Available in PDF, EPUB and Kindle. Book excerpt: Effective treatment for HIV and AIDS came in 1996. For sufferers in the developed world, this marked a true watershed moment: the end of the death sentence. But for many in the developing world, including in Southeast Asia, these new treatments remained far out of reach. In his early thirties, following the loss of his partner to an AIDS-related illness, Chris Beyrer wrote the first edition of War in the Blood. Three decades later, having served as president of the International AIDS Society, he believes we have arrived at an extraordinary milestone. For the first time, a patient has been demonstrably cured of HIV, new vaccine trials in Thailand have shown great promise, and the PrEP programme genuinely works. So why are over half of the estimated 38.8 million people living with HIV still not on treatment? War in the Blood is a labour of love, both a celebratory account of Southeast Asia and the story of our failure to protect those most vulnerable the world over – gay men, adolescent girls, sex workers, drug users, and transgender women. Beyrer offers an impassioned plea for our communities and governments – and our own hearts and minds – to stop denying the realities of sex, sexuality, and gender, and to take affirmative action.

Available in PDF, EPUB and Kindle. Book excerpt: HIV is the leading cause of premature death in Thailand. Since the first case of AIDS was reported in 1984 more than one million Thais have been infected. The social, human and economic costs of this burden are enormous. The Thai government has shown a strong commitment to providing care and support to persons living with HIV/AIDS by launching the National Access to Care Program (NAPHA) in 2003, which provides for publicly financed antiretroviral therapy (ART) to all HIV-infected people. This book documents through interviews how ART has radically changed the lives of those living with HIV. In the words of an HIV positive 29-year old man, ART is a "miracle." The book then develops an innovative analytical framework and uses it to show how the future sustainability and cost-effectiveness of this ambitious program depend critically on Thai government choices of AIDS treatment policy, HIV prevention policy and AIDS drug pricing. For the most likely assumptions, the book estimates that ART will save years of healthy life at a cost of between $700 and $2,400 per year. Successful AIDS treatment accumulates ever-increasing numbers of patients who need subsidized ART. Despite the magnitude of the resulting fiscal burden, the authors judge this expenditure to be a worthwhile public health investment for Thailand, However, they show that the future sustainability of the program will hinge critically on how well the government manages the quality of ART service delivery, on whether it is able to sustain its past successes in HIV prevention and on its negotiations with multinational pharmaceutical manufacturers on the prices of new AIDS drugs.

Available in PDF, EPUB and Kindle. Book excerpt: "Although there is debate about the estimated health burden of rabies, the estimates of direct mortality and the DALYs due to rabies are among the highest of the neglected tropical diseases. Poor surveillance, underreporting in many developing countries, frequent misdiagnosis of rabies, and an absence of coordination among all the sectors involved are likely to lead to underestimation of the scale of the disease It is clear, however, that rabies disproportionately affects poor rural communities, and particularly children. Most of the expenditure for post- exposure prophylaxis is borne by those who can least afford it. As a result of growing dog and human populations, the burden of human deaths from rabies and the economic costs will continue to escalate in the absence of concerted efforts and investment for control. Since the first WHO Expert Consultation on Rabies in 2004, WHO and its network of collaborating centres on rabies, specialized national institutions, members of the WHO Expert Advisory Panel on Rabies and partners such as the Gates Foundation, the Global Alliance for Rabies Control and the Partnership for Rabies Prevention, have been advocating the feasibility of rabies elimination regionally and globally and promoting research into sustainable cost-effective strategies. Those joint efforts have begun to break the cycle of rabies neglect, and rabies is becoming recognized as a priority for investment. This Consultation concluded that human dog-transmitted rabies is readily amenable to control, regional elimination in the medium term and even global elimination in the long term. A resolution on major neglected tropical diseases, including rabies, prepared for submission to the World Health Assembly in May 2013 aims at securing Member States' commitment to the control, elimination or eradication of these diseases. Endorsement of the resolution would open the door for exciting advances in rabies prevention and control."--Publisher's description.

Available in PDF, EPUB and Kindle. Book excerpt: The striking upsurge in population growth rates in developing countries at the close of World War II gained force during the next decade. From the 1950s to the 1970s, scholars and advocacy groups publicized the trend and drew troubling conclusions about its economic and ecological implications. Private educational and philanthropic organizations, government, and international organizations joined in the struggle to reduce fertility. Three decades later this movement has seen changes beyond anyone's most optimistic dreams, and global demographic stabilization is expected in this century. The Global Family Planning Revolution preserves the remarkable record of this success. Its editors and authors offer more than a historical record. They disccuss important lessons for current and future initiatives of the international community. Some programs succeeded while others initially failed, and the analyses provide valuable guidance for emerging health-related policy objectives and responses to global challenges.

Book excerpt: Malnutrition -- in the form of undernutrition, micronutrient deficiencies, and overweight and obesity -- imposes unacceptably high economic and social costs on countries at all income levels. The causes of malnutrition are complex, yet all forms of malnutrition share one common feature: nutritionally inappropriate diets. The State of Food and Agriculture 2013 makes the case that healthy diets and good nutrition begin with food and agriculture.
