Book excerpt: Based on current research studies, postoperative pain (POP) is a significant concern among surgical patients that causes anxiety as well as unwanted physiological effects. In addition to the physical and emotional outcomes that poorly controlled POP contributes to, lies the impact on health care costs as well as poor patient satisfaction scores which reflect on the quality of care a facility provides. Due to the problem of poorly controlled POP, many clinicians and facilities are leaning toward incorporating more of a multimodal approach to POP management. Research data has shown that the use of non-pharmacological interventions for pain control is safe, cost-effective, and can provide positive results, as the effects of opioid administration are lessened if not completely eliminated. Implementing non-pharmacological interventions into routine nursing care for POP can lower patient anxiety and pain levels, speed up recovery time, decrease length of recovery room stays and unwanted hospital admissions, decrease hospital stays and ultimately improve patient outcomes.

Available in PDF, EPUB and Kindle. Book excerpt: Like management of disease, management of pain is as old as the human race. When patients come to us with their pain, they present us with a wonderful opportunity: the chance to understand them, to understand how their pain is affecting their lives, the challenge of discovering what is causing their pain, and finally the opportunity to prescribe medications and lifestyle changes to help them gain relief from their pain. It is hoped that this book will provide the latest evidence-based updates on pain management in special circumstances and will serve as a ready reference for those embarking on pain management. Its intent is not to be a heavy book that can only be stored on a bookshelf, but a pocket-sized reference that can be carried, be easily navigated, and be available whenever a conceptual gap compromises pain physicians and their ability to treat their patients.

Book excerpt: Drug overdose, driven largely by overdose related to the use of opioids, is now the leading cause of unintentional injury death in the United States. The ongoing opioid crisis lies at the intersection of two public health challenges: reducing the burden of suffering from pain and containing the rising toll of the harms that can arise from the use of opioid medications. Chronic pain and opioid use disorder both represent complex human conditions affecting millions of Americans and causing untold disability and loss of function. In the context of the growing opioid problem, the U.S. Food and Drug Administration (FDA) launched an Opioids Action Plan in early 2016. As part of this plan, the FDA asked the National Academies of Sciences, Engineering, and Medicine to convene a committee to update the state of the science on pain research, care, and education and to identify actions the FDA and others can take to respond to the opioid epidemic, with a particular focus on informing FDA's development of a formal method for incorporating individual and societal considerations into its risk-benefit framework for opioid approval and monitoring.

The fact that a surgical procedure can leave any kind of pain casts a shadow over this tenet, which is seen as the basis of medical practice and anchor of its principle ethic... It is all the more surprising in that medicine has only paid attention to this paradoxical chronic pain situation for the past few years. Clarifying the knowledge acquired in this field has become all the more urgent for any care-giver today confronted by a legitimate request from patients: Why and how can a surgical procedure, which is supposed to bring relief, leave behind an unacceptable sequela? This is the approach which the contributors to this new subject of major clinical interest invite you to follow as you work your way through this book.

Book excerpt: Currently, two-thirds of patients undergoing surgery experience acute postoperative pain (POP), which increases the possibility of immediate postoperative complications and facilitates the development of chronic POP. Post-surgical pain does not have a useful biological function, so it must be prevented, avoided, and/or eliminated. The objective of the rational management of acute pain after surgery is to facilitate the rapid recovery of patients with a prompt integration into their usual preoperative activities. The global crisis of opioid abuse has forced us to use other analgesic drugs as well as regional analgesia techniques that are part of effective multimodal analgesia. The non-pharmacological approach has also gained importance in the comprehensive management of acute POP. This book reviews advances in the understanding, management, and complications of POP.

