remains in the literature on the implementation of IPSs through the provision of an organized S&C program by a CSCS lending to a reduction of injuries in high school sports. The purpose of this qualitative study is to explore the actions, goals, and perceptions of CSCS coaches as they relate to injury prevention when providing organized strength and conditioning programs in the high school sports setting. Method: A convenience sample of 10 high school CSCSs were recruited, interviewed, and asked to provide copies of their strength and conditioning programs subsequently, 8 subjects were included in the final analyses as two interviews were lost due to technical difficulties. The research questions focused on aspects integral to strength and conditioning and injury prevention. This study utilized grounded theory to examine the implementation of organized strength and conditioning programs and potential impact on injury rates. Results: Analysis of the data-set indentified four major themes: 1) coaches consistently emphasized the need to focus on motor skill competency, 2) the individual needs of youth athletes matter, 3) injury prevention exercise program strategies are an innate part of high school strength and conditioning programs, and 4) contextual factors may affect the impact an organized S&C program will have on the reduction of injury rates in high schol sports athletes. These themes provide evidence that a more holistic approach exists to the implementation of IPSs through the provision of organized S&C program by CSCS that could lend to the reduction of injuries in the high school sport context.

This report recommends actions that can be taken by a range of audiences - including research funding agencies, legislatures, state and school superintendents and athletic directors, military organizations, and equipment manufacturers, as well as youth who participate in sports and their parents - to improve what is known about concussions and to reduce their occurrence. Sports-Related Concussions in Youth finds that while some studies provide useful information, much remains unknown about the extent of concussions in youth; how to diagnose, manage, and prevent concussions; and the short- and long-term consequences of concussions as well as repetitive head impacts that do not result in concussion symptoms. The culture of sports negatively influences athletes' self-reporting of concussion symptoms and their adherence to return-to-play guidance. Athletes, their teammates, and, in some cases, coaches and parents may not fully appreciate the health threats posed by concussions. Similarly, military recruits are immersed in a culture that includes devotion to duty and service before self, and the critical nature of concussions may often go unheeded. According to Sports-Related Concussions in Youth, if the youth sports community can adopt the belief that concussions are serious injuries and emphasize care for players with concussions until they are fully recovered, then the culture in which these athletes perform and compete will become much safer. Improving understanding of the extent, causes, effects, and prevention of sports-related concussions is vitally important for the health and well-being of youth athletes. Book excerpt: Youth sports have demonstrated the ability to improve short- and long-term health benefits such as improved physical, mental, and social health. However, one current trend in the American youth sport culture that may impact these benefits is youth sport specialization. Sport specialization has been widely studied in the high school population, but little information is known about the middle school population. There is reason to believe that sport specialization affects sport dropout and physical activity, and we know that middle school athletes are at a great risk for quitting sport and for decreases in physical activity as they transition to high school. Therefore, there is a need to understand how sport specialization affects sport motivation, physical activity, and how overuse injuries are being treated in this population. The primary purposes of this dissertation were 1) to determine the association of sport specialization with motivation metrics in middle school athletes, 2) to explore the association of physical activity measurements with sport specialization in middle school athletes, and 3) to describe the confidence, knowledge, and barriers athletic trainers have in treating adolescent overuse injuries. Study 1: 178 middle school athletes completed an anonymous questionnaire. The questionnaire contained demographic questions, sport participation and specialization questions, and the Youth Behavioral Regulation in Sport Questionnaire. Overall, sport specialization was not associated with different types of motivation. However, multisport athletes were more likely to have higher intrinsic motivation compared to single sport athletes. Study 2: 37 middle school athletes wore an accelerometer for at least 5 days and completed a self-reported log of physical activity. Several other factors were recorded such as household family income, parent physical activity, grade, sex, and sport specialization level. Physical activity measurements were not statistically significant between sport specialization groups. It is important to note that highly specialized athletes on average had higher levels of moderate to vigorous physical activity than low and moderately specialized athletes. Study 3: 430 athletic trainers in the secondary school setting completed an anonymous survey on their confidence, knowledge, and barriers to treating overuse injuries in adolescent athletes. Athletic trainers were extremely confident in their knowledge of treating overuse injuries but were less confident in their ability to execute their treatment plan. 82% of athletic trainers cited that a patient's reluctance to reduce sport activities was a "moderate" or "extreme" barrier to treating adolescent overuse injuries. About 30%-35% felt "very" to "extremely" knowledgeable about their ability to diagnose and treat these types of injuries. Most athletic trainers recognized that growth spurts affected sport-related injury risk; however, only 37% of athletic trainers felt confident in their ability to determine if an adolescent athlete was going through a growth spurt.
