Book excerpt: Contexte : Sensibiliser les étudiants du deuxième cycle des études médicales (DCEM) à la médecine générale était au départ une volonté des diverses facultés de médecine puis est devenue une volonté politique. L’arrêté 26 du 25 Novembre 2006 instaure un stage chez le médecin généraliste et illustre cette volonté politique. La Faculté de médecine d’Angers l’a mis en place depuis septembre 2005.Objectifs : L’objectif de ce travail est triple : tout d’abord évaluer les acquis cliniques des étudiants, puis évaluer l’impact de ce stage sur leur choix de spécialité à l’ECN et enfin juger de l’influence de ce stage sur leur définition de la médecine générale.Méthode : Pour ce travail nous avons exploité statistiquement 61 réponses à un questionnaire auprès d’étudiants ayant fait ce stage mais aussi analysé l’influence du stage sur le choix de spécialités des 188 étudiants angevins s’étant présentés aux ECN 2007 et 2008 et enfin nous avons réalisé deux focus group auprès d’étudiants réalisant leur stage d’avril à juin 2008, l’un avant le stage l’autre après le stage.Résultats : La définition de la médecine générale par les étudiants correspond à celle de la WONCA Europe. Certaines notions telles que le suivi sont mieux cernées. Ce stage participe à leur formation clinique. Enfin il influe sur leur choix à l’ECN.Conclusion : Les étudiants apprécient ce stage et demandent beaucoup plus d’autonomie qu’il n’en est prévu actuellement. On note quelques différences d’apprentissage selon l’année universitaire de l’étudiant et selon son sexe mais dans l’ensemble on peut avoir une même approche pédagogique de l’étudiant à condition d’évaluer ses compétences en début de stage.

L’état sanitaire de la population du Nord Pas de Calais est moins bon que celui du reste de la France métropolitaine, et engendre une demande de soin plus importante. La démographie des médecins généralistes du Nord Pas de Calais n’est pas adaptée à cette demande de soin plus importante. L’étude montre l’intérêt porté par les médecins en exercice et les étudiants à la création d’un stage en médecine générale durant le second cycle des études médicales. Elle vise à mieux faire connaître aux étudiants cette discipline pour les inciter à un choix en médecine générale après l’examen classant national et à son exercice. L’aide du Conseil Régional a été sollicitée en raison de ses domaines de compétence dans la formation professionnelle et de la santé. Les résultats de l’étude montrent que les étudiants et les maîtres de stage sont très favorables à la mise en place de ce stage. Les problèmes du financement de la formation des maîtres de stage et du coût du déplacement des étudiants ou de leur hébergement sont majeurs, mettant en péril ce projet. C’est là que la réponse de l’institution régionale est attendue. La mise en place de ce stage est un des éléments possibles du renforcement de la densité médicale insuffisante de la région Nord Pas de Calais. Le projet a été présenté au Conseil Régional et est actuellement en attente de décision pour 2008.
