Book excerpt: La radiostérilisation est un procédé de stérilisation possédant des caractéristiques intéressantes (traitement des produits thermosensibles, sans résidu, stérilisation terminale, la dose reçue suivie par dosimétrie). Il parait être une bonne alternative lorsque la stérilisation par la chaleur humide, méthode de référence, n'est pas appropriée au produit. C'est pourquoi les applications sont nombreuses, au niveau terminal, mais aussi tout au long de la fabrication des médicaments. Deux technologies permettent la stérilisation par irradiation : les sources de rayons gamma, et les accélérateurs de faisceau d'électrons. Récemment, des systèmes de rayons X provenant d'électrons accélérés ont été introduites afin d'allier souplesse d'utilisation et fort pouvoir de pénétration. La condition la plus importante pour le centre de stérilisation est d'avoir son processus sous contrôle. Cela signifie que les paramètres de processus doivent être soigneusement établis, surveillés et contrôlés. La détermination de la dose stérilisante est l'étape clé de la mise en oeuvre de cette technique de stérilisation. Globalement, deux approches peuvent être envisagées : soit l'application d'un dose de 25 kGy ou 15 kGy est sélectionnée et justifiée par la méthode VDmax (méthode la plus simple). Soit une dose plus faible est déterminée par les méthodes 1 ou 2 (plus complexes, demandant plus d'essais). En routine, la dosimétrie permettra de surveiller et s'assurer que les doses reçues sont conformes. La détermination des biocharges et les audits de dose, offrent un contrôle supplémentaire de l'éfficacité du processus. La maitrise du procédé permet ainsi de garantir la qualité du produit en vue de la sécurité du patient.
