Book excerpt: Abstract: PURPOSE: The purpose of this study was to examine the relationship between strength, functional test outcomes, and perception of readiness and injury rates and severity among intercollegiate gymnasts. SUBJECTS: Nineteen members (6 male, 13 female) of the Springfield College gymnastics teams participated in the study. The male subjects trained for an average of 10.2 + 3.6 months per year and competed 3.8 + 2.1 months per year while the female subjects trained an average of 11.5 2 1.4 months per year and competed 4.4 + 2.3 months per year. METHODS: All subjects were tested before the start of the competitive season, and prior to the occurrence of injury to obtain baseline data. A questionnaire was utilized to collect data on variables related to demographics, history of gymnastics activity, previous injuries, and current perception of readiness to compete. Subjects completed an isokinetic test of quadriceps and hamstring strength at three speeds, and three functional tests: A cocontraction test, a vertical leap test, and a single leg hop for time test. One female subject sustained an injury that resulted in the inability to participate for 3 days and as a result met the operational definition of injury. This subject was re-tested 24 hours after clearance by an athletic trainer. STATISTICAL ANALYSES: Descriptive statistics were used to obtain a demographic profile, injury history profile, and sports participation profile. Pearson product-moment correlation coefficient was used to investigate relationships between variables. Linear multiple regression analyses and the coefficients of determination were used to explain variance in isokinetic and functional test outcomes. Independent t-tests were used to identify significant differences between males and females with respect to isokinetic strength or functional test outcomes. Wilcoxon signed ranks tests were used to compare the performances of the six subjects that participated in both the first and second years of this study. RESULTS: Significant differences were found between males and females in the isokinetic tests, and all three functional tests. Strong significant positive relationships were found between the three isokinetic strength speeds. The vertical jump test showed a high positive correlation with all three isokinetic testing speeds. The cocontraction test demonstrated a significant negative correlation with all three isokinetic testing speeds. The 6- meter single leg hop test showed a significant negative correlation with the isokinetic test at 60o/sec. The Wicoxon signed ranks test showed the only significant differences in the performance of the six subjects participating in both years of the study was found in the vertical jump and cocontraction tests. The coefficients of determination showed that each functional test can explain between 22% and 49% of the variance in the isokinetic strength test. CONCLUSIONS: The combination of the three functional tests (vertical jump, 6-meter single leg hop, and cocontraction) may be an adequate substitution for isokinetic strength testing as a measure of rehabilitation status. Future research is needed to establish normative values for the functional tests in different populations as well as determining additional variables that explain the remaining variance in the isokinetic strength testing.

Book excerpt: Abstract: Gymnasts are frequently referred to physical therapy due to a high incidence of injury within the sport. Once an injury has been treated, physical therapists are faced with a lack of research enabling them to determine appropriate timeliness in returning the athlete to participation. Many therapists believe this determination, as well as injury prediction, may be made based on the evaluation of strength. Purpose: The purpose of this study was to examine the relationships between isokinetic strength scores, functional test measures, and incidence of injury in collegiate-level gymnasts. Subjects: Two Springfield College female gymnasts participated in this study. Both participants were 18 years of age, and had been competing in gymnastics for a total of 8 years. Methods: All subjects were tested in October of 2000 and January of 2001. During each testing session, each subject was instructed to complete a demographic questionnaire prior to testing. Lumbar range of motion was then assessed using the BROM IIR. Isokinetic strength testing utilizing the CybexRNorm dynamometer was performed next. Testing protocol consisted of the performance of concentric/eccentric quadriceps contractions at three angular velocities (60/second, 180/second and 300/second). Following the isokinetic strength testing, each subject performed the modified vertical power jump test, co-contraction test, and single leg 6-m hop for time test. Isokinetic testing was not conducted during the final testing session in January 2001 due to mechanical failure of the CybexRNorm unit in October 2000. Analysis: Inadequate sample size left the researchers unable to statistically analyze the gathered data as intended. Descriptive statistics were evaluated to examine measurement trends within and across each testing session. Results: Subject one showed no improvement in performance from baseline testing (October 2000) to final testing (January 2001) except during the co-contraction test, and perception of readiness to compete. Intra-trial trends demonstrated that subject one achieved better performance in later trials (trials 2 and 3) than in earlier trials (trial 1), except in measurements obtained for lumbar extension range of motion and modified vertical jump trials recorded in January, 2001. Analysis of baseline testing information showed that subject two also performed more efficiently during later trials. Due to injury, final testing data could not be gathered for subject two. Conclusion: As a result of a non-correctable mechanical failure in the CybexRNorm Dynamometer and a limited sample size, the researchers were unable to accept or reject the null hypothesis. Further research is required to establish the relationships between isokinetic strength scores, functional test measures, and incidence of injury in collegiate-level gymnasts.

