Book excerpt: "Despite the availability of healthcare resources and information, there continues to be significant disparities in stroke incidence and mortality among the African-American population. The Centers for Disease Control and Prevention ([CDC], 2017) reports that stroke costs the United States approximately $60 billion per year; affecting more than 795,000 Americans per year. African-Americans are twice as likely to die from a stroke; and the rate of first strokes are doubled that of Caucasians (National Stroke Association, 2017). Therefore, an evidenced-based practice (EBP) change project consisting of a culturally-tailored educational intervention was implemented in a predominately African-American church. The purpose of the EBP change project was to increase stroke knowledge and awareness among the African-American population. Providing health education within a religious organization, has been shown to improve the health status of the African-American population. The EBP change project included 87 participants, of which 66% were African-American. The Revised Stroke Knowledge Questionnaire was used to assess pre-test and post-test stroke knowledge. The intervention was found to have a positive impact by increasing group mean knowledge scores by 36 to 40%. This increase in aggregate group mean scores exceeded the set benchmark of 12%. African Americans present a risk for first time stroke which is twice as high as Caucasians. Literature has shown the primary cause of increased incidence of stroke in African Americans is racial disparities related to health promotion among African Americans. This descriptive correlational study investigated the relationships in health promotion behaviors and stroke in African American men and women. Utilizing the Health Promotion Model as the theoretical framework, current health promoting behaviors and practices related to stroke were examined in a convenience sample of 24 African American men and women located at a church in the Southeastern United States. Stroke risks and health promotion were measured by Sullivan's Cerebrovascular Attitudes and Beliefs Scale-Revised. The goal is to increase awareness of stroke risk factors and promote healthy behaviors in African Americans to decrease mortality and racial disparity from stroke. The results of this study may increase healthcare professionals' knowledge and awareness of the importance of exploring health promotion behaviors related to stroke among African Americans.

Many strokes can be prevented through the recognition and control of risk factors. Stroke awareness includes having knowledge of risk factors and understanding how to control the risk. Additionally, recognition of stroke warning signs and symptoms is essential to early intervention, treatment, and reduction of disability and death. The purpose of this study was to examine African American women's knowledge of stroke risk factors and warning signs. A descriptive quantitative survey design was utilized to answer the study question. A convenience sample (n= 33) of African American women from a suburban town in South Carolina, a county located in the southeastern region of the United States, was included in this study. Leventhal's Common Sense Model was utilized as the theoretical framework to determine the ability of persons to appraise their stroke risk and to establish coping methods for the prevention or treatment seeking behaviors. The results of the study may provide a better understanding of the educational needs of minority groups specifically those of women and racial/ethnic groups.

Book excerpt: Objectives: The objective of this study is to estimate the impact of biological, psychosocial and other risk factors on stroke in African Americans enrolled in the JHS and determine the population attributable risk of risk factors. Methods: This study used a nested case control design. Cases were patients with stroke during the follow up period. Corresponding controls were selected from subjects who are alive at that time period and who are at risk of stroke by matching on the person-years spent in the cohort. Odds ratios (OR) and population attributable risks (PAR) were calculated to understand the association of risk factors with incident stroke. Results: A total of 129 cases of incident stroke were identified during the follow-up and 590 controls were identified. Hypertension and diabetes were the two strongest risk factors of stroke, with an OR of 1.8 (95% CI 1.1 - 3.1) for hypertension and an OR of 1.7 (95% CI 1.1 - 2.7) for diabetes, followed by the Framingham stroke risk score. The risk of stroke increased by 4% for every 1% increase in the Framingham stroke risk score (OR 1.04, 95% CI 1.01 - 1.08). Family history of stroke and alcohol consumption increased the risk of stroke, but the association was not significant. We found that psychosocial risk factors including depression, stress and major life events were not significantly associated with incident stroke in this study. The overall PAR including both biological and psychosocial risk factors accounted for 77.5% of incident stroke in our study. Conclusions: In conclusion, the six biological risk factors together accounted for 76.5% of PAR in the African Americans in the JHS. The association of psychosocial risk factors and incident stroke is unclear and further studies are necessary.

Book excerpt: The burden of stroke is well understood for whites, blacks and Hispanics in the United States, yet its nature, outcome, and medical care profiles have not been examined for other immigrant groups, including South Asians. Previous studies on gender differences document higher stroke incidence rates for men but worse outcomes for women. Factors related to stroke hospitalization have been shown to include failure to identify symptoms of stoke onset, delay in emergency department arrival or extended waiting time, and lack of insurance coverage. This dissertation asks whether these results can be further understood by examining stroke data among a multiracial/multiethnic cohort. Specifically it asks: (1) whether racial/ethnic-specific risk factors allow the prediction of subtypes of stroke; that is, large versus small vessel stroke, (2) whether total cholesterol and triglyceride levels can be used as predictors of risk for stroke between genders of the different races/ethnicities, and (3) whether access to emergency care and other health services for stroke differ by race/ethnicity and by stroke type. Stroke registry data were obtained from the New Jersey Neuroscience Institute, John F. Kennedy Medical Center. Data on 3,290 subjects who met the inclusion criteria were examined for socio-demographic, stroke risk, clinical and health profile variables. Results showed that South Asians were younger than whites at the time of admission for acute stroke. Gender comparisons revealed that South Asian and Hispanic men had higher proportions of patients with ischemic stroke than women, while the inverse was true for whites and African Americans. Twice as many young patients (18-44 years) suffered hemorrhagic stroke compared to ischemic stroke. Important predictive factors for stroke outcome included insurance status and use of emergency medical services as a mode of transport to hospitals following stroke symptom recognition. Variations in stroke incidence and outcomes may be partially explained by dietary and life style choices; however consideration must be given to competing morbidity, failure to recognize warning signs for stroke, and failure to use emergency services due to lack of knowledge, language barriers, or lack of insurance. These factors need to be more clearly examined in future studies.
