Book excerpt: Objectif : analyser l'articulation de la prise en charge de la douleur chronique à partir du cas clinique d'un patient jeune, lombalgique chronique, dont la prise en charge a été complexe du fait d'un nomadisme médical, associé à une escalade thérapeutique ayant menée à une dégradation de sa qualité de vie et de sa santé. Méthodes : étude qualitative basée sur l'analyse d'un cas clinique par entretiens semi-dirigés auprès du patient, de sa compagne et des professionnels de santé (médecins généralistes, pharmaciens, algologue) l'ayant pris en charge. Une analyse lexicale puis thématique ont été réalisées à partir du logiciel N'Vivo. Une revue de la littérature a été réalisée concernant les recommandations en vigueur au moment de la prise en charge. Résultats : le cas clinique a mis en évidence certains drapeaux jaunes, un nomadisme médical et pharmaceutique importants. L'analyse lexicale soulignait la prépondérance de la douleur, de la morphine et du médecin dans la prise en charge confortant la notion de médicalisation de la douleur. L'analyse thématique révèle un axe médicamenteux surinvesti aux dépends des autres axes. La morphine a été majorée rapidement et à des doses très élevées pour des douleurs chroniques non cancéreuses, sans bénéfice sur la douleur et la fonction voire responsable d'une altération de la qualité de vie. Le manque de communication, le nomadisme médical et paramédical pouvaient expliquer le manque d'action malgré le repérage des signes d'alerte dans tous les entretiens. La difficulté de suivre les recommandations a été évoquée par les professionnels de santé. La multiplication des avis et des prises en charge diverses expliquaient le recours tardif aux spécialistes de la douleur. Discussion : l'articulation des soins est apparue difficile du fait d'un nomadisme médical et paramédical et à l'origine d'une information parcellaire pour chaque intervenant. La coordination des soins entre médicaux et paramédicaux, est apparue primordiale, mais aussi entre les professionnels de ville et hospitaliers. Cela pourrait faire l'objet d'un travail sur la mise en place de réunions de concertations pluridisciplinaires ville-hôpital, en prenant en compte les contraintes de chaque exercice (temps, rémunération,...), afin d'améliorer la communication nécessaire pour une articulation des soins efficace.

Le repos était auparavant le traitement principal, alors qu'aujourd'hui l'activité physique en est la clé. Un mouvement sociétal est né autour de la course à pied. Les forces de compressions et rotations axiales de la colonne vertébrale en courant, demeurent encore associées à des croyances populaires négatives. L'objectif de cette étude, est de croiser les regards des patients et des médecins généralistes, sur les croyances autour de la lombalgie et course à pied. Méthode : Étude observationnelle, descriptive, quantitative, basée sur un questionnaire en ligne à destination des médecins généralistes des Hauts-de-France, et de leurs patients. Résultats : 174 patients et 133 médecins généralistes ont participé à l'étude, ils s'accordent tous sur le fait que l'activité physique fait partie du traitement de la lombalgie. 100% des médecins la recommandent d'ailleurs en cas de lombalgie chronique. Les impacts de la course à pied gardent un caractère traumatique pour les patients, et restent associés à des lésions de la colonne vertébrale (hernie discale, tassements, arthrose). La douleur demeure un signe de gravité dans leur esprit. Le fait de posséder une licence en club de sport, est statistiquement associé à une diminution des croyances négatives chez les patients, mais pas chez les médecins. Conclusion : L'opinion des médecins généralistes et des patients a évolué ces dernières années, et s'accorde désormais aux recommandations de prise en charge. Certaines croyances demeurent infondées, et des messages clés sont encore à faire passer. Le poster créé peut être un bon outil. La revue de la littérature est rassurante, la course à pied constitue un moyen de prévention contre l'ostéoporose, en augmentant la densité minérale osseuse et renforce le disque intervertébral. Des études complémentaires sont nécessaires pour quantifier l'effet dose-réponse pour cela.
