Book excerpt: This report considers the biological and behavioral mechanisms that may underlie the pathogenicity of tobacco smoke. Many Surgeon General's reports have considered research findings on mechanisms in assessing the biological plausibility of associations observed in epidemiologic studies. Mechanisms of disease are important because they may provide plausibility, which is one of the guideline criteria for assessing evidence on causation. This report specifically reviews the evidence on the potential mechanisms by which smoking causes diseases and considers whether a mechanism is likely to be operative in the production of human disease by tobacco smoke. This evidence is relevant to understanding how smoking causes disease, to identifying those who may be particularly susceptible, and to assessing the potential risks of tobacco products.

Available in PDF, EPUB and Kindle. Book excerpt: This Surgeon General's report returns to the topic of the health effects of involuntary exposure to tobacco smoke. The last comprehensive review of this evidence by the Department of Health and Human Services (DHHS) was in the 1986 Surgeon General's report, The Health Consequences of Involuntary Smoking, published 20 years ago this year. This new report updates the evidence of the harmful effects of involuntary exposure to tobacco smoke. This large body of research findings is captured in an accompanying dynamic database that profiles key epidemiologic findings, and allows the evidence on health effects of exposure to tobacco smoke to be synthesized and updated (following the format of the 2004 report, The Health Consequences of Smoking). The database enables users to explore the data and studies supporting the conclusions in the report. The database is available on the Web site of the Centers for Disease Control and Prevention (CDC) at http://www.cdc.gov/tobacco.

Book excerpt: It is often suggested that the incidence of cancer and coronary heart disease could be much reduced or even eliminated if only people would stop smoking cigarettes and eat fewer high-cholesterol foods. The evidence, however, shows that such views are simplistic and unrealistic and that, instead, cancer and CHD are the product of many risk factors acting synergistically. Psychosocial factors (stress, personality) are six times as predictive as smoking, cholesterol level or blood pressure and much more responsive to prophylactic treatment. This book admits that, while smoking is a risk factor for cancer and CHD, its effects have been exaggerated. A more realistic appraisal of a very complex chain of events incorporating many diverse factors is given, and appropriate action to prevent cancer and coronary heart disease is discussed.

Book excerpt: For those addicted to nicotine, the thought of being able to quit smoking and have fun while doing it has seemed impossible—until now. "I Don't Smoke!" offers a very different approach to smoking cessation: an approach that focuses on the smoker, not the nicotine; an approach that looks at quitting as a joyous adventure; an approach that will make smokers laugh and feel good while they free themselves from their addiction; an approach that works. Dr. Joseph Cruse, founding medical director of the Betty Ford Center, applies addiction recovery techniques in this guidebook that will help every addicted smoker to announce with confidence, "I don't smoke!"—and mean it.

Book excerpt: We is a dystopian novel written by Russian writer Yevgeny Zamyatin. Originally drafted in Russian, the book could be published only abroad. It was translated into English in 1924. Even as the book won a wide readership overseas, the author's satiric depiction led to his banishment under Joseph Stalin's regime in the then USSR. The book's depiction of life under a totalitarian state influenced the other novels of the 20th century. Like Aldous Huxley's Brave New World and George Orwell's Nineteen Eighty-four, We describes a future socialist society that has turned out to be not perfect but inhuman. Orwell claimed that Brave New World must be partly derived from We, but Huxley denied this. The novel is set in the future. D-503, a spacecraft engineer, lives in the One State which assists mass surveillance. Here life is scientifically managed. There is no way of referring to people except by their given numbers. The society is run strictly by reason as the primary justification for the construct of the society. By way of formulae and equations outlined by the One State, the individual's behaviour is based on logic.

Book excerpt: The Interfaces of Perinatal Addiction Ira J ChasnofT In the last few years, problems associated with drug use in pregnancy have become endemic. As cocaine has become the drug of choice for millions of Americans, including pregnant women, as AIDS has become more commonly recognized in women and infants, and as legal cases have begun to raise the question of fetal abuse, no professional group has come forward to serve as advocate for this special population of substance abusers. Meanwhile, however, physicians, nurses, social service agencies and public health officials have all been faced with increasing numbers of infants showing the detrimental effects of their mothers' drug use. Although problems of substance abuse in pregnancy have received increasing attention in the medical literature since the early 1970s, there has recently been a very rapid increase in the number of articles published related to this field. The reasons for this new interest are easily understood when current statistics from the National Institute on Drug Abuse are reviewed 1. Although patterns of abuse of alcohol, marijuana, heroin and other substances by women of childbearing age have changed very little over the last ten years, the incidence of cocaine use in this special population has been rising rapidly, a reflection of cocaine's increasing popularity among the general population of the United States.
