However, there are limited data on the long-term effects of weight-bearing exercise on BMD in adult males, particularly in men with osteopenia. PURPOSE: To 1) determine the effects of six months of resistance training (RT) or plyometrics (PLYO) exercise on changes in BMD in males with osteopenia; and 2) determine the effects of six months of RT or PLYO exercise on changes in bone turnover in males with osteopenia. METHODS: Twenty-one recreationally active (>4 h/wk of activity) healthy males (25-55 y) with osteopenia in the hip or lumbar spine (L1-L4) were randomized into a 6-mo RT (N = 9) or PLYO (N = 8) exercise program. Dual energy X-ray absorptiometry was used to measure bone area, BMC, and BMD of the lumbar spine (L1-L4), total left hip, and whole body (WB). Serum concentrations of osteocalcin (OC), bone-alkaline phosphatase (BAP), tartrate-resistant acid phosphatase isoform 5b (Trap5b) and carboxyterminal telopeptide of type I collagen (CTX) were measured determine bone turnover activity. RESULTS: Two-factor repeated measures ANOVA showed that WB BMD increased after the 6-mo exercise interventions (p=0.070) with no differences between RT and PLYO groups (+1.32 and 0.52 %, respectively). Although there were no statistically significant changes in bone turnover markers, the BAP/CTX (p=0.036) and OC/CTX ratios (p=0.077) were significantly increased after the exercise intervention with no differences between RT and PLYO. CONCLUSION: We are the first to show that participation in 6 mo of RT or PLYO can improve WB BMD and bone turnover ratios in osteopenic men.

The purpose of the present study was to determine the acute response of markers of bone turnover and hormones to a single-bout of resistance-training or plyometrics. Twelve recreationally active males, aged 24-63 years, participated in this partially randomized cross-over study, which included five trials: PLY (fed/fasted), RT (fed/fasted), and a no-exercise, fed control trial (CON) (n=6). Blood was drawn immediately prior to exercise (PRE), immediately following exercise (POST), and 15, 30, 60, 120 min, and 24 hr following PRE. Total testosterone (T), intact parathyroid hormone (PTH), and cortisol (COR) concentrations in serum were determined. Serum bone-specific alkaline phosphatase (BAP) (bone formation marker) and tartrate-resistant acid phosphatase, isoform 5b (TRAP5b) (bone resorption marker) were measured. The results of the present study suggest favorable changes in bone formation and bone resorption as assessed by bone turnover markers (BAP and TRAP5b) following a single-bout of RT or PLY. The decrease in PTH following plyometrics and resistance-training positively correlated with a decrease in TRAP5b, suggesting that PTH may mediate the exercise-induced changes in osteoclast activity.

Testosterone, free testosterone, and estradiol concentrations were assessed using ELISA kits. RESULTS: Whole body and leg BMD increased with no differences between RT and PLY. Hip BMD increased in the RT group. The relationship between the percent change in whole body LBM and the percent change in leg BMD was positive in the PLY group and negative in the RT group. The percent change in whole body LBM was positively correlated with the percent change in hip BMD in the PLY group. The percent change in arm LBM was positively correlated with the percent change in arm BMD in the RT group. The percent change in testosterone and estradiol were negatively correlated with the percent change in hip BMD. DISCUSSION: In conclusion, muscle contraction forces and impact forces may be potential mechanisms for osteogenesis in osteopenic men.

Book excerpt: We studied the role of body composition on C-reactive protein (CRP) and interleukin-6 (IL-6); determine the differences in CRP and IL-6 among HT users and non-users; determine the effect of 12-months of resistance-training and resulting body composition changes on IL-6 and CRP; determine the relationship between BMD and IL-6, CRP and creatine kinase (CK). Sedentary women (N=208, age 44-66, postmenopausal) taking HT (N=106) or not taking HT (N=102) were randomly assigned to resistance-training: HT-exercise (N=55), HT-no exercise (N=45), no HT-exercise (N=53), and no HT-no exercise (N=49). The program included three weekly 60-75 minute sessions of 8 exercises performed in 2 sets of 6-8 repetitions at 70-80% of 1RM. Total fat mass (TFM), lean soft tissue mass (LSTM) and BMD were assessed by dual-energy X-ray absorptiometry (DXA). For each subject, baseline and 1-year IL-6, CRP and CK were measured. High TFM, high IL-6 and HT use were independently associated with high CRP levels. A stronger relationship between IL-6 and AFM compared to TFM was found in HT users. High TFM and LSTM were significantly related to higher IL-6 levels. A stronger relationship between CRP and AFM compared to TFM was found in HT non-users. CRP was higher in HT users (5.47ł5.40 mg/L) compared to non-users (2.70ł3.05 mg/L) and was higher in oral (5.76ł5.29 mg/L opposed; 6.14ł5.97 mg/L unopposed) compared to transdermal HT users (2.65ł4.44 mg/L). CRP increased slightly (p=0.077) in exercisers (0.54 ł 0.34 mg/L) not taking HT compared to controls ( -0.39 ł 0.35 mg/L). Reductions in TFM were associated with reductions in IL-6 and CRP in HT users. CK was positively associated to all BMD sites at baseline. IL-6 change was positively associated with change in femur neck BMD. CRP change was inversely correlated with change in lumbar spine BMD. CK change was directly related to change in total body and femur trochanter BMD. Reductions in TFM were accompanied by reductions in IL-6 and CRP; AFM was more strongly correlated with inflammation than TFM; 12-months of resistance-training did not decrease IL-6 or CRP; IL-6 and CK were positively related to BMD, and CRP was inversely associated with BMD.

