Book excerpt: En dehors de la transplantation rénale qui est reconnu comme le meilleur traitement de l'insuffisance rénale terminale, plusieurs modalités de suppléance sont possibles quand la maladie évolue. Le processus de décision médicale partagée entre le patient et les professionnels de santé est recommandé par les autorités au stade terminal de la maladie rénale. Comment, dans ce contexte de parcours de soin qui rassemble des recommandations à soutenir le patient lors de ce choix, et des connaissances scientifiques qui démontrent une supériorité de la greffe sur les autres traitements, le patient prend-il part à la décision d'être greffé ? Cette recherche vise à explorer l'expérience des patients et les facteurs qui l'influencent en situation de décision, ainsi que d'évaluer l'impact de cette expérience sur leur devenir. Elle s'appuie sur deux méthodes mixtes de recherche (QUALI > quanti). Elle allie une analyse phénoménologique interprétative et une étude observationnelle mesurant le conflit décisionnel perçu par les patients en attente de greffe et les facteurs qui influencent le regret décisionnel, la qualité de vie et l'observance après la transplantation. Les résultats rapportent que les patients perçoivent l'inscription sur le fichier comme une étape obligatoire. Par ailleurs cette décision implicite forge le rapport des patients aux autres décisions et influence leur capacité d'adaptation vis-à-vis de l'incertitude inhérente à la maladie rénale chronique. L'enjeu de prendre en compte toutes les étapes de la décision médicale partagée est majeur en contexte de transplantation rénale pour soutenir la participation du patient sur l'ensemble de son parcours de soins.
