Book excerpt: Two key components of the upcoming health reform in the U.S. are a new regulation of the individual health insurance market and an increase in income redistribution in the economy. Which component contributes more to the welfare outcome of the reform? We address this question by constructing a general equilibrium life cycle model that incorporates both medical expenses and labor income risks. We replicate the key features of the current health insurance system in the U.S. and calibrate the model using the Medical Expenditures Panel Survey dataset. We find that the reform decreases the number of uninsured more than twice and generates substantial welfare gains. However, these welfare gains mostly come from the redistributive measures embedded in the reform. If the reform only reorganizes the individual market, introduces individual mandates but does not include any income-based transfers, the welfare gains are much smaller. This result is mostly driven by the fact that most uninsured people have low income.

Care Without Coverage examines the real consequences for adults who lack health insurance. The study presents findings in the areas of prevention and screening, cancer, chronic illness, hospital-based care, and general health status. The committee looked at the consequences of being uninsured for people suffering from cancer, diabetes, HIV infection and AIDS, heart and kidney disease, mental illness, traumatic injuries, and heart attacks. It focused on the roughly 30 million-one in seven-working-age Americans without health insurance. This group does not include the population over 65 that is covered by Medicare or the nearly 10 million children who are uninsured in this country. The main findings of the report are that working-age Americans without health insurance are more likely to receive too little medical care and receive it too late; be sicker and die sooner; and receive poorer care when they are in the hospital, even for acute situations like a motor vehicle crash.

Book excerpt: This project evaluates the performance of health insurance market reforms in seven states. Its focus is on state reforms to the small group and individual markets, but it also includes some preliminary findings about the early impact of the federal Health Insurance Portability and Accountability Act (HIPAA). Little is presently known about the details of how well these reforms have performed in achieving their various objectives and avoiding predicted harms. Other studies test for aggregate effects on enrollment and premiums using primarily quantitative measures. This project is a primarily qualitative evaluation that attempts to understand why these effects have occurred and how these reforms affect the inner workings of the insurance market. The principal reforms studied are: (1) guaranteed issue; (2) renewability and portability; (3) rating bands and community rating; (4) restrictions on underwriting practices such as risk selection and pre-existing condition exclusions; (5) reinsurance and risk adjustment; and (6) public purchasing cooperatives.The primary method is an intensive case study of seven states that have enacted varying reforms: Colorado, Florida, Iowa, New York, North Carolina, Ohio and Vermont. This multiple case study consists primarily of two rounds of structured, in-depth, open-ended interviews with over 100 insurance industry and regulatory sources. The study also includes an analysis of quantitative, documentary, and other secondary data, and a market test in which a potential purchaser contacted 144 agents.

Book excerpt: This research uses the input-oriented Data Envelopment Analysis (DEA) approach to examine the efficiency of the U.S. health insurers. It shows that more insurers are less efficient than the previous sample year; however, the results suggest that the federal health care reform have no significant effect on the overall efficiency of all the insurers as a whole which is very low but does not change much over time. This research explores how to improve the efficiency of the health insurance market by proposing the state, regional and national efficiency-based goal-oriented market models and an efficiency duplicating system, and discusses important implications to the health care compacts, the health insurance exchanges or marketplaces, and the national multi-state programs. It also analyzes further moves for efficiency enhancement with regard to payments methods and the health care delivery system. One interesting finding is that the Medicaid program is very efficient as provides support to the offering of Medicaid coverage and further expansion which enhances the health welfare of the society with fewer resources inputs from the perspective of efficiency. This research should provide important insights for the state and federal governments, policy makers, regulators, the health insurance industry, and consumers.

Book excerpt: We present and empirically implement an equilibrium labor market search model where risk averse workers facing medical expenditure shocks are matched with firms making health insurance coverage decisions. Our model delivers a rich set of predictions that can account for a wide variety of phenomenon observed in the data including the correlations among firm sizes, wages, health insurance offering rates, turnover rates and workers' health compositions. We estimate our model by Generalized Method of Moments using a combination of micro data sources including Survey of Income and Program Participation (SIPP), Medical Expenditure Panel Survey (MEPS) and Robert Wood Johnson Foundation Employer Health Insurance Survey. We use our estimated model to evaluate the equilibrium impact of the 2010 Affordable Care Act (ACA) and find that it would reduce the uninsured rate among the workers in our estimation sample from 20.12% to 7.27%. We also examine a variety of alternative policies to understand the roles of different components of the ACA in contributing to these equilibrium changes. Interestingly, we find that the uninsured rate will be even lower (at 6.44%) if the employer mandate in the ACA is eliminated.

Book excerpt: For decades small business owners have been struggling to offer health insurance to their employees. Health insurance reform has been an issue of contention over the last few decades, culminating in the passage of the Patient Protection and Affordable Care Act on March 23, 2010. One measure policy makers took to increase access to health insurance was to provide incentives to small business owners to offer health insurance. However, prior research has indicated that economic incentives alone do not guarantee the provision of health insurance coverage by small business owners. Previous research has rarely examined the noneconomic (social/cultural) factors behind why small business owners choose to provide or not to provide health insurance coverage for their employees. Rural Montana offers a unique opportunity to study economic and social factors that influence health insurance decision as it has high rates of employment in small firms. I conducted qualitative in-depth semi-structured interviews with small business owners in four rural western Montana communities to explore their reasoning for providing or not providing health insurance. Results indicate that small business owners are influenced by many interrelated practical/economic factors as well as social factors.
