Leur prise en charge est essentielle pour éviter des conséquences organiques et neuro-psychiques, à court et long terme. Notre étude prolonge le projet PHARE (Projet d’Harmonisation de l’Antalgie en Réanimation). Ce projet a permis de réaliser un état des lieux des pratiques de prise en charge des actes douloureux en 2004, dans sept services de réanimation polyvalente de Basse-Normandie, suivi d’une journée de formation pour tous les soignants des services. Notre nouvel état des lieux en 2009, montre une meilleure sensibilisation aux gestes douloureux et une amélioration des connaissances des échelles d’évaluation, particulièrement la BPS. On ne constate cependant pas d’amélioration par rapport à 2004 sur le terrain, avec une insuffisance majeure de l’évaluation de la douleur dans 16% des six actes douloureux étudiés, et seulement 25% de traitements antalgiques lors de ces actes. Les délais d’action des antalgiques sont incomplètement connus. A l’issue de ce travail, nos propositions pour une meilleure prise en charge des douleurs provoquées en réanimation sont les suivantes : des protocoles de monitoring de l’antalgie impliquant les équipes sous la responsabilité des référents douleur aux rôles clarifiés et renforcés, la prescription des délais d’action, le reporting des évaluations comme paramètre de monitorage, le développement des formations initiales et continues sur la douleur, le décloisonnement des services de réanimation dans leur établissement et aussi au niveau régional avec l’aide du Réseau Régional Douleur Basse Normandie et du futur Réseau Bas-Normand de Réanimation.
