Book excerpt: Introduction : L'Hypertension artérielle est le plus fréquent des facteurs de risque cardio-vasculaire dans le monde. La mesure à domicile tend à se développer de plus en plus. On a ainsi mis en évidence chez certains patients une hypertension artérielle masquée (normale au cabinet, élevée au domicile). Méthodes : Il s'agit d'une revue systématique de la littérature des dix dernières années en utilisant les bases de données MEDLINE et PASCAL. Les études ont été lues à plusieurs reprises, pour chaque étude il a été déterminé quels critères méthodologiques (issus des recommandations ESH/ESC 2013 pour la plupart) ont été ou non respectés. Résultats : 36 études ont été incluses et analysées. Concernant la mesure de la pression artérielle au cabinet, le taux de respect des critères issus des recommandations était de 44%, la médiane de 44%. Concernant la réalisation de la mesure ambulatoire de la pression artérielle, le taux de respect des critères issus des recommandations était de 26%, la médiane était à 30%. Et enfin concernant l'auto-mesure tensionnelle, le taux de respect des critères issus des recommandations était de 49% et la médiane à 53%. D'une façon globale, le taux de respect des critères issus des recommandations était de 37% avec une médiane à 38%. Discussion et conclusion : Ces relativement faibles taux de respect des recommandations dans la littérature peuvent être expliqués parfois par des contraintes financières ou de temps disponible. Ils sont également expliqués par une mauvaise formation méthodologique des opérateurs et l'absence d'informations écrites données aux patients inclus dans les études. On peut aussi mettre en cause les contraintes d'espace pour la rédaction des articles qui peuvent obliger les auteurs à ne pas donner toutes les informations. Néanmoins nos résultats invitent à lire les études publiées avec circonspection en étant attentif aux méthodes.

La prévalence de l’HTA augmente significativement avec l’âge et la proportion de patients âgés dans la population s’accroît régulièrement. En raison d’une altération du baroreflexe au cours du vieillissement la variabilité tensionnelle augmente avec l’âge. L’évaluation du vrai niveau tensionnel au cours de la consultation s’avère difficile à faire chez la personne âgée. Elle expose au risque de surestimation de la PA (effet blouse blanche) et de sous-estimation de la PA (HTA masquée). Dans ce cadre, les nouvelles recommandations de l’HAS soulignent la nécessité d’utiliser de façon quasi systématique des méthodes de mesure de la pression artérielle à domicile (automesure ou holter tensionnel sur 24 heures) chez le sujet âgé avant d’affirmer ou d’infirmer le diagnostic d’hypertension artérielle et de dépister l’HTA blouse blanche. La prévalence de l’HTA blouse blanche et de l’HTA masquée reste mal connue après 70 ans. L’objectif de notre travail a été d’évaluer la prévalence de l’HTA blouse blanche, et de l’HTA masquée dans une population de sujets âgés de plus de 70 ans venus consulter pour un bilan gérontologique. La recherche des principaux déterminants de ces deux entités a aussi été réalisée. Les résultats de notre travail ont permis de retrouver en cas d’HTA lors de la consultation une prévalence l’HTA blouse blanche de 30%. De même, devant une pression artérielle normale une HTA masquée a été observée dans 45% des cas. Les facteurs déterminants de l’HTA blouse blanche sont un âge moins élevé et une PAS moins élevée. Le principal déterminant de l’HTA masquée est un niveau tensionnel proche du seuil de 140 mmHg lors de la mesure de consultation. En conclusion, la mesure de consultation évalue mal le niveau tensionnel des personnes âgées de plus de 70 ans (risque de surestimation dans 30% des cas ou sous-estimation dans 45% des cas). Les résultats de ce travail justifient la réalisation systématique d’une automesure tensionnelle avant d’introduire ou de modifier un traitement antihypertenseur en raison du risque iatrogène mais aussi afin de dépister les sujets ayant une HTA masquée.
