The European and the American Societies more than any other organizations launched the idea, and the success of the European Society's International Meetings gave impetus to a growth of interest from a handful of specialists to the wide interdisciplinary study which microcirculation now represents. The meeting held in Canada in June 1975 was, however, the first truly international meeting devoted to the microcirculation. It, too, was a success from every point of view, and the exchange of knowledge and new ideas was rewarding. It is our present hope that the tradition of European meetings with their characteristic European flavor will continue, but larded by larger, international congresses conceived on a worldwide basis. For the present conference we were fortunate in the presence of Dr. B. Zweifach. He was once referred to as the "father of the microcircula tion." This claim, unfortunately, I cannot accept. That honor probably belongs to Harvey, who by one of the most brilliant strokes of inductive reasoning in medical history inferred the existence of capillaries though he could not see them. Ben Zweifach's role was rather that of the midwife, presiding at the birth rather than the conception. The baby he delivered long years ago has since thriven lustily and its growth is in no small measure due to the continuing zeal of Zweifach and his associates.

Book excerpt: Throughout the centuries, inflammation has been considered as a disease in itself. This misconception arose from the inability to distinguish between inflammatory changes and the insults which induce them. The understanding of the distinction between the genesis of inflammation and the tissue reactions that follow is attributed to JOHN HUNTER, who, at the end of the 18th century, substantially contributed to the analysis of inflammation in objective terms. Today, however, we are still trying to find explanations for Celsus' Signs in terms of structural and functional changes occurring in the inflamed tissue. There are drugs which modulate these signs but, without a detailed knowledge of the basic physiopathological events, it is impossible to understand their mechanism of action. Notwithstanding, the effects of anti inflammatory drugs provided new knowledge of the relevance of the signs and symptoms to the sequence of biochemical and morphological changes occurring in inflammation. When we accepted the invitation to edit a Handbook on Inflammation and Anti Inflammatory Drugs, we were aware of the magnitude of the task. We knew the impossibility of covering the whole field in detail, especially taking into account the rapid accumulation of experimental knowledge which would, in all likelihood, overtake the process of publication.

The concept is not new; however, its application has been limited because of lack of interest and willingness to make the extra effort required. Also there has been concern about the safety of the procedure. Homologous blood transfusion is easy and has allo wed us to salvage lives and perform procedures which would not have been feasible without the availability of whole blood or its components. Transfusion with homologous blood products is safe; however, com plications can and do occur. The transmission of AIDS by transfusion of homologous blood products has significantly increased patient and physician awareness of the dangers one faces. This potential complica tion has promoted autotransfusion. It is now commonly used and reques ted by patients and physicians. Blood for autotransfusion is collected pre or intraoperatively by phle botomy and retrieved intra and post operatively from sites of hemor rhage. Once collected, the blood is either stored or returned to the patient as whole or processed blood. To have a successful autotransfusion program requires anticipation of its needs and coordination and training of involved personel in the blood bank, emergency room, operating room and post-surgical care units. Pre deposit and processing of collected blood requires remission of donor criteria and labeling and storage procedures.
