Book excerpt: Implant therapy for replacing missing teeth is a predictable treatment with a high survival rate in the long term. However, some failures are seen during the first year after implant placement. Previous studies have been related the history of periodontitis, smoking habit and a poor oral hygiene with a higher risk to develop periimplantitis. However, more research is necessary in this regard to identify and consequently control all the conditions that can affect the long-term success of implant therapy.The aim of this study was to establish a standardized method to evaluate the long-term results of MISu00ae implants (V3 and C1), in terms of survival, and to assess the risk factors associated with biological and technical complications. We aim to study the results of different surgical scenarios and patient-related factors and, therefore, identify their influence for the long-term stability of the treatment. A prospective cohort study with five hundred implants (C1 and V3 MISu00ae) was conducted in patients older than 18 years old scheduled for implant therapy. Each patient was carefully informed about the objectives of the study and the informed consent was signed. All the surgeries were performed by postgraduate students at the UIC (International University of Catalonia). Patient-related and surgical related factors of each patient were saved in our database to test their influence in the long-term survival of the implants After 1-year of follow-up, 4.2% of 501 implants had failed; which were considered early failures; without osseointegration of the implant. No prosthetic or biologic complications were found. History of periodontal disease was significantly related with higher implant failure (OR:3.00).

Abstract: Background: Type I immediate implant placement has gained popularity because it may reduce treatment time, number of surgeries and post-extraction bone loss. However, this is potentially challenged by inadequate keratinized mucosa for flap adaptation and difficulties in achieving primary stability. Moreover, it has been proven that post-extraction bone loss is an inevitable biological process, which affects treatment outcomes. Objectives: To estimate survival and success rates of implants and the implantsupported prostheses, the prevalence of biological, technical and aesthetic complications, and the magnitude of soft and hard tissue changes following implant placement immediately into fresh extraction sockets. Material & methods: An electronic search in MEDLINE (PubMed) and the Cochrane library from 1991 to July 2010 was performed to include prospective studies on immediate implants with a mean follow-up time of at least 1 year. The survival rates were computed using the STATA statistical software. Weighted means of soft and hard tissue changes were obtained by the inverse variance method. Results: A total of 46 prospective studies, with a mean follow-up time of 2.08 years, were included. The annual failure rate of immediate implants was 0.82% (95% CI: 0.48%-1.39%), translating into the 2-year survival rate of 98.4% (97.3%-99.0%). Among the 5 factors analyzed [reasons for extraction, antibiotic use, position of implant (anterior versus posterior, maxilla versus mandible), type of loading], only the regimen of antibiotic use affected the survival rate significantly. Lower failure rates were found in groups that were provided with a course of post-operative antibiotics. The success of implant therapy was difficult to assess due to scarce reporting on biological, technical and aesthetic complications. Soft tissue changes occurred mostly in the first 3 months after the provision of restoration, and then stabilized towards end of the first year. Marginal bone loss predominantly took place in the first year after implant placement, with a magnitude generally less than 1mm. Controversy on hard tissue preservation with platform-switching technique remained unsolved. Conclusions: Despite the high survival rate observed, more long-term studies are necessary to determine the success of implant treatment provided immediately after tooth extraction. However, little data exist on implant survival, prevalence of biological and technical complications and patient-reported outcomes for implants placed and restored in private practice settings.Aim The aim of the present retrospective study was to assess the survival, complications and patient satisfaction of dental implants placed and restored in a private practice setting after an observation period of at least 5 years . Material and Methods203 parallel wall implants (89% Biomet 3i, 8,4% Straumann, 2,5% Nobel Biocare) were placed by the same practitioner in 58 patients. 93,5%% of the restorations delivered were screw-retained and 6,5% cemented. All restorative procedures were performed by a second practitioner. Biological parameters evaluated for each implant included radiographic bone loss (RBL), plaque index (PI), bleeding on probing (BOP), probing depth(PB), keratinized tissue width(KTW), recession, suppuration, mobility and peri-implant radiolucency .The prosthetic parameters evaluated were porcelain fracture , prosthetic and abutment screw loosening/fracture, metal framework fracture, resin plug replacement, loss of contact point and decementation. Patient-related parameters were recorded through patient questionnaires rating satisfaction with appearance, function and general satisfaction. The median follow-up time was 7.8 years. Descriptive statistics and chi square analysis for exploring associations were performed.ResultsThe implant survival rate was 95,7%. Plaque was present in 15,9% and BOP in 69.7% of the sites. PD was u22653 and u2264 5 mm in 33% of the sites and u2265 5mm in 11.3% of the sites. In 24.6% of the sites KTW was u2264 1,5mm, whereas in 21% of them recession was recorded. RBL ranged from 1.5 to 3mm in 16%of the sites and was u22655mm in 6.5% of the sites. KTW u2264 1.5mm was more frequent in the maxilla (p
