Book excerpt: Studies show that generally across industry human error is the cause in some 90% of incidents and that 70% of incidents could have been prevented by management action. They point to the crucial significance of a systematic approach to the management of health and safety, and the need to be aware of the human factor as a distinct element in that framework. Reference is made to the fundamental causes of the explosion at Peterborough, United Kingdom, in March 1989 to illustrate the way in which those pointers could equally apply in the explosives industry. A new Explosives Incidents Database Service (EIDAS) has been set up in the UK to promote the greater pooling and promulgation of information on explosives incidents. Modeled on the internationally recognized MHIDAS system that covers the more general major hazard incidents, the goal of EIDAS is to achieve an equivalent status and standing. The aims of EIDAS are as follows: (1) To establish a management and control system for collecting and analyzing explosives incidents data, (2) To provide an efficient system for storing and distributing the data, (3) To provide a "follow-up" service to obtain more detailed information on occurrences as requested by the customer, and (4) To make the system readily available to a world-wide network to gain maximum utilization.

Book excerpt: This report explains two explosions that took place on January 7, 1998, at an explosives manufacturing facility owned by Sierra Chemical Company, located in Mustang, Nevada. Four workers were killed and six were injured. Safety issues covered in the report include process safety management, process hazard analysis, training, language barriers, operating procedures, building siting, and employee participation. Recommendations concerning these issues were made to Sierra Chemical Company and other companies manufacturing explosives, the Institute of Makers of Explosives, the Nevada Occupational Safety and Health Enforcement Section, and the Department of Defense. The Chemical Safety and Hazard Investigation Board (CSB) is an independent federal agency with the mission of ensuring the safety of workers and the public by preventing or minimizing the effects of chemical incidents at industrial facilities and in transport. The CSB is a scientific investigatory organization; it is not an enforcement or regulatory body. Established by the Clean Air Act Amendments of 1990, the CSB is responsible for determining the probable causes of incidents, issuing safety recommendations, studying chemical safety issues, and evaluating the effectiveness of other government agencies involved with industrial chemical safety. Section 112 (r) (6) (G) of the Clean Air Act prohibits the use of any conclusions, findings, or recommendations of the CSB relating to any chemical incident from being admitted as evidence or used in any lawsuit arising out of any matter mentioned in an investigation report. Congress modeled the CSB after the National Transportation Safety Board (NTSB), which investigates aircraft and other transportation accidents for the purpose of improving safety. Like the NTSB, the CSB makes public its actions and decisions through investigation reports, safety studies, safety recommendations, special technical publications, and statistical reviews. Book excerpt: Improvised explosive devices (IEDs) are a type of unconventional explosive weapon that can be deployed in a variety of ways, and can cause loss of life, injury, and property damage in both military and civilian environments. Terrorists, violent extremists, and criminals often choose IEDs because the ingredients, components, and instructions required to make IEDs are highly accessible. In many cases, precursor chemicals enable this criminal use of IEDs because they are used in the manufacture of homemade explosives (HMEs), which are often used as a component of IEDs. Many precursor chemicals are frequently used in industrial manufacturing and may be available as commercial products for personal use. Guides for making HMEs and instructions for constructing IEDs are widely available and can be easily found on the internet. Other countries restrict access to precursor chemicals in an effort to reduce the opportunity for HMEs to be used in IEDs. Although IED attacks have been less frequent in the United States than in other countries, IEDs remain a persistent domestic threat. Restricting access to precursor chemicals might contribute to reducing the threat of IED attacks and in turn prevent potentially devastating bombings, save lives, and reduce financial impacts. Reducing the Threat of Improvised Explosive Device Attacks by Restricting Access to Explosive Precursor Chemicals prioritizes precursor chemicals that can be used to make HMEs and analyzes the movement of those chemicals through United States commercial supply chains and identifies potential vulnerabilities. This report examines current United States and international regulation of the chemicals, and compares the economic, security, and other tradeoffs among potential control strategies.

Attribute Based Site Planning (ABSP) is a new paradigm which gets away from the "Have you complied with the rule?" mentality and focuses on whether adequate protection is provided. Attributes such as hardness, accident likelihood, mission essentiality, and value would be assigned to each explosives source and target. Computer software would then evaluate each explosives source / target pair to determine the acceptability of the relationship. The computer would display mission impact, damage, level of threat, as well as compliance with minimum standards. Modern desktop and laptop computers have sufficient processing power to implement this paradigm, and to transition the static explosives safety site planning process into a dynamic explosives safety management system.
