Les medecins generalistes connaissent les specificites classiques de la medecine interne : medecine du diagnostic et de la prise en charge du symptome inexplique ; medecine globale dans les cas de polypathologie ; medecine des maladies systemiques mais aussi medecine des "niches" (incidentalomes, maladie orphelines). Les medecins generalistes sont satisfaits du service rendu (94%). Ils insistent sur l'importance de la transmission rapide des courriers (74%). Ils souhaitent la transmission de donnees recentes de la litterature par les nouvelles techniques de l'information : 71 % pour les listes de diffusion, 63 % pour les forum. La demande d'avis par courrier electronique recoit un accueil plus mitige (55 %). Surtout, la possibilite de consultations rapides, a leur demande, est souhaitee a 94 %. Ces constatations sont tout a fait en accord avec notre projet d'un service qui garde les valeurs de la medecine interne et qui se veut un service de "medecine interne generale". Elles nous confirment que la consultation reste l'acte principal et fondamental de l'interniste et que son acces doit etre facile.

Conclusions : Selon les intervenants sondés, cette forme de SASPAS présente un intérêt pédagogique dans la formation de l'interne en médecine générale. Elle paraît profitable pour le développement des compétences de l'interne et du futur généraliste en exercice. De plus, les internes seraient enclins à réaliser ce type de stage. Elle présente néanmoins un frein quant à sa mise en œuvre et à l'organisation dans les secteurs de spécialité

La médecine interne, par sa vision globale, représente idéalement la continuité hospitalière de la médecine générale ambulatoire. Mais ses contours ont évolué au cours du temps. Les objectifs de cette étude étaient d'analyser les attentes des médecins généralistes vis-à-vis de la médecine interne (en général et plus particulièrement vis-à-vis du service du CHU de Saint-Étienne), de savoir si elles s'étaient modifiées dans les deux dernières décennies, ainsi que d'évaluer la qualité de la communication entre les deux spécialités. Matériels et méthodes. Nous avons réalisé une étude observationnelle descriptive et quantitative auprès des médecins généralistes de la Loire, avec un questionnaire en ligne diffusé par l'Ordre départemental des médecins de la Loire et le Département universitaire de médecine générale de Saint-Étienne entre octobre et décembre 2018. Résultats. Cent trente-deux questionnaires ont été analysés, soit 21% des envois. Les médecins généralistes privilégiaient le recours aux internistes pour les pathologies inflammatoires et auto-immunes, ainsi que pour l'hématologie non maligne, les troubles psychosomatiques, les maladies génétiques, la gestion des incidentalomes et des symptômes généraux. Ces attentes ont peu changé en 20 ans, mais certains motifs de recours liés aux orientations historiques du service du CHU (endocrinologie, prise en charge du risque vasculaire) ont disparu. Les délais de rendez-vous de consultation et d'hospitalisation étaient estimés trop longs. Pour favoriser les échanges professionnels, la mise en place d'une ligne téléphonique directe dédiée et d'une adresse électronique sécurisée étaient souhaitées. Les comptes rendus hospitaliers étaient jugés de bonne qualité et pédagogiquement utiles, mais leur réception était jugée comme parfois trop tardive. Les praticiens hospitaliers étaient personnellement assez peu connus des généralistes. Conclusion. Les médecins généralistes ligériens sont globalement satisfaits du service de médecine interne pour la gestion des pathologies et problèmes traditionnellement dévolus à la spécialité et de la polypathologie. L'échange avec des interlocuteurs personnellement connus semble souhaité, et pourrait être favorisé par la mise en place de moyens de communication directs.

The social sciences have been accumulating knowledge since the early fifties as to how we are regulated by others. S. Milgram, S. Ashe, P. Zimbardo and J. Calhoun, detail the vulnerability to being duped and deceived and the difficulty of cooperating when values differ. Murray Bowen, M.D., the first researcher to observe several live-in families, for up to three years, at the National Institute of Mental Health. Describing how family members overly influence one another and distribute stress unevenly, Bowen described both how symptoms and family leaders emerge in highly stressed families. Our brain is not organized to automatically perceive that each family has an emotional system, fine-tuned by evolution and "valuing" its survival as a whole, as much as the survival of any individual. It is easier to see this emotional system function in ants or mice but not in humans. The emotional system is organized to snooker us humans: encouraging us to take sides, run away from others, to pressure others, to get sick, to blame others, and to have great difficulty in seeing our part in problems. It is hard to see that we become anxious, stressed out and even that we are difficult to deal with. But "thinking systems" can open the doors of perception, allowing us to experience the world in a different way. This book offers both coaching ideas and stories from leaders as to strategies to break out from social control by de-triangling, using paradoxes, reversals and other types of interruptions of highly linked emotional processes. Time is needed to think clearly about the automatic nature of the two against one triangle. Time and experience is required as we learn strategies to put two people together and get self outside the control of the system. In addition, it takes time to clarify and define one's principles, to know what "I" will or will not do and to be able to take a stand with others with whom we are very involved. The good news is that systems' thinking is possible for anyone. It is always possible for an individual to understand feelings and to integrate them with their more rational brains. In so doing, an individual increases his or her ability to communicate despite misunderstandings or even rejection from important others. The effort involved in creating your Mindful Compass enables us to perceive the relationship system without experiencing it's threats. The four points on the Mindful Compass are: 1) Action for Self, 2) Resistance to Forward Progress, 3) Knowledge of Social Systems and the 4) The Ability to Stand Alone. Each gives us a view of the process one enters when making an effort to define a self and build an emotional backbone. It is not easy to find our way through the social jungle. The ability to know emotional systems well enough to take a position for self and to become more differentiated is part of the natural way humans cope with pressure. Now people can use available knowledge to build an emotional backbone, by thoughtfully altering their part in the relationship system. No one knows how far one can go by making an effort to be more of a self-defined individual in relationships to others. Through increasing emotional maturity, we can find greater individual freedom at the same time that we increase our ability to cooperate and to be close to others.