Evaluation of isokinetic strength and voluntary activation testing are both commonly performed to detect inter-limb neuromuscular quadriceps deficits, yet it is unknown if inter-limb deficits in these two measures provide similar information regarding quadriceps function. Objective: Determine if inter-limb deficits differ between measures of isokinetic quadriceps strength and voluntary activation at 30°, 70° and 90° of knee flexion in healthy participants. Secondarily, we sought to determine the relationship between inter-limb deficits in quadriceps strength and voluntary activation. Design: Crossover. Setting: Research laboratory. Participants: Twenty-five participants (11 Males, 14 Females, age=25.08 ±4.67 years, height=172.88±7.74cm, weight=67.04±10.67kg) with no history of knee injury volunteered for the study. Interventions: Independent variables included testing measure (isokinetic strength and voluntary activation) and sex (male and female). The order in which testing measure were performed was randomly assigned. Isokinetic strength was performed at 60°/second through the full range of motion. Voluntary activation was assessed by administering a supramaximal electrical stimulus during a maximal voluntary isometric contraction at each target angle. Main Outcome Measures: Isometric quadriceps strength was determined by extracting torque measurements at 90°of knee flexion, while voluntary activation was measured via the central activation ratio. Inter-limb difference scores for each measure were expressed as an absolute value percentage of the dominant leg ((non-dominant - dominant leg)/ dominant leg). A 2x2 repeated measures analysis of variance was used to determine differences in inter-limb deficits between measures and sex. The current investigation was designed to determine the relationship between the hamstrings to quadriceps (H/Q)strength ratio and endurance exercise performance in trained female runners as measured by V02peak, running economy, lactatethreshold, and 10 K running time. Ten female runners (V02peak > 53 mL-1/kg-1/min) completed three exercise testing sessionsconsisting of a Modified McConnell V02peak test, isokinetic lower extremity strength test, and 10 K time trial. Blood lactate wasmeasured through capillary blood from the tip of the finger pre exercise, at every 3 min during exercise, and immediately post exerciseduring the V02peak test to determine lactate threshold. All V02peak tests were videotaped and stride rate and stride length weredetermined at 3.13 m/s, 3.58 m/s, and 4.02 m/s. No significant correlations were observed between the H/Q strength ratio at 60 0 /sand 180 0 /s and any of the performance variables. No significant correlations existed between the H/Q strength ratio at 60 0 /s and180 0 /s and stride rate and stride length at any speed. The training adaptations of endurance running could result in an increased H/Qstrength ratio. At high levels of competition, the H/Q strength ratio may not be the main determinant of performance. Since therelationship between the H/Q strength ratio and endurance running performance remains inconclusive, future research is warranted toinvestigate the functional H/Q strength ratio and performance during different running distances.

Book excerpt: Abstract: PURPOSE: The purpose of this study was to determine the magnitude of isokinetic strength differences between high school and masters level rowers. A secondary purpose was to determine the magnitude of isokinetic strength difference between male and female rowers. MATERIALS/METHODS: Thirteen volunteers between the ages of 15 and 54 who were members of a crew team (either high school or club) participated in this study. Subjects included both males and females and were obtained through convenience and snowball sampling methods. Inclusion criteria included: age of 60 years or younger, subjects must row 2 or more times per week during the in-season and must row 2 or more times per week or exercise at least 90 minutes per week during the off-season. Exclusion criteria included: lower extremity injury occurring within 6 months prior to data collection, lower extremity orthopedic surgery within a year prior to data collection or a serious medical condition that may place themselves or others at risk. Each subject completed a demographics/medical questionnaire and informed consent form. At the time of data collection, an investigator reviewed the forms to determine the subject's eligibility. Once cleared for participation, the subject's height, weight and blood pressure was measured. The subject completed a five-minute warm up on a cycle ergometer and stretches. The subject was then positioned on the isokinetic dynamometer and completed the testing protocol. Quadriceps and hamstrings peak torque and quadriceps/hamstring ratio was measured at angular velocities of 60, 180 and 240 degrees/second (deg/sec). Finally, the subject completed a cool down, which mimicked the warm up. RESULTS: No significant difference was found between male masters and high school subjects for hamstrings mean peak torque/body weight at 60 (t= -0.98, p=0.71), 180 (t=0.55, p=1.19) or 240 deg/sec (t=0.18, p=1.72), for quadriceps peak torque/body weight at 60 (t= -0.08, p=1.87), 180 (t=0.29, p=1.56) and 240 deg/sec (t=-0.05, p=1.92), or for quadriceps/hamstring ratio at 60 (t=0.64, p=1.08), 180 (t=1.28, p=0.47) and 240 deg/sec (t=1.06, p=0.64). Between male and female masters rowers, there was a significant difference found for hamstring mean peak torque/body weight at angular velocities of 180 (t=3.32, p=0.04) and 240 deg/sec (t=4.33, p=0.02). There was also a significant difference found for quadriceps mean peak torque/body weight at angular velocities of 180 (t=5.78, p=0.004) and 240 (t=4.82, p=O.OI) deg/sec. No significant difference was found at 60 deg/sec for either quadriceps (t=3.02, p=0.06) or hamstring (t= 2.03, p=0.20) peak torque/body weight. There was no significant difference found between male and female masters rowers for quadriceps/hamstrings ratio at 60 (t= 0.37, p=1.49), 180 (t=0.54, p=1.23) or 240 deg/sec (t=0.28, p=1.58). A post study power analysis indicated that only comparisons between males and females for quadriceps peak torque/body weight at all three speeds and hamstring peak torque/body weight at 180 and 240 degrees/sec achieved a power of 0.80 or higher. All other comparisons failed to reach a power of .80. CONCLUSIONS: The magnitude of differences varied greatly between subjects. The results of this study are consistent with the literature regarding the strength differences among men and women. Due to the small sample size of participants (along with other limitations), further research is warranted to be able to generalize the results of this study to a larger population. CLINICAL RELEVANCE: It is important for physical therapists, athletic trainers, coaches and athletes to be aware of the physiological changes with occur with age and the possible effects related to function and performance.
