Available in PDF, EPUB and Kindle. Book excerpt: Myasthenia gravis is a rare potentially fatal chronic autoimmune disorder. Circulating autoantibodies directed against components of the neuromuscular junction of skeletal muscles, most commonly nicotinic acetylcholine receptor and associated protein in the postsynaptic membrane, block neuromuscular transmission resulting in muscle weakness. This muscle weakness typically worsens with continued activity, improves on rest, and is of variable severity ranging from mild ocular muscle weakness to severe generalized muscle weakness, involving the respiratory muscle with impending respiratory failure. The worldwide prevalence of myasthenia gravis is 100-200 per million population, affecting more than 700,000 people all over the world. The prevalence rate has increased since the 1950s due to improved diagnostic precision and decreased mortality rate.

Available in PDF, EPUB and Kindle. Book excerpt: Autoimmune myasthenia gravis (MG) is a classical autoimmune disease, for which the target antigen, nicotinic acetylcholine receptor, has been cloned, sequenced and biochemically characterized. Antibodies to acetylcholine receptors destroy acetylcholine receptor at the neuromuscular junction, thus leading to defective neuromuscular transmission, muscle fatigue, and weakness. In the last few years, rapid advances have been made in unraveling the cellular and molecular mechanisms involved in the pathogenesis of MG, both in the animal model, experimental autoimmune MG (EAMG), and in human MG. Significant advances are being made in characterizing the cells and molecules involved in the autoimmune response to the acetylcholine receptor (AChR). These advances are leading to the development of specific methods of immunointervention in EAMG. Further understanding of the intricate involvement of the major histocompatibility complex (MHC) and non-MHC genes, T cell receptors (TCR), costimulator molecules, and specific cytokines in the afferent and efferent autoimmune response of AChR should pave the way to future antigen/clone-specific therapy of MG. This book is the outcome of the MG workshop proceedings in Mysore, India, 1998. The majority of the chapters in this book are contributed by world-renowned authors and their students. The book not only contains a timely review of specific topics, but also up-to-date findings. Immunologists and neurologists will find, in this volume, the latest in MG/EAMG cutting-edge research. Clinicians will be interested in the applications of the various immunointervention strategies into clinical trials in MG patients. Finally, students will not only be interested in reading the latest in EAMG/MG research, but will also find information to help them develop a future strategy to unravel the precise mechanism of disease. To summarize, in this book, the readers should find up-to-date information related to immunological mechanisms involved in MG pathogenesis and various modalities for possible approaches to immunointervention to treat MG.

Book excerpt: This book offers a practical tool for handling neuromuscular emergencies, and is divided by the main neuromuscular diseases on the basis of their anatomical and clinical location: diseases affecting the anterior horn of the spinal cord (tetanus), those that affect the nerve roots and nerves (SGB, multiple mononeuritis, porphyria), the neuromuscular junction (MG and botulism) and, lastly, muscle pathologies (rhabdomyolysis, malignant hyperthermia, periodic paralysis). It provides updated diagnostic/therapeutic protocols and decision-making algorithms for each pathology, and examines a range of scenarios on the basis of clinical cases. Emergencies in Neuromuscular Disease will be of interests for a wide variety of specialists including neurologist, neurophysiologists and anesthesiologists.

Book excerpt: Advances in the study and understanding of myasthenia gravis have led to the need for the publication of this important new edition. The goal of Myasthenia Gravis and Related Disorders, Second Edition is identical to the first -- to provide the clinician and the scientist with a common resource for understanding this complex disorder. This new edition begins with discussions of neuromuscular junction structure and function and follows with updated chapters covering a wide range of topics, such as the acetylcholine receptor, clinical presentation, diagnostic evaluation, and treatment. Importantly, new supplemental chapters have been added; these discuss rigorous clinical assessments of patients for research trials and the epidemiology and genetics of myasthenia gravis. The discussion of the most challenging aspects of myasthenia gravis, its impact on patients’ psychological make-up, has been expanded as well. Myasthenia Gravis and Related Disorders, Second Edition retains the “personal approach” of the authors regarding treatment and is a valuable resource for meeting the many and varied needs of patients with myasthenia gravis.

Yet to date, there has been no single, compact book that offers all the relevant information related to the management of these disorders. This book fills that gap, presenting the state of the art in the field and addressing practical problems together with their solutions. Each chapter covers disorder characteristics, clinical differentiating points, relevant investigations and their interpretation, available genetic testing, best management approaches and counselling. Illustrative cases provide valuable insights, while extensive tables and illustrations mean that information can be rapidly found.

Available in PDF, EPUB and Kindle. Book excerpt: Epilepsy is the most common neurological disorder globally, affecting approximately 50 million people of all ages. It is one of the oldest diseases described in literature from remote ancient civilizations 2000-3000 years ago. Despite its long history and wide spread, epilepsy is still surrounded by myth and prejudice, which can only be overcome with great difficulty. The term epilepsy is derived from the Greek verb epilambanein, which by itself means to be seized and to be overwhelmed by surprise or attack. Therefore, epilepsy is a condition of getting over, seized, or attacked. The twelve very interesting chapters of this book cover various aspects of epileptology from the history and milestones of epilepsy as a disease entity, to the most recent advances in understanding and diagnosing epilepsy.

This book was released on 2020-07-12 with total page 319 pages. Available in PDF, EPUB and Kindle. Book excerpt: The Social Security Administration (SSA) administers two programs that provide disability benefits: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. SSDI provides disability benefits to people (under the full retirement age) who are no longer able to work because of a disabling medical condition. SSI provides income assistance for disabled, blind, and aged people who have limited income and resources regardless of their prior participation in the labor force. Both programs share a common disability determination process administered by SSA and state agencies as well as a common definition of disability for adults: "the inability to engage in any substantial gainful activity by reason of any medically determinable physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than 12 months." Disabled workers might receive either SSDI benefits or SSI payments, or both, depending on their recent work history and current income and assets. Disabled workers might also receive benefits from other public programs such as workers' compensation, which insures against work-related illness or injuries occurring on the job, but those other programs have their own definitions and eligibility criteria. Selected Health Conditions and Likelihood of Improvement with Treatment identifies and defines the professionally accepted, standard measurements of outcomes improvement for medical conditions. This report also identifies specific, long-lasting medical conditions for adults in the categories of mental health disorders, cancers, and musculoskeletal disorders. Specifically, these conditions are disabling for a length of time, but typically don't result in permanently disabling limitations; are responsive to treatment; and after a specific length of time of treatment, improve to the point at which the conditions are no longer disabling.

Book excerpt: Clinical Neurophysiology, Third Edition will continue the tradition of the previous two volumes by providing a didactic, yet accessible, presentation of electrophysiology in three sections that is of use to both the clinician and the researcher. The first section describes the analysis of electrophysiological waveforms. Section two describes the various methods and techniques of electrophysiological testing. The third section, although short in appearance, has recommendations of symptom complexes and disease entities using electroencephalography, evoked potentials, and nerve conduction studies.
