Book excerpt: The health care system in the United States has a safety net to provide care for the millions of Americans without health insurance and lack access to care. The safety net system includes hospital emergency departments, federally qualified health centers, and community clinics. With the costs of health care rising, ways to utilize and maximize the effectiveness of safety net providers is critical. One way to conserve costs involves preventing those without insurance from using the emergency room as a primary care provider. This study surveyed two uninsured groups, one using a local free clinic for primary care (n = 54) and one from the community that did not use the free clinic (n = 70). The uninsured group that used the free clinic had a statistically significant lower average of emergency room visits than the uninsured group that did not use the free clinic. The results suggest that providing primary care for the uninsured decreases emergency room usage.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: In the United States (US), where access to and affordability of healthcare remains a critical challenge, emergency departments (ED) have increasingly served as a source for primary care and a portal to hospital admissions. Although barriers to timely primary care and lack of insurance are considered influential factors, the inter-relationship between these issues and ED use is not well-understood. We, therefore, examined the relationship between self-reported insurance status, perceived barriers and ED use among non-institutionalized US adults. We analyzed data from 378,502 participants of the 1999 to 2011 National Health Interview Survey. The following were considered as barriers to timely primary care as experienced during the past year: unable to get through on telephone, unable to obtain appointment soon enough, long wait in the physician's office, limited clinic hours, and lack of transportation. Individuals were classified by source of insurance into Private, Medicare, Medicaid, Other insurance, and No insurance. National estimates were obtained using appropriate sampling weights. Logistic regression models were used to estimate odds ratios (OR) and 95% confidence intervals (CI) for [greater then/equal to] 1 ED use and to test for the interaction between self-reported barriers and source of insurance. Overall, 10.0% of adults had [greater then/equal to] 1 barrier to care and 20.2% reported [greater then/equal to] 1 ED visit in the past year. Insurance profile was as follows: Private (57.8%), Medicare (17.7%), Uninsured (16.3%), Medicaid (5.0%), and Other Insurance (2.6%). Compared to those without any barrier, adults who experienced [greater then/equal to] 1 barriers were more likely to report [greater then/equal to] 1 ED visits in the past year (32.6% vs. 18.8%). In multivariate models that included both barriers and source of insurance, the odds of an ED visit was greater among adults with [greater then/equal to] 1 vs. no barriers (OR [95%CI] - 2.01 [1.95-2.07]), and varied across source of insurance (OR [95%CI]-Private: reference; Medicare: 1.76 [1.72 - 1.81]; Medicaid: 3.11 [3.00-3.23]; Other: 1.83 [1.73 - 1.94]; Uninsured: 1.26 [1.23 - 1.30]). The magnitude of association between barriers and ED use varied across insurance categories. Among non-institutionalized adults in the US, the source of insurance was seen to influence the association between perceived barriers to timely care and ED use. Further analyses are planned to determine whether this relationship persists after controlling for socio-demographic and clinical characteristics.

As Medicaid expansion begins to bring millions of newly eligible individuals into the Medicaid program, this information is invaluable for the decision making of legislators as well as health care leaders around the country.

Since its passage, emergency rooms continue being overcrowded by new Medicaid enrollees. This has created a change in hospital payer mix, which has affected hospital finances (Nikpay et al, 2014). Emergency rooms continue to provide acute care for twenty eight percent of the population (McClelland et al, 2014). The shortage of primary care doctors and locations has further fueled the overcrowding of the ER. A health disparity gap will continue to grow among expansion and non-expansion states as the previously uninsured population start to receive better access to care (Nikpay et al, 2014). As the effects of Medicaid expansion vary from state to state, some state policymakers are using research data to address these issues. This literature review will provide an overview of the findings discovered by empirical research.