PURPOSE: To assess the relationship between changes in IGF-1 and changes in BMD during a 12-mo exercise intervention involving either PLY or RT. METHODS: Twenty males with osteopenia were randomly assigned to either of two, 12 month exercise interventions: RT [n=10] or PLY [n=10]. Dual energy X-ray absorptiometry [DXA] was used to measure bone area and BMD of the LS, WB, Hip, and FN at 0 and 12-mo time points. IGF-1 concentrations were assessed at 0 and 12-mo time points with immunoassays. RESULTS: Subjects in both groups experienced increases in WB BMD [time, p=0.025] and LS BMD [time, p=0.047]. Hip BMD was improved only in the RT group [group x time, p = 0.068]. IGF-1 concentrations were increased in both groups [group x time, p=0.000]. No significant relationships were found between percent change in IGF-1 concentration and percent change in BMD at any site. CONCLUSION: RT and PLY may be effective interventions to maintain or improve BMD in osteopenic men. However, changes in circulating IGF-1 concentration are not related to changes in BMD during a 12-mo exercise intervention in men with osteopenia.

Female volunteers (N = 120), 18-26 years of age have been categorized into normal bone density (NBD, N = 60) and low bone density (LBD, N = 60) subgroups, based on whole body bone density measurements. Subjects subsequently have been randomly assigned within NBD and LBD subgroups for either concentric (n = 30 LBD, n = 30 NBD) and eccentric (N = 30 LBD, N = 30 NBD) isokinetic exercise training. Exercises are performed using non-dominant arms and legs only, leaving dominant limbs as self-controls Results of this study will enhance understandings of effects of specialized isokinetic resistance exercise training on bone adaptations among young adult females. This work has important implications for reducing stress fractures arising from combat- readiness activities in women and their risks for future osteoporosis.

Failure to attain peak bone mass (PBM) during growth can have adverse short and long term musculoskeletal effects, with numerous studies demonstrating inferior bone health in jockey populations. However, no study to date has addressed counteracting the deleterious effects that participation in this sport has on bone health. The purpose of this six-month double-blind randomised placebo controlled trial was to examine the efficacy of 800mg calcium and 400 IU vitamin D daily supplementation on improving bone mineral properties at the tibia (weight-bearing) and radius (non-weight-bearing) using peripheral Quantitative Computed Tomography (pQCT) and blood-borne markers of bone turnover.Three inter-related studies were designed following the findings of a systematic review and meta-analysis examining the effects of calcium and vitamin D supplementation on bone mineral density in healthy males. Study one examined the effect that the protocol had on markers of bone metabolism and bone properties of the radius. Study two was designed to investigate the impact of six months supplementation on weight-bearing bone while study three further explored alterations to radial and polar cortical bone properties at the tibial mid-shaft following the clinical trial.Methods: Twenty-nine male jockeys (age=20.18 ± 3.23yrs) were originally recruited to the study with 17 completing the intervention. Bone properties at the ultra-distal (4%) and proximal (66%) radius and tibia using pQCT and serum vitamin D, Procollagen type 1 N propeptide (P1NP) and C-terminal telopeptide of type I collagen (CTx) were assessed at baseline and six months. Bone properties at the 66% tibial site were further analysed using BoneJ pQCT distribution plug-in. Polar and radial volumetric bone mineral density (vBMD) was measured in 36, ten degree cortical sectors (polar) and three concentric cortical divisions (radial). Polar distribution was further consolidated into four, 90 degree quadrants aligned to anatomical planes. Cortical mineral mass, endocortical and pericortical radii were also analysed.Results: After co-varying for height, body mass and baseline bone measurements, the analysis of co-variance (ANCOVA) results of these combined studies demonstrate that six months calcium and vitamin D supplementation stimulated a reduction in bone resorption together with significant improvements to bone material properties at the proximal tibia in the supplemented group. ANCOVA serum analysis indicated significantly higher vitamin D levels (18.1%, p=0.014) and lower CTx (ng/L) (-24.8%, p=0.011) in the supplemented group with P1NP unchanged. A high peak BMD can prevent or delay the onset of osteoporosis and its complications. Exercise and diet may affect peak BMD by as much as 20 to 40% each and have been identified as the two most important controllable factors determining BMD. The current study investigated the effect of a calcium, vitamin D, and vitamin K supplement combination during initial military cadet exercise training on: BMD, stress fracture occurrence, hormones associated with BMD, and biochemical markers of bone turnover. Significant changes in BMD, either between the supplemented group or the unsupplemented group or across time for both groups were not found. The majority of participants (n = 22) had unexpectedly high levels of physical activity prior to enrollment, and the initial military exercise training program included only moderate levels of activity.
