Il existe un retard à l'instauration de l'insulinothérapie, faisant pourtant partie de la stratégie thérapeutique. Comprendre le vécu des patients sous insulinothérapie a pour but d'améliorer nos pratiques lors de l'instauration et le suivi de l'insuline. Méthode: Une étude qualitative par analyse thématique de contenu a été réalisée grâce à des entretiens semi-dirigés. L'échantillon de patients diabétiques de type 2, suivi en médecine générale, est raisonné diversifié. Les entretiens ont été menés jusqu'à saturation des données, avec triangulation des chercheurs. Résultats: Dix-neuf entretiens ont été réalisés. Les réactions à l'annonce de l'instauration d'une insulinothérapie n'étaient pas toutes négatives. Pour certains, elle était accueillie comme une alternative séduisante aux antidiabétiques oraux. La facilité d'utilisation du système d'injection, l'absence de douleur et la gestion facile du traitement étaient des atouts majeurs. L'insulinothérapie était jugée comme efficace et plus physiologique que les autres médicaments, avec également moins d'effets secondaires. Néanmoins, elle était parfois vécue comme un handicap, une rupture avec une vie «antérieure». C'était ainsi la prise de conscience de leur statut de patient diabétique. La relation des patients diabétiques avec leurs proches était souvent modifiée depuis le diagnostic du diabète, mais pas forcément depuis l'instauration de l'insuline. Ils étaient en effet impliqués dans le suivi des règles hygiéno-diététiques. Par contre, un manque d'informations des patients était retrouvé, notamment lors de l'instauration de l'insuline en ambulatoire. L'espoir en un avenir meilleur, était également remarquable. Conclusion : L'amélioration de l'encadrement des patients, par le développement de l'éducation thérapeutique personnalisée, devrait permettre à moyen terme, de pallier au manque d'information des patients. Il est fondamental d'inclure les aidants dans cette démarche éducative. En ayant une meilleure connaissance de leur maladie et de leur traitement, le vécu de l'insulinothérapie par nos patients ne pourra que s'améliorer.
