The resulting case studies, of 52 individual countries and five regional groupings, cover 152 countries or territories, or three-quarters of the world’s nations. Each chapter author was asked to think 5–15 years into the future and make a prediction on how their health system could be strengthened as a result of the successful unfolding of their case study. The types of projects our authors have chosen to explicate into the future are wide-ranging. They vary from e-consultation services in Estonia, achieving universal health coverage in Argentina and Mexico, reforming long-term care in the Netherlands, reassessing care for the aging population and the frail elderly in Australia, streamlining the health system through Lean Thinking in Nigeria, using regulation to improve care in South Africa, developing a new accreditation model in Turkey, through to a critique of physician specialization in Russia and applying IT initiatives to improve care in China, Lebanon, Taiwan, Papua New Guinea, the United Arab Emirates, Venezuela and Wales. Chapter writers recognized that the improvement work they were doing was part of a moving target. There was general agreement that the effective use of limited resources and overcoming hurdles and constraints were crucial to enhancing health systems in order to deliver better care over the medium term. While some initiatives required considerable funding, many were relatively inexpensive. These case studies demonstrate ways in which fruitful application of partnerships and creativity can make considerable gains in strengthening healthcare delivery systems. Book excerpt: From the early 2000s, a new discourse emerged, in Africa and the international donor community, that higher education was important for development in Africa. Within this zeitgeist of converging interests, a range of agencies agreed that a different, collaborative approach to linking higher education to development was necessary. This led to the establishment of the Higher Education Research and Advocacy Network in Africa (Herana) to concentrate on research and advocacy about the possible role and contribution of universities to development in Africa. This book is the final publication to emerge from the Herana project. The project has also published more than 100 articles, chapters, reports, manuals and datasets, and many presentations have been delivered to share insights gained from the work done by Herana. Given its prolific dissemination, it seems reasonable to ask whether this fourth and final publication will offer the reader anything new. This book is certainly different from previous publications in several respects. First, it is the only book to include an analysis of eight African universities based on the full 15 years of empirical data collected by the project. Second, previous books and reports were published mid-project. This book has benefited from an extended gestation period allowing the authors and contributors to reflect on the project without the distractions associated with managing and participating in a large-scale project. For the first time, some of those who have been involved in Herana since its inception have had the opportunity to at least make an attempt to see part of the wood for the trees. Different does not necessarily mean new. An emphasis on the newness of the data and perspectives presented in this book is important because it shows that it is more than a historical record of a donor-funded project. Rather, each chapter in this book brings, to a lesser or greater extent, something new to our understanding of universities, research and development in Africa.

This book assesses spatial planning in the second-tier cities of the country. Secondary cities are vital as they perform essential regional, and in some cases, global economic roles and help to distribute the population of a country more evenly across its surface. Apartheid planning left South African cities fragmented segregated and with low densities. Post-apartheid policies aim to reverse these realities by emphasising integration, higher densities and upgrading. Achieving these aims has been challenging and often the historical patterns continue. The evidence shows that two opposing patterns prevail, namely increased densities and continued urban sprawl. This book presents ten case studies of spatial planning and spatial transformation in secondary cities of South Africa. The book frames these case studies against complexity theory and suggests that the post-apartheid response to apartheid planning represents a linear deviation from history. The ten case studies then reveal how difficult it is for local decision-makers to find appropriate responses and how current responses often result in contradictory results. Often these cities are highly vulnerable and they find it difficult to plan in the context of uncertainty. The book also highlights how these cities find it difficult to stand on their own against the influence of interest groups (property developers, mining companies, traditional authorities, other spheres of government). The main reasons include weak municipal finance statements, the dependence on national and provincial government for capital expenditure, limited investment in infrastructure maintenance, the lack of planning capacity, the inability to implement plans and the unintended and sometimes contrary outcomes of post-apartheid planning policies.

A significant State-driven restructuring from around 1997 to 2005 resulted in what is largely the current configuration of public universities. But just over two decades later, for a variety of reasons, the higher education sector in South Africa appears beset with numerous challenges. Nelson Mandela University is one of the public universities that emerged from the restructuring process. The university is in an ongoing state of evolution, of becoming. It developed out of the amalgamation of the University of Port Elizabeth, Port Elizabeth Technikon and incorporation of the Port Elizabeth campus of Vista University as Nelson Mandela Metropolitan University in 2005. In 2017, it was renamed Nelson Mandela University, after the world-renowned statesman, rather than the metropolitan area in which the university is primarily located. The renaming was conceptualised as more than a marketing opportunity to rebrand the university, but as an opportunity to reorientate the university, to reposition Nelson Mandela University as an engaged and socially-embedded university in the service of society, striving to be the academic expression of the values and ethos of its iconic namesake. Endeavouring to be something greater and different from the norm imbues its strategy, public statements and practices. The determination to ‘achieve Mandela University’ serves, or is intended to serve, as both an organising principle and a lodestar. A cross-section of writers from different backgrounds situates Nelson Mandela University within the contemporary historical moment from which it emerged and examines its subsequent evolution. While Nelson Mandela University has performed the usual work expected of any university, it has also sought to turn the university outwards, to achieve a higher purpose, framing itself as a values-based university on a journey to become something else. In Achieving Nelson Mandela University? the university attempts to give an account of itself. The book is an intellectual and scholarly reflection on where the university has come from and where it is seeking to go.

Book excerpt: In Uganda, conditions in reproductive, maternal, newborn, child, and adolescent health (RMNCAH) remain the primary drivers of morbidity and mortality, accounting for 60 percent of years of life lost. The high burden of these conditions can be attributed to a poor quality of care resulting from inadequate financial, human, and material resources compounded by weak multisectoral coordination. Moreover, the country's high population growth rate and a young population imply that RMNCAH service delivery will continue to dominate health sector reforms--even with the increasing prevalence of noncommunicable and infectious diseases. Over the past two decades, Uganda has focused on improving the quality of RMNCAH service delivery, leading to declines in the maternal, infant, and under-five mortality ratios and the increased use of modern contraception among married women. However, the neonatal mortality and teenage pregnancy rates have stagnated, and the low civil registration of births and deaths remains challenging. Investing in Reproductive, Maternal, Newborn, Child, and Adolescent Health in Uganda: What Have We Learned, and Where Do We Go from Here? comprises 12 studies conducted as part of the RMNCAH Operational Research Program drafted between 2019 and 2021 and finalized and disseminated in October 2022 across 45 districts of Uganda with funding from Sweden and the World Bank. These studies underscore important lessons learned and offer suggestions for enhancing the delivery of RMNCAH interventions. Each chapter represents one study and discusses service delivery, the health workforce, financing, health information systems, and governance and leadership. Two appendixes summarize key findings and recommendations and explain the roles of key stakeholders in the RMNCAH Operational Research Program.
