Available in PDF, EPUB and Kindle. Book excerpt: This is the fourth volume of our series Progress in Anti-cancer Therapy. For the past four years we have taken the challenge to select each year, some of the most interesting topics on the wide field of oncology. As usual, this volume continues the tradition and covers five cancers (breast, prostate, bladder, lym phoma and ovarian), the role of HIV and HTLV in cancer, as well as some deve lopmental pharmacology and behavioral issues. This year, we have particularly focused our attention on one of the most com mon (although rarely addressed) cancer accounting for more than 315,00 newly diagnosed cases and 41,000 cancer-related deaths annually (Landis SH et al., 1999) : prostate cancer. Five chapters address some of the issues concerning this disease. The chap ter by Schroder describes the hopes and pitfalls of early detection of prostate cancer. It reviews the literature on screening studies that have been performed both in the US and in Europe. These studies suggest that cancer mortality could be reduced by screening for prostate cancer, mostly through PSA testing. However, the effectiveness of early detection still needs to be definitely confir med and ongoing randomized studies are described, the results of which will probably help the medical community determine the worth of PSA-based scree ning for prostate cancer. On the same hand, the paper from von Eschenbach is reviewing the clinical problems that are related this disease.

Book excerpt: These proceedings emanate from the Second Prouts Neck Conference on prostate cancer held on October 17-19, 1986, the theme of which was treat ment, with focus on current issues and future research that is needed to answer critical questions related to optimal management of the various stages of prostate cancer. The objective was to reveal the most crucial problems impeding progress and to crystallize the combined multidisci plinary input generated by the conference into focused concepts or recommendations for presentation to the National Cancer Institute (NCI) , with the ultimate intent of targeting research to address the priority issues identified. In organizing the workshop, every effort was made to maintain a multidisciplinary balance among nationally renowned authorities on prostate cancer. Thus, leading surgeons, radiation and medical onco logists and biostatisticians were in equal presence. While there were spirited exchanges with careful scrutiny and critique of all data pre sented, there was a common belief that the challenge of prostate cancer would be best approached in this multidisciplinary Organ Systems-oriented fashion. During the course of the conference, it became apparent to all present that major nomenclature and procedural barriers have made it generally difficult, and frequently impossible, to compare results of clinical research.

Available in PDF, EPUB and Kindle. Book excerpt: Carcinoma of the prostate increasingly dominates the attention of urologists for both scientific and clinical reasons. The search for an explanation and the prediction of the variable behaviour of the malignant prostatic cell continues unabated. The search for more precise tumour staging and more effective treatment is equally vigorous. Editors Andrew Bruce and John Trachtenberg have assembled acknowledged leaders in prostate cancer to present those areas of direct interest to the clinician. There are a number of other topics that might have been considered but most of these, such as experimental tumour models or biochemical factors affecting cell growth, still lack immediate application for the clinician. Carcinoma of the prostate continues to have its highest incidence in the western world, and the difference in comparison with the incidence in the Far East appears to be real and not masked by diagnostic or other factors. A number of other epidemiological aspects need careful analysis: Is the incidence increasing? Is the survival improving? Is the prognosis worse in the younger patient? Epidemiological data are easily misused and misinterpreted so that a precise analysis of the known facts makes an important opening chapter to this book.

Available in PDF, EPUB and Kindle. Book excerpt: More than half of all men will develop prostate problems during their lifetimes, while one in eight will be diagnosed with prostate cancer. Even when prostate cancer is successfully treated, the side effects from the treatments (such as urinary incontinence, erectile dysfunction, fatigue, and sepsis) can be debilitating. Dr. Gavi, Clinical Assistant Professor of Medicine at Stanford University, teamed up with researcher Maya Eylon to compile more than 100 recent, reliable, and relevant international studies on the effects of diet on prostate health. The good news is that scientific research has confirmed that 50 percent of prostate cancer cases can be prevented by making simple dietary changes, along with screening for precursors, maintaining a healthy weight, and exercising adequately. Dr. Gavi summarizes these findings in easy-to-understand terminology with each study fully referenced. Readers discover how key compounds in specific foods work to prevent cancer cells from forming. Cruciferous vegetables (glucosinolates), tomatoes (lycopene), soy foods (isoflavones), and green tea (catechins) are the superfoods that make a difference. Recommendations are provided for the optimal way to prepare these foods and for how much of each food is needed to reap the greatest health benefits. The two types of prostate check-ins (screening and diagnostic tests) are also discussed, and a prostate cancer self-screening checklist is provided. The findings and information presented are powerful tools that can empower men to make vital lifestyle changes that will have a significant impact on their health.

