Book excerpt: The EUROHIS project has focused on the comparability of health surveys and has uncovered some problems that arise when aiming to compare data from different countries. Similar problems of cross-cultural comparability also arise when comparing data from other sources, such as health registers, and are just as complex and difficult to tackle. One of the main advantages of health surveys, however, is the relatively low cost and time involved in adapting them according to the needs of health policy-makers. This work shows that the output from the EUROHIS project provides common instruments for the measurement of eight health indicators. The development of these instruments has required careful consideration of relevant common concepts and how these should be defined and operationalized. The instruments are freely available for use by all countries, with the aim of enhancing national health information systems and facilitating cross-national comparisons of health data.

The main goals for healthcare are: Equal access to health care for everyone; Cost-efficient production of health services and Cost-control of public expenditure for medical services. Especially cost-control seems to be a global problem. One of the key issues in the debate on how to improve healthcare is rationing. It is an important challenge to understand the various methods of rationing in medical care, to analyse the effects of rationing and the ways to harmonize the various rationing cultures in Europe. This publication gives a comprehensive overview of the perception of different population groups in an international context and it shows how the different population and occupational groups estimate the possibilities, forms and also limits rationing in the near future. Patients who are affected by rationing decisions could ask for treatment in foreign European countries. In order to limit social inequities caused by capacity problems in less rationed countries it is important in a first step to demonstrate the differences in rationing procedures between European countries.

Book excerpt: Over recent years, there has been increasing interest in the fundamental role played by local mechanical parameters in chondrocyte regulation and cartilage dysfunction as a first step in the development of osteoarthritis. This is how the idea of mechanobiology and the concept of mechanotransduction were born in the 90's. Indeed, a broad diversity of physiological phenomena is induced by mechanical stimuli (hearing, orientation to gravity, touch, tissue remodeling...) but the mechanism by which mechanical forces may regulate a physiological response is still unknown. In other respects, the concept of regenerative medicine has recently developed in parallel to this. Regenerative medicine is an emerging multidisciplinary field involving medicine, biology, chemistry, mechanics and engineering that is likely to revolutionize the ways we improve the health and quality of life by restoring, maintaining or enhancing tissue and organ functions. Indeed, human tissues do not regenerate spontaneously and healing is only a stopgap that may be associated with contraction which in turn may prevent regeneration. interesting alternative. Today, the in vitro preparation of biological tissues such as cartilage, bone, tendon, vessel, heart muscle, skin, brings out major expectations for the next decade. However, to each type of tissue correspond a large number of potential methods related to the support, the cells used (tissular cells or stem cells) and to the conditions of the environment (culture medium and mechanical forces). The aim of this publication is to provide a thoughtful and balanced view on new crucial concepts with clinical implications.

Available in PDF, EPUB and Kindle. Book excerpt: Differential Diagnosis in Neurology grew out of the author’s daily morning reports with neurology residents and the examination of patients in front of colleagues over the last 30 years. The essence of “differential diagnosis” is “splitting” rather than “lumping”. It requires bringing knowledge to the table and then adding experience. The book is meant to be a skeleton that will give the clinician a general background with regard to the disease at hand. Molecular genetics, physiology and biochemistry will uncover mechanisms and associations that will further expand differential diagnosis of all neurological disease. It is hoped that the clinician will use this volume as a workbook in which new entities are added or older classifications revised.

Book excerpt: The enzyme acetylcholinesterase (AChE) has drawn the attention of scientist as it has played a striking role in Alzheimer's disease, Cardiac diseases and other diseases. A good discussion and overview document on AChE enzyme and its various aspects was felt lacking since a long time in the field of enzyme biochemistry as well as enzyme biotechnology and this publication fills this gap. Before this publication there was no data available on AChE enzyme inhibition, its kinetics, its peripheral sites, their role in catalytic and noncholinergic function of enzyme, ACh receptors, AChE and BuChE genes etc. All these topics together form a good basis for researchers and post graduate students in this field of interest. This book is a collection of recent advanced reports and data on AChE enzyme.

Book excerpt: Provides insights not only into the work of the National Institutes of Health, but the relationship between institutional and governmental structures and the manner in which they influenced the direction taken by individual scientists. The recollections of the individuals in the intramural program juxtaposed alongside whatever primary sources have survived also provide an equally fascinating contrast. It provides a perspective that can illuminate contemporary policy debates about the nature and direction of biomedical and social science research as well as the relationships between government and science.

Available in PDF, EPUB and Kindle. Book excerpt: During the last decades, healthcare in the various industrialized countries has been deteriorating steadily. The enormous progress of medicine in the same period could only partly compensate for the loss. This book describes the deterioration of healthcare that increases continually. It analyses its causes and outlines the measures that might prevent or at least slow the change. The deterioration of healthcare, variable in different fields, progressing at different paces in the various industrialized countries, is produced mainly by two types of causes. Minor ones, resulting from lack of knowledge, or by inadequate equipment or procedures, can be corrected by relatively simple means, such as education and technical or procedural improvements. The major causes for the deterioration involve general policy whose effects are the steady decrease in monitoring health care and the change of healthcare into profit-seeking service. The data collected and its analysis is based on over 300 articles in major medical and nursing journals. It has three parts: part 1 deals with individual doctors' errors, part 2 with errors of the medical establishment, and part 3 is devoted to more theoretical a

Book excerpt: This monograph was written to present the results of the concerted action called Ethical Function in Hospital Ethics Committees funded by the European Union in the context of the BIOMED II programme. Ethics committees as a principle - national, clinical or for research - do appear as quite innovative in the hospital environment. They offer an opportunity, deep in the clinical practice, to think about wide and subtle issues of medicine, throughout a variety of discourses coming from philosophy, social sciences, law, and laymen as well.
