Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: It is generally accepted that satisfaction with health care is multidimensional and includes many factors removed from the direct delivery of medical services. The analysis seeks to determine if patient satisfaction with health care varies by type of insurance coverage held and whether overall satisfaction is higher within market based health care systems as found in the United States. Using the National Center for Health Statistic's Joint Canadian United States Survey of Health, 8700 participants (61% American 39% Canadian) made available self-reported information on their health care utilization and health status. Comparison are made between MEDICARE, MEDICAID, employer based insurance, and the National Health Insurance of Canada. The results indicate that there is significant variation with satisfaction depending on type of insurance. When compared to the national Canadian system, American patients with employer based insurance report higher levels of satisfaction.

Book excerpt: This paper examines satisfaction with various aspects of health care by type of health plan among three groups of health-plan enrollees: those with a consumer-driven health plan (CDHP), those with a high-deductible health plan (HDHP), and those with traditional coverage. The findings presented in this paper are derived from the 2011 EBRI/MGA Consumer Engagement in Health Care Survey, an online survey that examines issues surrounding consumer-directed health care, including the cost of insurance, the cost of care, satisfaction with health care, satisfaction with health care plans, reasons for choosing a plan, and sources of health information. This paper also incorporates findings from the 2011 survey as well as findings from the 2005, 2006, and 2007 EBRI/Commonwealth Fund Consumerism in Health Care Survey, and the 2008, 2009, and 2010 EBRI/MGA Consumer Engagement in Health Care Survey. Satisfaction levels have been trending up among CDHP enrollees, and trending down among traditional-plan enrollees. Satisfaction levels with getting doctor appointments were high relative to other aspects of health care, regardless of plan type. Individuals in consumer-driven health plans (CDHPs) and high-deductible health plans (HDHPs) were less likely than those in traditional plans both to recommend their health plan or to stay with their current plan, if they had the opportunity to switch. Dissatisfaction with out-of-pocket costs may be driving overall satisfaction trends. Book excerpt: Disparate healthcare experiences continue to pose a challenge; vulnerable populations such as low-income and racial and ethnic minorities may not be able to afford or utilize care when needed or receive quality care. The sources of disparities are complex and multi-factorial, which include health care system-level factors such as insurance and health care workforce. It is relatively less known to what extent these contribute to disparities related to a patient's overall health care experience across three important domains -- affordability, utilization and satisfaction with care. This dissertation has three objectives. First, to assess how insurance benefit design affects health care utilization among poorest adults. Second, examine the role of insurance in addressing racial and ethnic disparities in access to preventive care. Finally, examine the role of health care providers in differences related to satisfaction with care among low-income patients. To answer questions posed in this dissertation, two different types of datasets are used: a unique hospital administrative data from a coverage program for low-income adults and 2008-2014 Medical Expenditure Panel Survey (MEPS). To examine the role of insurance and health care providers in disparities related to different outcomes of patient experience, several models are estimated; including mixed effects linear probability and negative binomial regressions, decomposition and multivariate linear probability models. Several efforts are being made to address inequalities through coverage expansions, removal of financial barriers for preventive services and incentivizing health care providers to improve patient satisfaction. The findings suggest that differences in utilization and satisfaction with care continue to persist among low-income and racial and ethnic minorities. However, policy levers and system-level reforms including value-based insurance designs that may curb healthcare costs without shifting the cost burden to poorer adults, continued reforms to expand coverage and improve access to a usual of care, and policy interventions that extend beyond improving workforce diversity and enhance provider skills to elicit patient communication preferences may foster positive patient experiences and ameliorate existing disparities. Improving patient experiences of care will thus require policy efforts with a comprehensive multi-level strategy that targets broad sectors - including payers, health care providers and society at large.

A portion of the uninsured reflects unemployment rates; however, this rate is primarily a reflection of the fact that when most health plans meet an individual's needs, most times, those health plans are not affordable. Research shows that people without health insurance are more likely to experience financial burdens associated with the utilization of health care services. But even among the insured, underinsurance has emerged as a barrier to care. The Patient Protection and Affordable Care Act (ACA) has made the most comprehensive changes to the provision of health insurance since the development of Medicare and Medicaid by requiring all Americans to have health insurance by 2016. An estimated 30 million individuals who would otherwise be uninsured are expected to obtain insurance through the private health insurance market or state expansion of Medicaid programs. The success of the ACA depends on the design of the essential health benefits (EHB) package and its affordability. Essential Health Benefits recommends a process for defining, monitoring, and updating the EHB package. The book is of value to Assistant Secretary for Planning and Evaluation (ASPE) and other U.S. Department of Health and Human Services agencies, state insurance agencies, Congress, state governors, health care providers, and consumer advocates.

For instance, a majority either strongly or somewhat support health insurance market reform, the availability of a public plan option, mandates on employers and individuals, and expansion of public programs to cover more of the uninsured. When the implications of health reform are raised in public opinion polls, support for health reform drops. This paper examines public opinion by insurance status and health status, using data from the 2009 EBRI/Mathew Greenwald & Associates Health Confidence Survey. It finds that the uninsured are more likely than individuals with insurance coverage to be dissatisfied with the quality of health care received and they are less confident in various aspects of health care. The gap in satisfaction and confidence has also grown over time. Similar differences in satisfaction and confidence are found by health status. While these sharp differences in attitudes are not surprising, shoring up the system for the uninsured and individuals with chronic conditions means changing the health care system for everyone - notably for those with insurance coverage and in good health. Ultimately, will the needs of the few outweigh the satisfaction of the many?
