Book excerpt: Introduction : les traitements systémiques sont de plus en plus prescrits dans les dermatoses inflammatoires sévères de l'enfant. Notre objectif était de réaliser un état des lieux de nos cas pédiatriques de dermatoses inflammatoires sous traitement systémique. Matériel et méthodes : il s'agit d'une étude observationnelle, rétrospective, sur l'ensemble des patients âgés de 0 à 17 ans suivis dans le service de dermatologie du CHU d'Amiens entre 2011 et 2017 pour une dermatose inflammatoire nécessitant un traitement systémique. Résultats : nous avons inclus 59 enfants. Les pathologies les plus fréquentes étaient le psoriasis (59,3%), la DA (16,9%) et l'UCS (8,5%). Les pathologies étaient souvent sévères et la qualité de vie altérée. La durée d'évolution avant traitement était plus longue pour la DA (91,6 mois). L'acitrétine (34,8%) et le MTX (29%) étaient les traitements les plus prescrits pour le psoriasis ; le MTX (47,6%) et la ciclosporine (38,1%) pour la DA ; l'omalizumab (80%) pour l'UCS. Parmi les biothérapies prescrites dans le psoriasis, l'étanercept était la plus fréquente. La tolérance était souvent bonne sauf pour la ciclosporine. En raison de l'hétérogénéité de la population étudiée, l'analyse statistique de nos données était limitée. Discussion : nos résultats sont souvent conformes aux pratiques décrites dans la littérature. Des études bien menées dans la population pédiatrique sont nécessaires afin d'établir des recommandations adaptées. Conclusion : notre étude porte sur l'état des lieux de la prise en charge des dermatoses inflammatoires sévères de l'enfant. Ce partage d'expériences peut être une aide dans la gestion des traitements systémiques.
