Book excerpt: Trois traitements permettent actuellement de guérir les cancers débutants des cordes vocales : la radiothérapie, la chirurgie laser, et la chirurgie laryngée partielle. Cependant, la voix est altérée à la fois par la maladie et par le traitement, ce qui peut avoir des répercussions sur le quotidien des patients. Objectif de l'étude : analyser l'altération vocale post-thérapeutique et son impact sur la qualité de vie en comparant les trois modalités de traitement. Patients et méthode : il s'agit d'une étude prospective regroupant 37 patients répartis selon le traitement reçu (radiothérapie, cordectomie laser, laryngectomie partielle). Des mesures instrumentales ont été recueillies, débit d'air buccal (DAB), pression sous-glottique estimée (PSGE), intensité (Int), les patients ont répondu au questionnaire du Voice Handicap Index et au questionnaire de qualité de vie QLQ-C30 et son module spécifique H&N35. Résultats et conclusion : il persiste une altération vocale post-thérapeutique quel que soit le traitement mais celle-ci ne constitue pas un handicap vocal. Les patients des trois groupes ont un bon niveau de qualité de vie sans différence significative. Les patients les plus gênés par leur voix sont ceux qui n'ont pas pu conserver leur activité professionnelle après traitement.

Recueil de mesures instrumentales de la voix et passation de questionnaires psychométriques: Voice Handicap Index; MOS SF 36, auto évaluation de l'intelligibilité et de la fatigabilité vocale, questionnaire sociologique. Conclusions : Quel que soit le traitement on constate une altération vocale. La qualité de la voix est meilleure chez les patients ayant subi une radiothérapie. L'altération vocale est considérée comme un handicap léger dans 6 cas sur 7. On ne constate pas de modification au niveau social pour les trois groupes de traitement. Seuls les patients ayant subi une radiothérapie notent une légère modification de leur vie quotidienne.

Book excerpt: Avec environ 3 000 nouveaux cas par an en France, le cancer du larynx est un des cancers des VADS les plus fréquemment diagnostiqués. Les tumeurs des plis vocaux de stade précoce sont généralement de bon pronostic. Elles font l'objet d'un traitement par chirurgie partielle ou par radiothérapie. Les séquelles de ces traitements sont essentiellement vocales et affectent la qualité de vie des patients. Une intervention orthophonique spécifique, adaptée à ces difficultés, a été prouvée efficace pour améliorer la qualité vocale des sujets. Notre étude a pour but de proposer une intervention intensive, adaptée aux séquelles retrouvées après chirurgie partielle ou radiothérapie, sous la forme d'un entraînement à domicile. Nous avons étudié la faisabilité de la mise en place de l'entraînement, en autonomie après modélisation, auprès de trois patients. Nous avons recueilli leur avis vis-à-vis du protocole afin de tester leur adhésion. Nous avons ensuite évalué les effets de l'intervention grâce une analyse objective et subjective de la qualité vocale et du handicap vocal ressenti avant et après l'entraînement. Au terme de notre étude, nous constatons que la mise en place du protocole, dans les conditions définies, est réalisable, et ce de façon reproductible. Nous obtenons une bonne adhésion des trois sujets participant à sa mise en place, au regard de leur régularité dans l'entraînement et de la satisfaction qu'ils ont exprimée à son issue. Une amélioratïon quantifiable du handicap vocal ressenti est constatée chez chacun des sujets, mais pas d'amélioration vocale significative. Le protocole créé présente un intérêt pour la pratique clinique car il pourrait être exploité par des orthophonistes rééduquant la voix des patients décrits. Il pourrait donc faire l'objet d'une validation auprès d'une cohorte de sujets plus importante et auprès d'orthophonistes.
