Opioid overdose is easily reversed with the opioid antidote naloxone and with basic life support. Such care is generally only available in medical settings, however. These guidelines recommend that people who are likely to witness an opioid overdose, including people who use opioids, and their family and friends should be given access to naloxone and training in its use so that they can respond to opioid overdose in an emergency if a medical response is not available. Naloxone can be injected or administered intra-nasally and has minimal effects in people who have not used opioids. While naloxone administered by bystanders is a potentially life-saving emergency interim response to opioid overdose, it should not be seen as a replacement for comprehensive medical care.

En collaboration avec les équipes de SMUR, les infirmiers sapeurs-pompiers (ISP) participent activement aux missions de secours à personne grâce notamment aux protocoles infirmiers de soins d'urgence (PISU), au nombre de 24 au sein du Service départemental d'incendie et de secours de la Loire (SDIS 42). L'objectif principal de cette étude était de décrire la création et la mise en place de ces PISU. Méthode. Une étude d'évaluation des pratiques professionnelles a été menée par analyse des données de retour d'intervention des ISP du SDIS 42 en 2018. Résultats. 2 320 fiches d'intervention comprenant 2 947 protocoles ont été analysées. Les pathologies les plus fréquentes étaient les traumatismes (39.4 %), puis les pathologies cardio-vasculaires et neurologiques. Le protocole antalgie était celui qui a le plus été utilisé (1 124 utilisations), suivaient les protocoles à orientation cardio-vasculaire et la gestion de l'hypoglycémie. Dans 98.7% des cas, le PISU a été choisi de façon adéquate par rapport à la situation clinique et dans 98.8% il a été correctement réalisé. Dans 68% des cas la fiche d'intervention a été correctement retranscrite. Comme pour le protocole antalgie avec l'introduction du méthoxyflurane en 2018, les PISU sont régulièrement révisés en fonction des données actualisées de la science afin de permettre une prise charge adaptée des victimes. Discussion. L'analyse exhaustive des fiches d'intervention a permis de montrer que les ISP agissent dans leur champ de compétences, et qu'ils appliquent correctement les protocoles établis. Ils limitent le nombre de déplacements des équipes de SMUR pour des situations ne nécessitant pas leur présence, et agissent en complément lorsque celles-ci sont requises.
