L’obtention de ce résultat passe notamment par la réalisation d’un profil d’émergence physiologique assurant la santé et le maintien des tissus parodontaux. En denture naturelle, ce défini par l’angle d’émergence formé par l’inclinaison de la face vestibulaire d’une dent au niveau sulculaire et son grand axe. En prothèse dento-portée le praticien se base sur les tissus dentaires non préparés et sur les tissus mous marginaux pour le recréer. En prothèse implantaire ces repères n’existent pas, de ce fait le praticien devra coordonner toutes les étapes du traitement dans le but de recréer un profil d’émergence optimal au niveau de la restauration. La connaissance des mécanismes d’intégration de l’implant aussi bien au niveau des tissus durs que des tissus mous préalablement à la mise en place d’implants est primordiale car elle permet au praticien d’anticiper sur la cicatrisation tissulaire. Le choix de le morphologie implantaire se fait en fonction de divers critères cliniques tels que le volume osseux résiduel, la qualité osseuse et le profil d’émergence de la dent à remplacer. Les états de surfaces rugueux seront préférés aux surfaces lisses pour leurs meilleurs résultats cliniques. Le positionnement de l’implant est aussi dépendant de la situation clinique mais obéit à des règles précises dans les trois plans de l’espace dont le non respect entrainerait un profil d’émergence non physiologique et donc une instabilité tissulaire à long terme. Après la phase chirurgicale, les étapes prophétiques de la gestion du profil d’émergence débutent par la temporisation. Pendant 6 à 8 semaines, les éléments transitoires : piliers individualisés ou couronnes provisoires vont sculpter la forme des tissus péri-implantaires tels que la gencive libre marginale et les papilles interproximales lors de la cicatrisation. Le berceau prophétique ainsi obtenu devra être enregistré fidèlement et transmis au laboratoire de prothèse grâce à des transferts d’empreinte personnalisés ou même à la couronne provisoire. Le prophétiste réalise ensuite un modèle en plâtre intégral ou muni d’une fausse gencive souple amovible lui permettant de construire la couronne en adéquation totale avec l’environnement tissulaire. Cette couronne vissée ou scellée repose sur un pilier prophétique dont le choix tient compte entre autre de la hauteur prophétique disponible, de la hauteur et de la qualité des tissus mous péri-implantaires. Ce pilier peut être en continuité avec le col implantaire ou sous dimensionné, on parle alors de platform-switching. Ce concept limite de façon significative la résorption osseuse cervicale et les récessions tissulaires garantissant ainsi l’obtention d’un profil d’émergence optimal.
