Book excerpt: En la sociedad actual de los países desarrollados existe un notable incremento en el uso de la radioterapia para el tratamiento de tumores, ya sea con un objetivo paliativo o curativo; la maquinaria empleada es cada día más precisa en la determinación de los volúmenes a irradiar y a la vez permite irradiar con niveles de intensidad más elevados; esto presenta ventajas e inconvenientes. La necesidad del conocimiento de la dosis impartida en piel debido al aumento de toxicidad en los tratamientos con técnicas de modulación de intensidad (IMRT) es el motor inicial del proyecto. Cuando se irradia en un tratamiento de radioterapia externa la piel siempre recibe radiación: tanto si la piel es el volumen a irradiar como si es el órgano en riesgo debe conocerse con la máxima exactitud el nivel de dosis, ya sea para correlacionar la posible toxicidad a la dosis administrada durante el tratamiento como para garantizar la correcta irradiación del paciente. Los avances producidos en la actualidad en el diseño de linacs permiten aumentar el nivel de dosis a impartir sin comprometer la afectación de los órganos en riesgo; en la técnica de modulación de intensidad (IMRT) el nivel de dosis en piel se puede incrementar notablemente respecto a técnicas convencionales, más aún si se hace uso de inmovilizadores. Los niveles de radiación se conocen con el uso de detectores. Para conocer el nivel de radiación en la capa basal o zona de build-up, situada a una profundidad de 7mg/cm2 de la superficie de la piel, se debería introducir el detector dentro del paciente, cosa inviable. Se han de plantear otros métodos que permitan conocer estos valores con el máximo de precisión para optimizar el tratamiento y sus efectos adversos. Actualmente los sistemas de planificación utilizados en el tratamiento permiten una fusión de imágenes adquiridas previamente al tratamiento, que mejoran la determinación de la zona a irradiar pero no resuelven con suficiente precisión la zona de build-up; un estudio realizado por el Task Group 53 de la Asociación Americana de Físicos Médicos con detectores habituales en el ámbito clínico han determinado diferencias del 20% y del 50% entre el valor planificado y el medido experimentalmente dependiendo del tipo de haz. El instrumento de referencia para medir el nivel de dosis donde no existe equilibrio electrónico es la cámara de extrapolación, en este proyecto se ha utilizado el modelo 23392 de la casa PTW para determinar los factores de corrección necesarios para conocer el nivel de dosis en piel durante los tratamientos de radioterapia con una precisión inferior al 2-3%. La cámara de extrapolación que se ha utilizado en este proyecto ha estado en desuso durante algunos años; por ello en la primera fase se ha realizado el proceso de ajuste midiendo con una fuente calibrada de partículas beta en el Institut de Tècniques Energètiques de la UPC; dado que la finalidad es poder utilizar la cámara en un hospital para calibrar el haz de los equipos utilizados en tratamientos de radioterapia, paralelamente al ajuste se ha diseñado y construido un soporte para la cámara que ha permitido situarla en una posición vertical (el montaje original sólo permitía situarla en posición horizontal). Garantizado el funcionamiento de la cámara y sabiendo configurarla correctamente, se han realizado las mediciones en el Hospital de la Santa Creu y Sant Pau en el servicio de Radiofísica y Radioprotección con la finalidad de poder caracterizar los niveles de radiación en piel, cosa que permitirá a los radiofísicos utilizar los valores obtenidos con la cámara de extrapolación como referencia para las cámaras que utilizan habitualmente en el hospital y que tienen mucha menos precisión.

The term also can be used to describe the quantity of an ultimate, active agent as delivered to the appropriate target material within a biological system. Thus, for mutagens, one can speak of DNA dosimetry, meaning the number of adducts produced in the DNA of target cells such as bone-mar row stem cells or spermatogonia.
