Ces deux mécanismes vont engendrer une concentration accrue de glucose dans le sang qui sera responsable des différentes complications. Selon de nombreuses études, l'hyperglycémie sanguine aurait une influence sur l'état de santé orale des patients atteints de diabète, et notamment sur la santé parodontale et la denture, mais le mécanisme étiopathogénique reste encore mal connu. Les personnes âgées, bien que majoritairement concernées par cette pathologie, sont peu représentées dans les études publiées à ce sujet. Une enquête épidémiologique descriptive a donc été menée sur une cohorte de 78 patients diabétiques de type 2 de plus de 65 ans au CHU de Rouen entre janvier 2015 et novembre 2015. Les personnes âgées, bien que majoritairement concernées par cette pathologie, sont peu représentées dans les études publiées à ce sujet. Une enquête épidémiologique descriptive a donc été menée sur une cohorte de 78 patients diabétiques de type 2 de plus de 65 ans au CHU de Rouen entre janvier 2015 et novembre 2015. A travers les données relevées, ce premier ouvrage contribue à objectiver l'impact d'un déséquilibre glycémique sur l'état de la denture de ces patients. Après une description des complications médicales et odontologiques pouvant survenir dans le cadre du diabète, ce travail présente l'étude menée, son protocole clinique et ses résultats. Il servira également de point de départ à la réalisation d'une enquête épidémiologique de plus grande ampleur, rentrant dans un projet coopératif international franco-brésilien.

Il est admis aujourd’hui qu’il existe un lien bidirectionnel entre la santé orale et le diabète. C’est alors que l’acquisition des connaissances et des bonnes pratiques en santé orale chez ces patients devrait alors être indispensable dans leur prise en charge. Nous avons donc décidé de réaliser une étude exploratoire au CHRU de Montpellier en 2021-2022, dans le service de diabétologie, afin d’évaluer le niveau de connaissance et d’analyser l’état de santé orale des patients diabétiques. Les résultats de cette étude démontrent la mise en place nécessaire d’actions de prévention auprès des patients diabétiques, au vu d’un niveau de connaissances perfectible, de leur état de santé orale, de leur implication dans leur pathologie ainsi que de leur prise en charge médicale. En effet, encore trop peu de ces patients sont redirigés vers un chirurgien-dentiste (17%) pour un bilan initial avec près d’un patient sur deux décrivant au moins une pathologie dentaire. Enfin, l’intégralité des patients ayant fait le bilan bucco-dentaire via le dispositif de caméra intra-orale (télémédecine) avait un contrôle de plaque insuffisant. Le volet santé orale devrait être intégré dans le parcours de soins de tous les patients pour espérer diminuer les pathologies bucco-dentaires, améliorer leur santé générale et donc leur bien-être quotidien
