Néanmoins, lorsque l'espace prothétique est réduit, des alternatives aux reconstructions osseuses existent : il s'agit de l'utilisation d'implants courts ou de la réalisation d'une latéralisation du nerf alvéolaire inférieur. Afin de faciliter le choix de la technique de reconstruction à adopter, nous avons procédé à une revue de la littérature sur les différentes techniques d'augmentation osseuse verticale, ainsi que sur leurs alternatives thérapeutiques. Les indications, limites et morbidité de chaque méthode ont été évaluées, ainsi que leurs résultats en termes de gain vertical taux de résorption, taux de survie et taux de succès implantaire. Nous en ressortons les avantages et inconvénients des différentes techniques, ainsi que les critères décisionnels à prendre en compte lors du choix thérapeutique."

Pour permettre une implantation stable et sûre, plusieurs techniques ont été développées : greffes osseuses autologues, déroutement du nerf alvéolaire inférieur, distraction alvéolaire, et régénération osseuse guidée.Les autogreffes constituent à l’heure actuelle la technique de référence. En marge des greffes en onlay classiquement décrites dans la littérature, nous rapportons ici une série de 18 greffes d’interposition dans les secteurs mandibulaires postérieurs. Nous avons montré que cette technique permettait un gain de hauteur moyen de 7,9 mm à six mois, avec un faible taux de résorption (18% contre 41% pour les greffes en onlay). La complication la plus fréquemment observée était l’hypoesthésie du nerf alvéolaire inférieur (55% des cas à 1 mois), régressive dans 70% des cas. Nous avons relevé quatre échecs, correspondant à deux greffes d’interposition bilatérales.Le délai moyen avant implantation était de 6 mois 1⁄2, et le taux de succès après un recul moyen de 14 mois était de 94%, correspondant aux taux observés avec les autres techniques. Au total, la greffe d’interposition mandibulaire postérieure est une technique efficace dont les complications peuvent être évitées si l’on en respecte les indications.
