Book excerpt: Over the past two decades it has been increasingly recognized that whole-body ultrasound is an invaluable tool in the critically ill. In addition to offering rapid whole-body assessment, it has the advantage of being a bedside approach that is available at all times and can be repeated at will. Accordingly, it permits the immediate institution of appropriate therapeutic management. Whole-Body Ultrasound in the Critically Ill is the sequel to the author’s previous books on the subject, which were first published in French in 1992 and 2002 and in English in 2004. This new volume reflects the latest state of knowledge by including a variety of improvements, revised definitions, and updated algorithms. Findings in respect of individual organs are clearly presented, and a particular feature is the in-depth coverage of the lungs, traditionally regarded as an area unsuitable for ultrasound. Throughout, the emphasis is on the practical therapeutic impact of the technique. Its value in a variety of settings, including unexplained shock, management of hemodynamic instability, acute respiratory failure (the BLUE protocol), and the critically ill neonate, is carefully explained. Interventional ultrasound and less widely recognized applications, such as mesenteric infarction, pneumoperitoneum, and intracranial hypertension, are also described. Pitfalls of the technique receive due attention. Today, whole-body ultrasound touches upon every area of critical care. This book, from the chief pioneer in the field, shows that the technique enables critical care physicians to detect therapeutically relevant signs easily and quickly. It will serve as an invaluable guide to the practice of a form of visual medicine.

Available in PDF, EPUB and Kindle. Book excerpt: Nine years have passed since the first edition of Care of the Critically III Patient was published. As its editors Maurice Rapin and I were fortified by the many positive and helpful comments of reviewers and readers: the work appeared to have fulfilled our original aims and objectives. Intensive care medicine has, of course, advanced and changed in many ways since 1982 and it therefore seemed appropriate to consider the preparation of a second edition. Encouraged by the reception of the first and the timely prompting of the publishers we began, in 1986, to prepare for a new edition. Caring for critically ill patients is of universal concern and we were therefore delighted when Warren Zapol joined us as C'n editor widening our international base. The three of us subsequently spent many happy and productive times together planning the content, priorities, scale and contributions for the new edition. A great tragedy then overcame us: Maurice Rapin died suddenly. His death deprived us of a true friend and colleague who had been an international pioneer during the development of intensive care medicine. I will always remember, with affection and professional pride, the many hours he and I worked together in preparing the first edition of this book and also as founding editors of the now firmly established journal Intensive Care Medicine. An obituary, written by Warren Zapol, that appeared in the journal follows the Preface.

Book excerpt: Only very few therapeutic modalities are used as extensively as mechanical ventilation in intensive care units, during anaesthesia and in emergency situations. Hence theoretical and practical knowledge in this technique had to be made available to workers in a number of medical specialities. In addition to anaesthetists, who are most familiar with artificial ventilation for historical and practical reasons, surgeons, internists, paediatricians and emergency physicians also need a foundation. Furthermore, the widespread application of this life-supporting method requires that paramedical personnel such as nurses and respiratory therapists be trained to use mechanical ventilation, to understand how it works and to be aware of specific side effects and dangers. This book, edited by Frañois Lemaire, is a well-designed present ation of a number of the relevant aspects, types and problems of mechanical ventilation which are important for physicians and paramedical personnel who use it. After a description of the technical principles and maintenance of an artificial ventilator, the main part of the book is devoted to the most frequently used types of mechanical respiratory support, with their specific indications, the pathophysiology of their effects on pulmonary gas exchange and the specific choice and regulation ofthe mechanical variables involved. Older and new types of ventilatory support are discussed; there is a good balance of enough specific information for the inexperienced as well as a critical analysis of the indications for more exotic techniques, such as mandatory minute ventilation, independent lung ventilation and airway pressure release.

Available in PDF, EPUB and Kindle. Book excerpt: Intensive Care Medicine has been continuously growing and expanding, culturally, technically and geographically. Monitoring and instrumentation are continuously improving and more and more hospitals are getting Intensive Care facilities. The costs have proportionally increased over the years, so that ICUs represent today a major cost for health structures. Since the available resources are limited, a real need is emerging to set the limits and indications of Intensive Care. It is understood that the problem not only involves medical considerations, but also ethical and economical aspects of the utmost importance. For the first time in Europe, this book edited by Reis Miranda and his colleagues tackles systematically the many structural aspects of the European Intensive Care. The organisation and financing of health care in the Old Continent is deeply different from the American one, and the results and consequent proposals obtained in the USA cannot simply be transferred to this side of the Atlantic Ocean. Weare extremely pleased to welcome this first European attempt to discuss the Intensive Care problem. It lays no claims to giving definite replies in a continuously developing field, but it will surely become the basis for future discussions and proposals. I am particularly happy that this work has mainly developed within the European Society of Intensive Care, whose final target is to ensure a common standard of therapy in our old Europe, beyond national differences. We warmly congratulate the authors, and I am sure that their work will find wide diffusion and consent.

Available in PDF, EPUB and Kindle. Book excerpt: Written by a pioneer in critical care ultrasound, this book discusses the basic technique and “signatures” of lung ultrasound and explains its main clinical applications. The tools and clinical uses of the BLUE protocol, which allows diagnosis of most cases of acute respiratory failure, are first described in detail. Careful attention is then devoted to protocols derived from the BLUE protocol – the FALLS protocol for diagnosis and management of acute circulatory failure, the Pink protocol for use in ARDS, and the SESAME protocol for use in cardiac arrest – and to the LUCI-FLR program, a means of answering clinical questions while reducing radiation exposure. Finally, the book discusses all the possible settings in which lung ultrasound can be used, discipline by discipline and condition by condition. Lung Ultrasound in the Critically Ill comprehensively explains how ultrasound can become the stethoscope of modern medicine. It is a superb complement to the author’s previous book, Whole Body Ultrasonography in the Critically Ill.

Book excerpt: Continuous update in critical care medicine is a real challenge due to the growing dimensions of its contents; these elements are the result of new research acquisitions, and of those clinical situations where the physician is able to intervene at a given moment, with the use of effective prevention techniques. They are also the result of new technologies able to define complex sub-clinical diagnostic aspects; lastly, they are the result of effective therapeutic techniques available, and of treatment strategies able to influence radically and positively the patient's clinical course. Critical care medicine is playing a key role in the most advanced environments, as it represents a cross-sectional field of action, involving several specialties, such as anesthesiology, general medicine, surgery, pediatrics.

Book excerpt: Authoritative investigators active in the discovery, development, and application of biological anti-infective agents concisely review their use and potential in preventing and treating human disease. Focusing on biotherapeutic entities that have been tested in controlled studies, the prominent experts illuminate the scientific underpinnings of their therapeutic power, assess their possible risks in the treatment of infectious diseases, and outline the research needed to better define their effectiveness. In addition, they also consider how biotherapeutic agents may be genetically engineered for maximum intestinal and vaginal production of bioactive substances in vivo. Biotherapeutic Agents and Infectious Diseases brings together all the evidence needed to understand and capitalize on the considerable promise of this significant new class of biotherapeutic entities.
