Book excerpt: L'objectif principal de cette étude est d'évaluer la prescription de la pilule à l'œstrogène naturel en médecine générale et de la comparer dans le cadre d'une activité de gynécologie médicale. Les objectifs secondaires sont d'analyser l'information des médecins généralistes sur les différents moyens de contraception (dont la pilule à l'œstrogène naturel comme Qlaira®, Zoély®), leurs comportements et leur place en matière de prescription. Cette étude prospective, quantitative puis comparative a été réalisée auprès des médecins généralistes (MG) et de gynécologues médicaux (GM) de la ville de Marseille durant une période de 5 mois (mai à septembre 2012). Sur 450 MG contactés, 250 ont acceptés de répondre par mail, fax ou courrier et 90 ont réellement répondus. Sur les 70 GM contactés, 40 ont accepté de répondre et 16 ont réellement répondu. Soit un taux de réponses supérieure à 25% rendant l'interprétation des résultats généralisables. Au cours de cette étude nous avons montré que la pilule reste le moyen de contraception préféré des françaises. Le médecin généraliste tient une place et une influence importante dans les choix de contraceptifs de ses patientes car il instaure dans 90% une contraception. Cela rejoint les résultats du sondage de l'Institut National de Prévention et d'Education pour la Santé. Les médecin généraliste reste fidèle aux recommandations de l'HAS dans la prescription contraceptive tout en faisant appel à son expérience. Le remboursement par l'assurance maladie reste pour lui une priorité. La pilule à l'œstrogène naturel représente 10% des prescriptions de pilules en médecine générale grâce à l'information fournie essentiellement par les revues professionnelles. Elle a montré son efficacité et sa bonne tolérance clinico-biologique. Si à l'avenir, des études à long terme prouvaient que la pilule à l'œstrogène naturel diminue les risque de maladie thrombo-embolique veineuse, elle serait alors une option de choix chez les patients dont les antécédents cardio-vasculaires contre-indiquent la prise d'une contraception hormonale. Par ailleurs du fait de sa prise en continue elle garantirait une sécurité d'emploi autant pour la patiente que son médecin. Facilité de prise et bonne tolérance étant des atouts nécessaires pour une contraception bien comprise et bien acceptée obligatoire pour baisser le nombre d'IVG toujours trop nombreuses dans notre région.
