Book excerpt: It is well known that behavior problems are a salient characteristic of children and adults with mental retardation. That is not to say that all persons with mental retardation experience behavior disorders; how ever, most studies indicate that the incidence of emotional disturbance in this population is four to six times greater than that observed in similar intellectually nonhandicapped children and adults. It is equally well known that the principal form of treatment accorded clients with mental retardation and behavior disorders is pharmacotherapy or the prescrip tion of behavior modifying drugs. Recent studies show that 6 out of every 10 individuals with mental retardation have been prescribed drugs as treatment for disorders of emotion or behavior. Unfortunately, further studies indicate that only one or 2 out of every 10 clients receiving medication are determined to be "responders," such that some thera peutic benefit is derived from their drug treatment. As noted by the title, the single major thrust of this volume is to review approaches to the treatment of behavior disorders in persons with mental retardation from a nondrug perspective. This requires the presentation of a wide range of material on treatment: basic behavior modification programming, cognitive-behavioral strategies, habilitative approaches, counseling and psychotherapy, designing therapeutic living environments, managing medical factors bearing relevance to emotional illness, intervening with families, training special education teachers and direct care staff, and supplying information on the client's rights to obtain treatment in the least restrictive and least intrusive manner.

Mental retardation, a condition characterized by deficits in intellectual capabilities and adaptive behavior, can be particularly hard to diagnose in the mild range of the disability. The U.S. Social Security Administration (SSA) provides income support and medical benefits to individuals with cognitive limitations who experience significant problems in their ability to perform work and may therefore be in need of governmental support. Addressing the concern that SSA's current procedures are consistent with current scientific and professional practices, this book evaluates the process used by SSA to determine eligibility for these benefits. It examines the adequacy of the SSA definition of mental retardation and its current procedures for assessing intellectual capabilities, discusses adaptive behavior and its assessment, advises on ways to combine intellectual and adaptive assessment to provide a complete profile of an individual's capabilities, and clarifies ways to differentiate mental retardation from other conditions.

Book excerpt: This eight-year study of an American city traces the answer to the question "Who is retarded?" by analyzing the labeling process in a large number of community agencies. Data for the study are drawn from a representative sample of 7,000 persons under fifty years of age who were tested ans screened for "symptoms" of mental retardation. The author finds that that schools label more persons as mentally retarded than any other agency and share their labels more widely with others in the community. Relying on IQ test scores for diagnosis, schools place many persons with scores above 70 and with no physical disabilities in the role of retardate. The author contends that both the statistical model of "normal" and the unicultural viewpoint of educators and clinicians work to the disadvantage of the poor and the ethnic minorities. Given the opportunity, many persons demonstrate by their ability to cope with the problems in other areas of life that they are not comprehensively incompetent. The author makes serval policy recommendations. First, she suggests lowering the IQ score cutoff point used by schools in determining who shall be labeled as retarded. Second, she recommends that the clinicians use the two-dimensional definition of retardation proposed by the American Association of Mental Deficiency, subnormality in both intellectual performance and adaptive behavior. Third, she concludes that pluralistic assessment procedures must be employed to take into account cultural biases in IQ tests designed to measure cognitive skills. Book excerpt: Children living in poverty are more likely to have mental health problems, and their conditions are more likely to be severe. Of the approximately 1.3 million children who were recipients of Supplemental Security Income (SSI) disability benefits in 2013, about 50% were disabled primarily due to a mental disorder. An increase in the number of children who are recipients of SSI benefits due to mental disorders has been observed through several decades of the program beginning in 1985 and continuing through 2010. Nevertheless, less than 1% of children in the United States are recipients of SSI disability benefits for a mental disorder. At the request of the Social Security Administration, Mental Disorders and Disability Among Low-Income Children compares national trends in the number of children with mental disorders with the trends in the number of children receiving benefits from the SSI program, and describes the possible factors that may contribute to any differences between the two groups. This report provides an overview of the current status of the diagnosis and treatment of mental disorders, and the levels of impairment in the U.S. population under age 18. The report focuses on 6 mental disorders, chosen due to their prevalence and the severity of disability attributed to those disorders within the SSI disability program: attention-deficit/hyperactivity disorder, oppositional defiant disorder/conduct disorder, autism spectrum disorder, intellectual disability, learning disabilities, and mood disorders. While this report is not a comprehensive discussion of these disorders, Mental Disorders and Disability Among Low-Income Children provides the best currently available information regarding demographics, diagnosis, treatment, and expectations for the disorder time course - both the natural course and under treatment.

Everybody needs to be socially competent for living a better life in society, having good relationships and interactions with others. Researchers have concluded that deficits in social competence can affect later success in life. Social competence has frequently been cited as a critical component of life adjustment (e.g., Epstein & Cullinan, 1987; Neel, 1988). In particular, the importance of social competence and related personality features has been stressed for individuals who have mental retardation or other developmental disabilities (e.g., Balla & Zigler, 1979). As a consequence, social skills instruction has increasingly been recognized as a key component to be included in intervention programs for students who are mildly mentally retarded. (Gable. A.R & Warren. F.S., 1993). The American Association on Mental Retardation (2002), defines mental retardation as “Mental retardation is disability characterized by significant limitation both in intellectual functioning and in adaptive behavior as expressed in conceptual, social and practical adaptive skills. This disability originates before age 18”. (p.1). Social skills are specific behaviors that facilitate interpersonal interactions and maintain a degree of independence in daily functioning. Social competence involves the use of those skills at the right times and places, showing social perception, cognition, and judgment of how to act in a particular situation and how to adjust one’s behavior to meet different situations (Greenspan, 1979, 1990; Kerr & Nelson, 1989; Sargent, 1989).
