Prior to the 1900s, this interest was somewhat episodic. Because of anxiety about infection, early attempts flourished only briefly and then were no more. In the twentieth century, however, as a result of renewed interest in intrauterine contraception, particularly in the developing countries, a number of individuals throughout the world began experiment ing with a variety of new intrauterine devices. Since then, a great number of these devices have been studied; a few have survived careful scrutiny, and IUDs now represent the second most commonly used form of medical contraception. It is estimated that approximately 15 million devices are in use at the present time, 3 to 4 million of them in the United States.

Available in PDF, EPUB and Kindle. Book excerpt: The term 'medical devices' covers a wide range of equipment essential for patient care at every level of the health service, whether at the bedside, at a health clinic or in a large specialised hospital. Yet many countries lack access to high-quality devices, particularly in developing countries where health technology assessments are rare and there is a lack of regulatory controls to prevent the use of substandard devices. This publication provides a guidance framework for countries wishing to create or modify their own regulatory systems for medical devices, based on best practice experience in other countries. Issues highlighted include: the need for harmonised regulations; and the adoption, where appropriate, of device approvals of advanced regulatory systems to avoid an unnecessary drain on scarce resources. These approaches allow emphasis to be placed on locally-assessed needs, including vendor and device registration, training and surveillance and information exchange systems.

This book was released on 1987 with total page 96 pages. Available in PDF, EPUB and Kindle. Book excerpt: The mechanism of action, safety, and efficacy of IUDs were reviewed by a WHO Scientific Group in 1986. The Scientific Group concluded that the IUD should continue to be supported, in both developed and developing countries, as a safe, reliable method of fertility regulation. The newer copper-releasing devices are comparable to oral contraceptives in terms of safety and efficacy. When compared to women who use other reversible methods of contraception, IUD users have the lowest mortality resulting from deaths directly attributable to those methods or to the consequences of unwanted pregnancy. In the past decade, research has concentrated on the development of new devices that have both higher continuation rates and lower rates of expulsion and removal for bleeding abnormalities. An important recent concern has been the possible increased risk of pelvic inflammatory disease (PID) and subsequent tubal infertility associated with IUD use. However, it now appears that methodological problems have caused the IUD-associated risk of PID to be overestimated. The increased risk with IUDs seems to be limited to the 1st 4 months of use. No increased risk of tubal infertility has been found among IUD users in stable, monogamous sexual relationships. The use of a copper IUD after the 1st pregnancy is not associated with secondary infertility due to tubal disease. Finally, the newer copper IUDs have low rates of ectopic pregnancy.

Book excerpt: This document is one of two evidence-based cornerstones of the World Health Organization's (WHO) new initiative to develop and implement evidence-based guidelines for family planning. The first cornerstone, the Medical eligibility criteria for contraceptive use (third edition) published in 2004, provides guidance for who can use contraceptive methods safely. This document, the Selected practice recommendations for contraceptive use (second edition), provides guidance for how to use contraceptive methods safely and effectively once they are deemed to be medically appropriate. The recommendations contained in this document are the product of a process that culminated in an expert Working Group meeting held at the World Health Organization, Geneva, 13-16 April 2004.

Book excerpt: In this brutal indictment of the birth control industry, Nicole J. Grant documents the history of the Dalkon Shield case and places the controversy within a larger historical and social context, examining the ways in which social conditions affect women's choices about contraception. Drawing upon an analysis of thirty years' worth of articles in medical journals and popular magazines, family planning literature, medical texts, government documents, and transcripts from interviews with women discussing procreative choices, Grant examines the IUD's place in the history of contraception and the politics of bringing the Dalkon Shield to the market, including the commercial bias of the device's inventor and chief medical consultant. She discusses the dissemination of false and misleading advertising about the device, the manufacturer's disregard of evidence suggesting that it was unsafe, and the stake of various family planning agencies in promoting its use. She goes on to examine the social conditions under which women chose to use the Dalkon Shield and considers the fact that many women, wishing to exercise control over procreation, freely selected it over alternative forms of contraception, even though they knew there were associated risks. According to Grant, the Dalkon Shield case is not a historical anomaly. Today, advances in technology are making new methods of contraception available to women, but these methods carry risks as serious as those associated with the Dalkon Shield. Moreover, the social context in which women are making decisions about contraception is much the same as it was during the time when the Dalkon Shield was widely used. Grant concludes her study by proposingways in which women can enhance their informed risk/benefit analysis of all options while maintaining their autonomy.
