Book excerpt: Brazil’s public policy response to the AIDS epidemic preceded those of many developing countries. During my tenure as President, in 1996, Brazil adopted a law guaranteeing free and universal access to AIDS treatment for all people living with HIV/AIDS. Brazil became the first developing country to provide publicly-financed AIDS treatment for all people living with HIV/AIDS. We now have one of the world’s most successful AIDS programs that is considered a model for other dev- oping countries. Today, 185,000 people receive life-saving AIDS cocktails in Brazil, and thousands of lives have been saved. But this was not an easy battle. There were many challenges along the way. Twenty years ago, Brazil’s achie- ments today might have seemed impossible. During the 1980s, in Brazil, as elsewhere, there was overwhelming stigma associated with AIDS; people living with HIV often lost their jobs and died quickly before the advent of life-saving antiretroviral drugs. Brazil’s AIDS movement was extraordinarily important in promoting progressive AIDS policies; associations of people living with HIV were the first to denounce pervasive AIDS-related discri- nation and called public attention to the importance of AIDS. Activists protested in the streets for over a decade, engaged the media, and framed AIDS as a human rights issue.

Book excerpt: This study assesses the potential demand by the public sector for a preventive HIV/AIDS vaccine in Brazil and the costs of alternative strategies for a vaccination program. Brazil has a mature AIDS epidemic: the percent of the population living with HIV or AIDS (about 0.6 percent of adults) is not as high as in other severely affected developing countries, but infection rates in specific risk groups in the population are very high and HIV has spread beyond these groups into the general population of low-risk individuals.Preventive HIV/AIDS vaccines are still in the testing stage. The characteristics of the first vaccines developed, in terms of their efficacy, duration of effectiveness, ease of administration, and price, are still unknown. But the potential benefits of such a vaccine in Brazil would be high. The study reviews the cost and impact of HIV/AIDS in Brazil, in terms of disease and economic burden, as a proxy for the benefits of an HIV/AIDS vaccine. The epidemiology of AIDS and Brazil's experience with immunization coverage with other vaccines are used to assess the number of vaccines, delivery strategies, and possible costs of an HIV/AIDS immunization program in Brazil, assuming the availability of a 100 percent effective AIDS vaccine that lasts a lifetime under different pricing and dosing assumptions.A low-cost, highly effective vaccine would likely be affordable to an upper-middle-income country like Brazil and yield large benefits from a policy of universal, publicly subsidized immunization. But if prices are higher and the impact less favorable, the costs and effects would have to be compared with other AIDS prevention programs or other health interventions. Both political and economic considerations will likely figure into public policy on HIV/AIDS vaccination, when such a vaccine is developed.This paper - a product of Public Services, Development Research Group - is part of the research project on quot;The Economics of an HIV/AIDS Vaccine in Developing Dountries: Potential Impact, Cost-Effectiveness and Willingness to Pay,quot; sponsored by the European Commission and the Development Research Group of the World Bank. Book excerpt: It has now been 25 years since the apocryphal report in the CDC Morbidity and Mortality Weekly Report dated June 5, 1981 entitled, “Pneumocystis Pneumonia - Los Angeles”, which announced what was to become HIV/AIDS. HIV has now affected virtually all countries that have looked for it and has had a devastating impact on the public health and medical care infrastructure around the world. HIV/AIDS has also disproportionately affected nations with the least capacity to confront it, especially the developing world nations in Sub-Saharan Africa, South and Southeast Asia, and the emerging republics of Eastern and Central Asia. The pandemic, unlike any other disease of our time, has had profound impacts on the practice of public health itself: bringing affected communities into decision making; demanding North-South partnerships and collaborations; and changing the basic conduct of clinical and prevention trials research. While much has been written in scholarly publications for medical, epidemiologic and disease control specialists, there is no comprehensive review of the public health impact and response to HIV/AIDS in the developing world. This edited volume seeks to systematically describe the emergence and form of the epidemics (epidemiology), the social, community and political response, and the various measures to confront and control the epidemic, with varying levels of success. Of particular importance are strategies that appear to have been useful in ameliorating the epidemic, while contrasting the situation in a neighboring country or region where contrasting prevention or care initiatives have had a deleterious outcome. Common to all responses has been the international multi-sectoral response represented by the Global Fund for HIV/AIDS, Malaria and Tuberculosis, the President's Emergency Plan for AIDS Relief, and the Gates Foundation, among others, to promote HIV pharmacologic therapy in resource-poor settings. The chapter authors will explore the political challenges in meeting HIV/AIDS prevention and care in concert with the public health realities in specific country and regional context.