Book excerpt: Although much progress has been made in the four years since the first edition of Management of Prostate Cancer, prostate cancer remains a significant biological, me- cal, and personal challenge for millions of men. In this interval, some important trends and observations have emerged that represent real progress in the field and which will shape the direction of clinical practice and research in the next 5–10 years. These obs- vations include: (1) a decline in prostate-related cancer mortality in the United States, likely owing to a combination of factors including screening, more aggressive and earlier therapy, and improvements in specific therapies; (2) a significant downward pathological stage migration, so that an individual’s chance of cure for a given stage, grade, and PSA is better now than it was early in the PSA era, even without associated improvements in individual therapies; (3) the recognition of new PSA isoforms that may refine screening strategies; (4) several randomized, phase III clinical trials demonstrating survival adv- tages of one form of therapy over another in selected populations (external beam rad- therapy with adjuvant hormones vs radiotherapy alone, radical prostatectomy vs observation); (5) a focus on the biology of bone and bone metastasis, and new agents that reduce skeletal-related events and which may inhibit the growth of new metastases; (6) second-generation anti-PSMA antibodies with improved potential for imaging and therapy; (7) the development and widespread adoption of nomograms that assist in cli- cal decision-making for individual patients; (8) the identification of new genes that

Available in PDF, EPUB and Kindle. Book excerpt: A 2022 Choice Reviews Outstanding Academic Title An important history of the development of cancer centers of excellence and the revolution in cancer treatment. In the 1960s a coalition of concerned citizens, scientists and politicians joined forces to convince the federal government to focus its efforts on conquering cancer. The National Cancer Act of 1971 resulted and was signed into law on December 23, 1971 by President Nixon. The national “War on Cancer,” was declared with some leaders naively arguing that the disease would be conquered by the nation’s bicentennial—a mere five years in the future. Over the next five decades scientific discoveries demonstrated the great complexity of what had formerly been thought of as a single disease – with the advent of the genetic characterization of cancers, it is now recognized that there are almost an infinite number of cancers as defined by their many genetic mutations. The National Cancer Act established the infrastructure for the designation of centers by the National Cancer Institute (NCI) and these centers have evolved into models of multidisciplinary, collaborative cancer research, treatment and prevention contributing to a reduction in cancer mortality and increase in quality of life and survival that has translated into more than 17 million cancer survivors in the United States in 2021. Centers of the Cancer Universe: A Half-Century of Progress Against Cancer tells the story of how cancer research was not front and center at most universities and research institutions before the National Cancer Act of 1971, and why many physicians were reluctant even to treat patients with cancer in the early 20th century. It follows the behind-the-scenes lobbying, resistance and negotiating that preceded signing the Act into law, and how the cancer centers of today came to fruition, and shaped how cancer research, clinical trials and treatment would be conducted.

Book excerpt: We know more about cancer prevention, detection, and treatment than ever beforeâ€"yet not all segments of the U.S. population have benefited to the fullest extent possible from these advances. Some ethnic minorities experience more cancer than the majority population, and poor peopleâ€"no matter what their ethnicityâ€"often lack access to adequate cancer care. This book provides an authoritative view of cancer as it is experienced by ethnic minorities and the medically underserved. It offers conclusions and recommendations in these areas: Defining and understanding special populations, and improving the collection of cancer-related data. Setting appropriate priorities for and increasing the effectiveness of specific National Institutes of Health (NIH) research programs, to ensure that special populations are represented in clinical trials. Disseminating research results to health professionals serving these populations, with sensitivity to the issues of cancer survivorship. The book provides background data on the nation's struggle against cancer, activities and expenditures of the NIH, and other relevant topics.

Book excerpt: There is now increasing awareness by the general public in European countries that prostate cancer is a serious threat to health, and this has created higher expectations for improved and more effective methods for detecting and treating the disease. However, urologists are very conscious of the limitations of the diagnostic methods that are available and are even more concerned about the apparent lack of therapeutic advances made during the past 50 years since Huggins discovered the fundamental principles of endocrine treatment for is theo prostate cancer. Recent efforts to detect the disease when it retically "curable" have been successful, certainly in the USA, but this has highlighted our uncertainty about the best treatment for early stage prostate cancer, and there is no doubt that radical pro statectomy is sometimes carried out on men who may not be threa tened by their illness. While it is generally accepted that many men with prostate cancer will die of old age rather than this malignancy, it cannot be ignored that this disease kills many others in a process that is frequently lingering, miserable, and humiliating, not only for the victim but also his family. There are many important issues about prostate cancer that remain unclear at the present time, some of which are addressed by the reviews in this book. The debate about early detection and screening can arouse considerable heat in otherwise placid urological mee tings.

The cancer which develops in the prostate is known as prostate cancer. In the initial stage, it may not cause any symptom, but it can cause several complications in the later stage. Some of the common symptoms include problems associated with urination, like frequent urination, nocturia, hematuria and dysuria. It may also cause problems associated with sexual organs, such as painful ejaculation and difficulty in achieving erection. Ultrasound, digital rectal examination (DRE), cystoscopy, transrectal ultrasonography and magnetic resonance imaging (MRI) are some of the common diagnostic methods useful in the diagnosis of prostate cancer. Treatment methods include radiation therapy and radical prostatectomy. Prostate cancer research is an upcoming field of medical research that has undergone rapid development over the past few decades. It presents researches and studies performed by experts across the globe. The book is appropriate for students seeking detailed information in this area as well as for experts.
