Book excerpt: Excerpt from The Prognosis and Treatment of Diseases of the Heart In the course of the ages the heart, like other organs of the body, has been viewed in a great variety of ways. In ancient times it seems to have been regarded as a sort of heel of Achilles, in that it was held to be immune to all ordinary diseases. It was not unnatural that the inability to detect cardiac abnormalities should lead to the belief that the heart itself could not become diseased. Even the great discovery of Harvey was not followed by any immediate increase in the knowledge of cardiac pathology. Book excerpt: For many years, there has been a great deal of work done on chronic congestive heart failure while acute heart failure has been considered a difficult to handle and hopeless syndrome. However, in recent years acute heart failure has become a growing area of study and this is the first book to cover extensively the diagnosis and management of this complex condition. The book reflects the considerable amounts of new data reported and many new concepts which have been proposed in the last 3-4 years looking at the epidemiology, diagnostic and treatment of acute heart failure.

Highlights include a simplified method for recognition of, and a practical therapeutic approach to, arrhythmias, as well as a more logical approach to drug management of hypertension than that given by the Joint National Committee, instructive algorithms that simplify the diagnosis and treatment of syncope, and extensive diagnostic information on hypertrophic cardiomyopathy. A large number of illustrative electrocardiograms that help to clarify the most often misinterpreted of all cardiologic tests and extensive discussions of practical cardiovascular pharmacology complete this magisterial survey.

Available in PDF, EPUB and Kindle. Book excerpt: Arterial hypertension, coronary heart disease and heart fail ure are the commonest cardiovascular conditions to present in clinical practice. Over the past few years it has become in creasingly clear that they are closely and causally interrelated and that their relationship can have a significant bearing on prognosis. Epidemiological studies have shown that arterial hypertension is one of the most important risk factors for de veloping heart failure. Only one in four patients with hyper tension is adequately managed, and in 50% of cases, the hypertension has not been recognised or treated. Patients with pre-existing hypertension who go on to suffer an acute myocardial infarction have usually not previously had typi cal angina symptoms, the infarct territory is larger, life threatening arrhythmias are commoner and hence in-hospi tal mortality and long-term prognosis are markedly worse. The presence of raised blood pressure in the post-infarct phase doubles the risk of manifest heart failure. The close relationship between hypertension, coronary heart disease and heart failure makes the choice of therapeu tic strategy particularly important. Agents and classes of agents that have prognostic value in all three conditions should be considered first, as synergy might result in addi tional benefits. In such patients, this sort of therapeutic deci sion-making might have further advantages. The use of these agents may prevent complications which are not yet clinically obvious (such as heart failure).

Book excerpt: Excerpt from Diseases of the Heart, Their Diagnosis, Prognosis, and Treatment by Modern Methods: With a Chapter on the Electrocardiograph The object of this book is to present, in as concise a form as possible, information which will be of service in the diagnosis, prognosis, and treatment of diseases of the heart by modern methods. I have dealt with the subject from the point of view of the clinician, and have carefully avoided matter of merely theoretical interest. It is generally recognised that within recent years there has been a great advance in our knowledge of cardiac disorders, and this advance has been of so practical a character and of such vital importance in the diagnosis, prognosis, and treatment of these diseases, that it is the duty of every clinician to make himself acquainted with its nature and scope. Many of the problems in the study of diseases of the heart which have baffled clinicians for generations have been practically solved, while others are in process of solution. This progress has been mainly due to the introduction of what are called graphic methods in the examination of the cardiac mechanism. By means of the clinical polygraph and the electro-cardiograph we are now able to analyse the cardiac action in a manner which was never possible before. The expert knowledge and use of these instruments, however, arc for practical purposes frequently not necessary; for, while it is true that most of our recent knowledge in the domain of cardiac disorders has been gained by means of them, all that is necessary in a large proportion of cases is the use of a Dudgeon's sphygmograph.
