This book was released on 2012-12-06 with total page 664 pages. Available in PDF, EPUB and Kindle. Book excerpt: The first invasive evaluation of cardiac arrhythmias in humans was performed in 1967 in Paris (Prof. P. Coumel) and Amsterdam (Prof. D. Durrer). This was the start of a rapid increase in our knowledge of the diagnosis, mechanism and treatment of cardiac arrhythmias. In that same year Prof. Hein J.J. Wellens became cardiologist in the Wilhelmina Gasthuis in Amsterdam. Initially in Amsterdam (1967-1977) and later on in Maastricht (from 1977), he was the driving force for many breakthroughs in clinical cardiac electrophysiology. With an active interplay between the knowledge derived from the 12-lead electrocardiogram and the recordings made with invasive electrophysiology, he composed new ideas leading to major contributions in clinical cardiac electrophysiology and, more generally, in arrhythmology. He published over 650 scientific papers and 14 books, and had numerous functions within scientific boards of prestigious journals. In addition he trained more than 120 cardiologists in clinical cardiac electrophysiology. On the occasion of the congress `2000, Future of Arrhythmology: Lessons From the Past, Promises For Tomorrow', we highlight the scientific work of Prof. Hein J.J. Wellens. A selection of more than 60 articles over the whole time span has been selected. These articles are accompanied by comments from an expert, co-worker and/or former fellow in order to place the paper in a scientific time frame, including the relationship of the author with Prof. Hein J.J. Wellens.

Available in PDF, EPUB and Kindle. Book excerpt: As arrhythmias may be transient in nature and not seen during the shorter recording times of the standard ECG, ECG Holter monitoring allows the physician to make better informed decisions for the cardiac patient. The devices are worn by patients on an outpatient basis for days or weeks and can also be implanted subcutaneously. ECG Holter recordings are especially useful since they can be programmed individually for activation and specific tracing analysis. Designed for rapid study, this book contains 100 illustrative cases in ECG Holter monitoring. Each case consists of a tracing followed by a brief explanation of the findings. 100 Cases in ECG Holter is the perfect resource for busy physicians looking to optimize their skills at interpreting ECG Holter readings.

Book excerpt: In the last 15 years we have had the opportunity to teach Electrocardiography to many different types of student: doctors preparing to become cardiologists, cardiologists attending weekly 'refresher' sessions at our hospital, general practitioners who wish to become adept at electrocardiography and attend our yearly courses and, finally, the medical students of the Universidad Aut6noma of Barcelona. We cover everything with these students from the basics of electrophysiology to applied electrocardiographic semiology. This quadruple experience has proved stimulating, constantly motivating the search for better and more precise material, and the most appropriate didactic presentation for each type of student, each of whom has different requirements. I have always felt that didactic capability is not related to the intelligence of the professor, or to the amount of knowledge this person possesses, but really depends on the 'quality' of this knowledge, the 'desire' to transmit it and the 'capacity' to adapt to each teaching situation.

Book excerpt: Mastery of ECG interpretation is achieved not only by pattern recognition, but equally importantly, by a clear, practical understanding of how electricity moves through the heart and how disruption of that movement manifests itself via ECG tracings. ECGs for Beginners, written by one of the world's most respected electrophysiologists with over 40 years experience of training clinicians, will provide cardiology and electrophysiology trainees with an easy to follow, step-by-step guide to the topic, thus enabling them to both understand and interpret ECG readings in order to to best manage their patients. Packed with over 250 high-quality ECG tracings, as well as management algorithms and key points throughout, every chapter also contains self-assessment questions, allowing the reader to test themselves on what they've just learnt. All kinds of arrhythmias will be covered, as well as morphological abnormalities such as atrial and ventricular problems. Importantly, normal ECG readings will be presented alongside abnormal readings, to best demonstrate how and why abnormalities occur. ECGs for Beginners is an essential purchase for all cardiology and electrophysiology trainees, as well as being a handy refresher guide for the experienced physician.

Available in PDF, EPUB and Kindle. Book excerpt: For centuries the analysis of the heart rhythm has belonged to the foundations of medical art. We know that doctors in ancient Tibet used the interpretation of the heart rate to draw prognostic conclusions—somehow a modern rationale—that deserves further attention. The rapid advancement of science is providing more and more information about the details, but the subatomic resolution of structures hides the risk and the complex procedures are fragmented into static impressions. The same has happened to the ECG. The revolutionary development, acknowledged by the Nobel Prize for Einthoven, ledfromtheanalysisofthedynamicheartratetothestaticanalysisofthe heartstream curve. It is only with the ECG Holter recording over longer periods that the cardiologists rediscovered the old dynamic. With the continuous recording of the heart rate and its periodicity, it became accessible to a new dimension, a dim- sion that requires technically well-de?ned foundations for accurate data coll- tion, detailed knowledge of the electrocardiologic particularities of arrhythmia, and medical knowledge for the translation of the results into a diagnostic synthesis. With the ECG Holter the issue is no longer just to detect an arrhythmia, but also to determine dynamic circumstance in which the critical event occurred. In fact, we investigate the trigger, the event, and the context, and we have to integrate all of that information within the clinical picture, from the pathology right through to the symptom—indeed a multi-dimensional task.

Available in PDF, EPUB and Kindle. Book excerpt: Electrical activity in the myocardium coordinates the contraction of the heart, and its knowledge could lead to a better understanding, diagnosis, and treatment of cardiac diseases. This electrical activity generates an electromagnetic field that propagates outside the heart and reaches the human torso surface, where it can be easily measured. Classical electrocardiography aims to interpret the 12-lead electrocardiogram (ECG) to determine cardiac activity and support the diagnosis of cardiac pathologies such as arrhythmias, altered activations, and ischemia. More recently, a higher number of leads is used to reconstruct a more detailed quantitative description of the electrical activity in the heart by solving the so-called inverse problem of electrocardiography. This technique is known as ECG imaging. Today, clinical applications of ECG imaging are showing promising results in guiding a variety of electrophysiological interventions such as catheter ablation of atrial fibrillation and ventricular tachycardia. However, in order to promote the adoption of ECG imaging in the routine clinical practice, further research is required regarding more accurate mathematical methods, further scientific validation under different preclinical scenarios and a more extensive clinical validation

Available in PDF, EPUB and Kindle. Book excerpt: In recent years there has been considerable interest in the diagnosis and understanding of ventricular repolarisation, particularly the QT interval prolongation and abnormal T and T/U wave morphology associated with torsades de pointes. Advances in ion channel cloning have greatly improved our understanding of the role of ionic channels in mediating cardiac repolarisation. Unfortunately, it is increasingly recognised that a number of drugs, both those associated with altering repolarisation, and others for non-cardiac conditions can increase the propensity for polymorphic ventricular tachycardia, syncope and even ventricular fibrillation and sudden death. In this volume, arrhythmia specialists from St. George’s Hospital Medical School, London discuss the mechanisms behind QT prolongation and torsades de pointes. They focus particularly on the risk of individual cardiac and non-cardiac drugs in provoking long QT syndrome, providing a comprehensive review which will be useful for all electrophysiologists treating polymorphic ventricular tachycardias, and will expose important regulatory issues for pharmaceutical authorities and for the wider medical community.
