This is a relatively recent and extremely excit ing addition to the field of hypertension research. The development of new analytical procedures, such as radio ligand-binding methods, has made it possible to identify and quantify the adrenoceptors in different tissues and in circulating blood cells. As a consequence, much new and valuable information has been obtained. An under standing of the role of the fJ-adrenoceptor-adenylatecyclase-cAMP system in hypertension is one such product of this new approach. Both review articles and new experimental and clinical data are included in this volume. New aspects of the distribution, identifica tion and regulation, and the chronopharmacology of adrenoceptors, as well as beta blockade, will be highlighted. The clinical contributions concern the normal regulation of the fJ adrenoceptor-adenylatecyclase-cAMP system, and how it is altered in hypertensives as a possible mechanism in the pathogenesis and maintenance of essential hypertension. With this knowledge it is be coming easier to explain the molecular and cellular action behind the familiar role of the sympathico-adrenergic system in the devel opment of hypertension and the mechanism of beta blockade. The vast majority of the data available in this field is confined to periph eral mechanisms, owing to the limited possibilities for investigating the central nervous system under clinical conditions.

Book excerpt: Within the field of general medicine, the last two decades have seen the curative procedures increasingly supplemented by func tions that can be summed up under the heading of "preventive health care." There is a close link between this development and the changing morbidity pattern. In highly developed countries the morbidity rate attributable to infectious diseases has been much reduced, while the various types of arteriosclerosis, some tumorous conditions, and so-called functional syndromes have attained massive incidence rates. Systematic epidemiologic studies have yielded the concept of risk factors, i. e., certain exogenous and endogenous factors whose elimination, ideally, would make it possible to avoid the manifestation of an illness or to ensure it did not become mani fest until later in life or in a milder form. The socioeconomic and sociomedical significance of these aspects is considerable, as can be seen from all the statistics on the reasons for incapacity and early retirement and on the relative contributions of different ill nesses to total mortality. Effective avoidance of illnesses by primary prophylaxis (elimi nation of the causes of disease) or early detection of the symp toms of disease (secondary prophylaxis) leading to early treat ment would be a particularly significant step forward, since in the case of most of the conditions mentioned above only pallia tive forms of treatment are available, and none that have any cu rative effect.

Book excerpt: Hypertension is a major health problem and contrary to ischemic heart disease, which occurs only in Western countries, its distribution is almost universal. It is this universality that has prompted us to gather, in this book, data on arterial blood pressure obtained in different parts of the world. Moreover, cerebrovascular mortality, which is the commonest cause of death from hypertension, is decreasing in most Western countries and in Japan, and the reasons for this are still far from clear. A major problem in comparing blood pressure values from different centers is the standardization of the measurement. Complete standardization will never be achieved if one takes into account the numerous factors that may influence blood pressure in epidemiological studies. Whether blood pressures are measured by doctors or by technicians, are recorded at home, in the working place or in a hospital, in sitting, standing or supine position, and is taken during the same examination-all of these whether a blood sample factors can influence blood pressure measurement. But meals, time of day (blood pressure being higher in the evening), heart rate, cuff size, stethoscope used, digit preference, month of year, temperature, etc., can equally influence the measurement. Home reading of arterial blood pressure at standardized times is probably the best answer to all of these problems and has been used with gratifying results in a comparative study between Belgium and Korea.

Book excerpt: In addition to standardized casual blood pressure readings, ambulatory blood pressure monitoring (ABPM) - using automatic noninvasive (= indirect) devices for home readings and fully automated monitors for 24-h profiles - have become a widely used necessary tool in clinical research. This book summarizes the state of the art in the whole field of indirect blood pressure monitoring. It is based on two international meetings and on invited papers. We have divided the subject matter into two main areas: 1) Automatic blood pressure devices for discontinuous registration, and 2) Portable, fully automated programmable monitors for continuous monitoring. The availability of all new technologies is described in detail and current technical and physiological problems have been covered in depth. Both topics have been subdivided into a) Methods and Techniques, and b) Clinical Applications. Both parts are updated and have critically evaluated available automatic sphygmomanometers and portable computers equipped with different techni ques (e. g. , auscultation, oscillometry, plethysmography). Reliability in the intensiv Care unit as well as in outpatients management, common clinical problems, clinical relevance compared to casual blood pressure are described in the first part. In the second part, ten years of experience on fully automated noninvasive methodology - compared to intraarterial techniques - have been elaborated by international experts; the possibilities and limitations are clearly demonstrated. Analyses in different clinical fields in the diagnosis of primary and secondary hypertension are given. Different statistical analyses of blood pressure variability and circadian rhythms are discussed.
