Although this may not have resulted in a dramatic breakthrough in our understanding of the condition, at least we are now more aware of our ignorance. Possibly this partly explains the increasing medical concern for, and sympathy in, the management of the obese. In the introductory chapter, John Garrow says that he believes it would be better to treat a few people well than many subjects unsuccessfully. This theme is developed in those chapters which deal with specific forms of therapy including exercise, protein-sparing fasting and bypass surgery, while the chapters on pharmacological agents review our knowledge, and our ignorance, of the mode of action and efficacy of the currently available drugs. Almost in contrast, the chapter on behavioural therapy and group therapy suggests that obesity is a problem which could best be tackled on a community basis. Clearly such an approach is very attractive, and the combination of behavioural therapy and slimming organizations would appear to offer the best prospects of controlling most subjects' weight problems. There will always remain, however, the individual patient in whom there will be a place for the various special techniques now available. It is hoped that this book will contribute to a greater understanding of the various problems faced by the obese, and the regimes that can be offered. J. F. Observations from medi cal, psychological, sociological, and anthropological perspectives on the healthy and sick woman through the various stages of her life presented a wide spectrum of scien tific research. Sessions covering such fundamental topics as the problems of the young mother, women in the postgenerative phase, and the early mother-child relationship were enriched by many recent scientific contributions. Further subjects, ranging from the dialogue in the gynecologist's consulting room through the emotional situation of the gynecologist, body language in female sexuality, and sexual disorders experienced by gynecological patients to psychosomatics and cancer, were pursued in small workshops. Of particular psychoendocrinological interest were the contributions on obesity and the premenstrual syndrome. The results of this workshop and another on unwanted pregnancy have ap peared recently in separate monographs.

The scheme depicted in Fig. 1. 1 illustrates the fate of energy in the body as seen by the nutritionist. After allowing for losses of energy in faeces and urine, the metabolizable energy obtained from food is utilized for main taining and increasing body energy content (maintenance, external work, growth and production). The transformation of metabolizable energy into these forms of net energy also involves inevitable energy losses in the form of heat - thermic energy. Similarly, maintaining homeothermy in cold en vironments involves shivering and non-shivering thermogenesis (NST) and the energy costs of assimilating nutrients and retaining net energy results in obligatory heat losses due to diet-induced thermogenesis (DIT). This obligatory DIT is mainly due to the energy cost of protein and fat synthesis but, in addition to this, there is an adaptive component of DIT that helps maintain body energy content (i. e. body weight) by dissipating the metabolizable energy consumed in excess of the requirements for maintenance, growth and production.
