Le vieillissement de la population a conduit à un accroissement de personnes conductrices de plus de 70 ans. Le taux d'accidents de la route augmente pour cette tranche d'âge, ce qui interroge sur la durée de validité du permis de conduire. Une proposition de loi visant à instaurer une visite médicale d'aptitude obligatoire à la conduite de toutes les personnes âgées de 70 ans est discutée depuis 2017. Le nombre insuffisant de médecins agréés pour l'aptitude à la conduite laisse supposer une possible participation des médecins généralistes. Objectif. L'objectif de la thèse est de recueillir les expériences de pratiques des médecins généralistes face aux conducteurs âgés ayant une réduction de leurs capacités fonctionnelles interférant sur leurs aptitudes à conduire et de connaitre leur implication dans le processus d'arrêt ou de maintien de la conduite. L'objectif sous-jacent est d'identifier la place et le rôle des médecins généralistes dans cette prise de décision collective (personnes âgées, proches aidants, institutionnels, professionnels soignants...). Méthode. Nous avons fait le choix d'entreprendre une analyse qualitative compréhensive à partir d'entretiens individuels semi-dirigés auprès de 15 médecins généralistes de la Loire. Résultats. Les récits d'expériences mettent en évidence la prise de conscience générale des médecins généralistes du caractère dangereux de la conduite lié à l'avancée en âge, alors même que les seniors ne soient pas à l'heure actuelle les plus accidentogènes. Le sentiment de liberté et d'autonomie sociale relative à la conduite automobile est également souligné par les acteurs interrogés. Du point de vue de la formation universitaire sur la conduite automobile, les médecins généralistes apparaissent démunis face à cette problématique, allant jusqu'à occulter dans les consultations l'abord de la conduite avec leurs patients. Nous avons confirmé le rôle majeur joué par l'entourage familial dans le processus d'arrêt de la conduite. Enfin, en raison du lien de confiance établi dans la relation médecin-patient, les médecins généralistes interrogés ne semblent pas favorables à assurer eux-mêmes une visite médicale obligatoire s'ils sont parallèlement le médecin traitant du conducteur. Conclusion. La poursuite de la conduite est le signe d'un vieillissement réussi. Le médecin généraliste apparait comme un acteur incontournable pour évaluer l'aptitude à la conduite de la personne âgée. Or, il se place dans une ambivalence entre son rôle de bienveillance et son rôle de sanction, ce qui peut mettre en péril sa relation avec son patient. En ce sens, la relation de confiance médecin-patient est à prendre en compte dans tout nouveau projet de Loi sur la conduite.
