Book excerpt: Optimizing outcomes for women in labor at the global level requires evidence-based guidance of health workers to improve care through appropriate patient selection and use of effective interventions. In this regard, the World Health Organization (WHO) published recommendations for induction of labor in 2011. The goal of the present guideline is to consolidate the guidance for effective interventions that are needed to reduce the global burden of prolonged labor and its consequences. The primary target audience includes health professionals responsible for developing national and local health protocols and policies, as well as obstetricians, midwives, nurses, general medical practitioners, managers of maternal and child health programs, and public health policy-makers in all settings.

Book excerpt: Prolonged Labour is a comprehensive, scholarly and balanced account of New Labour's domestic policies in its first two terms in office. It mobilizes ministerial speeches, elite interviews and policy documents, plus the best available, contemporary scholarship and political journalism, to build an accurate and full account of New Labour in power. It then subjects that record to systematic and sympathetic evaluation. Prolonged Labour is an invaluable guide to Government performance since 1997, and as such an essential tool for students and voters alike.

Counselling for Maternal and Newborn Health Careis divided into three main sections. Part 1 is an introduction which describes the aims and objectives and the general layout of the Handbook. Part 2 describes the counselling process and outlines the six key steps to effective counselling. It explores the counselling context and factors that influence this context including the socio-economic, gender, and cultural environment. A series of guiding principles is introduced and specific counselling skills are outlined. Part 3 focuses on different maternal and newborn health topics, including general care in the home during pregnancy; birth and emergency planning; danger signs in pregnancy; post-abortion care; support during labor; postnatal care of the mother and newborn; family planning counselling; breastfeeding; women with HIV/AIDS; death and bereavement; women and violence; linking with the community. Each Session contains specific aims and objectives, clearly outlining the skills that will be developed and corresponding learning outcomes. Practical activities have been designed to encourage reflection, provoke discussions, build skills and ensure the local relevance of information. There is a review at the end of each session to ensure the SAs have understood the key points before they progress to subsequent sessions.

Book excerpt: The World Health Organization has updated its 1988 publication on the partograph by addressing the findings of a multicenter trial conducted during 1990-91. The second part of the updated publication, "Preventing Prolonged Labour: A Practical Guide--The Partograph, Part II: User's Manual," provides guidelines on the use of the partograph in the management of labor. It focuses on the practical aspects of using the partograph as a managerial tool in labor and does not address theoretical aspects. The partograph helps identify early women in slow labor and prevent and manage any resultant complications. It should be used only for women who have no pregnancy complications that require immediate action and who have actually started labor. The partograph allows health workers to plot the progress of labor (i.e., cervical dilatation, descent of the fetal head, and uterine contractions), the fetal condition (i.e., fetal heart rate, membranes and amniotic fluid, and molding of the fetal skull), and the maternal condition (i.e., pulse, blood pressure, temperature, urine [volume, protein, acetone], drugs and IV fluids, and oxytocin regime). The manual addresses what needs to be done in the case of abnormal progress of labor which incorporates prolonged latent phase (>8 hours) and prolonged active phase. It then moves on to discussing management of labor by focusing on normal latent and active phases, between alert and action lines, at or beyond active phase action line, prolonged latent phase, and further discussion on oxytocin, membranes, and fetal distress. The manual provides two exercises and answers to test knowledge on correct partograph use. Annex I is an aid cutout to be reproduced in plywood to help health workers accurately measure cervical dilatation.

Available in PDF, EPUB and Kindle. Book excerpt: Concise, with a structured approach, Algorithms for Obstetrics and Gynaecology supports development of key problem-solving skills obstetricians and gynaecologists are required to maintain under pressure. All algorithms are designed to support rapid decision making in the most clinically relevant situations to minimize the risks of a poor outcome. This book covers everything from common non-life-threatening emergencies such as hyperemesis in pregnancy, to life-threatening acute events such as ectopic pregnancy, acute fetal distress, or maternal collapse. Each topic is devoted to a different clinical problem or emergency. Key learning points aid trainees by distilling the essential information from the chapter. Each topic has an algorithm, a care pathway, or table of key information, suiting the learning needs of trainees. However, this is also an important resource of information for day to day practice as algorithms and care pathways have been created using current national guidelines and clinical evidence. Every algorithm and care pathway has been carefully structured to ensure a logical progression of thought to aid anticipation, early diagnosis, and prompt and appropriate management. Ethical, legal and social issues are also discussed where relevant. Book jacket.
