Available in PDF, EPUB and Kindle. Book excerpt: 'Medical technology is beneficial for well researched dangerous diseases. However, most symptoms that people bring to their primary care physician have no single clearly identifiable cause: investigations and drugs do more harm than good - and also waste resources - ' - Wilfrid Treasure Diagnosis and Risk Management in Primary Care teaches that adopting an evidence-based approach to primary care improves patient care and treatment outcomes. It demonstrates that brief clinical assessments, repeated if necessary, allow effective diagnosis while avoiding the costs and complications associated with more advanced testing. Adopting a fresh approach, this book sets consultation skills alongside evidence-based information by both itemising the specific techniques and facts that are needed in the consulting room, and providing detailed information on odds and likelihood ratios to quantify risk and deal with uncertainty. This book provides food for thought, and helps doctors develop communication skills that support their personal styles of consulting, encouraging a more traditional, intuitive treatment. It provides a map of the consultation and a compass to navigate through symptoms, signs and evidence - listening to their patients with one ear and, with the other, to the reflective inner voice of reason. General Practitioner Specialist Trainees and their teachers will find much of interest, as will established General Practitioners with an interest in maintaining traditional models of care. Undergraduate medical students and candidates for the MRCGP will find this an ideal reader for the clinical skills assessment. 'What a breath of fresh air to find an author capable of putting the patient back at the centre of the consultation and who is able to entertain at the same time as he informs and to stimulate critical reflection while nudging us in the direction of a rigorous approach to diagnosis, and the assessment and communication of risk.' From the foreword by Roger Jones

Available in PDF, EPUB and Kindle. Book excerpt: In this monograph, the authors summarize their findings in complex neuroimaging work (cranio-, spondylo-, myelo- and angiography as well as CT and MR imaging of the brain and spine) during their longstanding experience at the N. Burdenko Neurosurgical Institute in Moscow. The book begins with a review of modern neuroimaging techniques: CT and MR angiography, perfusion and diffusion imaging, tractography, spectroscopy and functional MR imaging. The problems and various other aspects of diagnosis of intra- and extra-axial brain tumors (more than 30,000 verified cases) as well as of cerebrovascular, infectious, demyelinating, degenerative and traumatic brain and spine lesions are discussed. The volume is well illustrated with angiographic, CT and MR images of complex diagnostic studies. The numerous images represent a "visual text," which can be used as an atlas by practical clinicians. This book is a comprehensive reference manual for neurologists, neurotraumatologists and radiologists. It may also be of interest to technicians, medical physicists, students and other specialists interested in neurovisualization and diagnostic imaging.

Available in PDF, EPUB and Kindle. Book excerpt: Cancer of the rectum continues to be a significant health problem in industrialized co- tries around the world. Relative 5-year survival rates in the USA for cancer of the rectum from 1995 to 2001 improved to 65%, a 15% improvement over 20 years (American Cancer Society, 2007). The reasons for this dramatic improvement include more accurate pr- perative staging, aggressive neoadjuvant therapy and improved surgical technique as well as specialty-trained surgeons. Despite advances in nonoperative techniques of radiation therapy, chemotherapy and immunotherapy, surgical extirpation continues to be the cornerstone of curative treatment of this potentially lethal disease. Radical cancer excision with total mesorectal excision has become the preferred surgical procedure for even early-stage cancers of the rectum. Over the past decade the enthusiasm for local excision (and other local treatments) has given way to persuasive (predominantly retrospective) evidence that the incidence of locoregional recurrence due to unsuspected lymphatic metastases and positive lateral margins is un- ceptably high even for stage T tumors. Vigorous attempts to find characteristics of the 1 tumor that would allow successful local treatments are ongoing.

Book excerpt: Colorectal Cancer Screening provides a complete overview of colorectal cancer screening, from epidemiology and molecular abnormalities, to the latest screening techniques such as stool DNA and FIT, Computerized Tomography (CT) Colonography, High Definition Colonoscopes and Narrow Band Imaging. As the text is devoted entirely to CRC screening, it features many facts, principles, guidelines and figures related to screening in an easy access format. This volume provides a complete guide to colorectal cancer screening which will be informative to the subspecialist as well as the primary care practitioner. It represents the only text that provides this up to date information about a subject that is continually changing. For the primary practitioner, information on the guidelines for screening as well as increasing patient participation is presentedd. For the subspecialist, information regarding the latest imaging techniques as well as flat adenomas and chromoendoscopy are covered. The section on the molecular changes in CRC will appeal to both groups. The text includes up to date information about colorectal screening that encompasses the entire spectrum of the topic and features photographs of polyps as well as diagrams of the morphology of polyps as well as photographs of CT colonography images. Algorithms are presented for all the suggested guidelines. Chapters are devoted to patient participation in screening and risk factors as well as new imaging technology. This useful volume explains the rationale behind screening for CRC. In addition, it covers the different screening options as well as the performance characteristics, when available in the literature, for each test. This volume will be used by the sub specialists who perform screening tests as well as primary care practitioners who refer patients to be screened for colorectal cancer.