Available in PDF, EPUB and Kindle. Book excerpt: Millions of procedures, surgical and non-surgical, are performed around the world each year. Management of perioperative pain is of great importance to patients, and a critical management issue for physicians and other health professionals who provide perioperative care. Healthcare facilities and national accrediting organizations have established standards surrounding proper management of perioperative pain. There is now an increased burden on every hospital and training program to ensure that healthcare providers understand the essentials of pain management and are able to recognize and treat pain in a timely fashion. Poorly controlled pain leads to patient dissatisfaction and contributes to increased morbidity and mortality, such as myocardial infarction, pneumonia, and emotional effects. Proper management, including appropriate pharmacologic management and regional analgesic techniques, can improve function and shorten length of hospital stay. Patients who are undergoing procedures under sedation or anesthesia need to receive adequate pain relief with drugs or regional anesthetic techniques. In the recovery room following the procedure, the staff taking care of the patient needs to be properly trained to diagnose and treat post-procedural pain. Part of the Oxford American Pain Library, this concise, evidence-based clinical guide serves as a tool for every clinician who wishes to understand the basic mechanisms, pharmacology, invasive and noninvasive treatment modalities, guidelines and development of pain management protocols. The authors address new technologies, chronic pain issues, running an acute pain service, opioid and non-opioid pharmacology (including newly approved drugs), epidural and other regional anesthesia, and special populations such as pediatric patients, the elderly, and patients with a co-existing disease.

Available in PDF, EPUB and Kindle. Book excerpt: Management of postoperative pain has constantly become a major concern to healthcare providers throughout the years. As part of the healthcare team, nurses play an important role in educating surgical patients of different approaches to promote optimal pain relief, which includes non-pharmacological interventions. The study aimed to determine the effectiveness of utilizing photonovela as an alternative educational tool to enhance the knowledge, skill and compliance of surgical patients regarding non-pharmacological pain management strategies. The study utilized quasi-experimental research design. Thirty (30) patients, with ages ranging from 24-65 years old and who are scheduled for major abdominal surgery, were selected for this study. The control group (n=15) received the routine preoperative health education, while the experimental group (n=15) were provided with a copy of the photonovela. The content of the photonovela included instructions on relaxation technique (slow deep breathing), distraction techniques, and splinting exercise. The difference in the knowledge, skill, and compliance of the patients were measured using a self-constructed questionnaire and checklist. Independent t-test and paired t-test were used to treat the data. The findings of this study showed that there was a significant difference in the pretest and post-test scores in terms of knowledge and skill and also in the compliance scores to health instructions between the control group and experimental group. Therefore, the developed photonovela can effectively contribute in the improvement of patient education.

Book excerpt: The rapid expansion in same day surgery centers has facilitated a shift in the surgical nursing interventions needed to treat our patients. The evolving evidence-based practice has played a major part in influencing the nurse-led changes that are rapidly happening, such as pain management through preoperative and postoperative interventions. The aim of this paper is to show why using alternative therapies such as guided imagery and music therapy, in conjunction with pain medications, is the most effective way of treating patients' pain postoperatively. Patients with chronic pain report higher levels of pain, and the majority of patients come in for surgery experiencing some type of pain. Preoperative pain can cause anxiety over the situation, as well as fear about receiving inadequate pain control. Both guided imagery and music therapies can greatly reduce the patient's preoperative anxiety, and reduce their postoperative pain; leading to faster discharge times and greater patient satisfactions. The only way these changes can be made is through nurse and patient education. Nurses are leading the way in non-pharmacological pain reduction methods. There is an increased need for nurses to understand these alternative therapies, so they can feel confident when applying this knowledge to their patients' care. It's up to the nurses to teach the patients what alternative therapies are, and how they are essential to them in their pain control management and to reduce their anxiety. Education has a positive effect on nurses, patients, and their outcomes. The healthcare changes of today demands that these kinds of initiatives be implemented in our patient centered care. The process of building this evidence-based research looked into the problem of postoperative pain, consisting of an in-depth look at current research regarding alternative therapies for treating postoperative pain. Selection of the best, most appropriate literature through the process of a literature review was done. The evidenced was compiled and grouped into main themes, education of both nurses and patients learning different alternative therapies, and using both guided imagery and music therapy to accomplish the goal of nurse interventions. The implementation plan was set up incorporating the nursing interventions, education, and the use of guided imagery and music therapy with patients both preoperative and postoperative. The plan was evaluated through the use of chart reviews and patient surveys, and the dissemination plan showed all of the key stakeholders and nursing staff the positive results of this evidence-based practice change.
