Previous research has examined a variety of client factors (age, gender, etc.) and some therapy process factors (perceived burden of treatment, alliance, etc.) that are potentially related to therapy dropout; however, the results are far from conclusive. With data gathered as part of a previous randomized controlled trial, multilevel modelling was used to examine the extent to which child age, race/ethnicity, problem area, income, baseline symptom severity, therapist discipline, and the therapy process factors of therapeutic alliance, adherence, and competence are predictive of dropout. Results indicated that of these factors, only problem area was a significant predictor. Caregiver-rated therapeutic alliance and race/ethnicity also approached significance

Book excerpt: The presence of behavioral and emotional problems in children and adolescents can have far reaching implications for the children, their futures, and those they interact with (Deighton et al., 2016). Evidence-based practices (EBPs) are effective in addressing the symptoms and general functioning of children and adolescents impacted by behavioral and emotional problems (Deighton et al., 2016). However, a variety of client demographic characteristics and treatment factors influence the duration and outcome of mental health treatment (de Graaf, Speetjens, Smit, de Wolff, & Tavecchio, 2008). These demographic factors are associated with predicting treatment outcomes and dropout for children and adolescents (de Haan, Boon, Vermeiren, Hoeve, & de Jong, 2015; Walrath, Ybarra, & Holden, 2006). The current study was designed to identify the factors associated with predicting treatment outcome and dropout among children and adolescents that received services from one of the three evidence-based practices through a community mental health facility in the greater Los Angeles area. Significant differences between treatment modalities and dropout were revealed. There was also a significant relationship between symptom severity and dropout. Additionally, child age and diagnostic classification significantly predicted outcome. This study contributes to the existing body of literature in identifying child treatment outcomes and dropout predictors in the context of specific evidence-based practices.

It can especially a problem at the university counseling center, where there is increased demand for a variety of psychotherapy services at campus centers which can be underfunded. At these centers, a dropout represents an opportunity cost with impacts at the client, therapist, and administrative level where it is most prohibitive. While several predictive factors for dropout have been identified from previous research studies, the current study aims to examine potential predictors using a very large (n=23,144 clients in the final presented model), representative data-set gathered through a practice research network, the Center for Collegiate Mental Health (CCMH), for an in-depth analysis of a specific treatment setting. Using multiple logistic regression, previously empirically validated variables are examined along with clinically derived variables specific to the college counseling demographic, and predictive models for dropout models are presented using these variables available at intake.

Book excerpt: Clinicians often have difficulty accurately predicting whether their patients are progressing as planned while enrolled in therapy. Use of a measurement feedback system (MFS) has been shown within the adult literature to improve mental health treatment outcomes, especially for those who are most likely to dropout from treatment. An MFS’s effects are theorized to work through clinician behavior (i.e., tailoring treatment) that is elicited in response to receiving patient feedback. The current study examined an MFS’s effects on treatment length and dropout rate after being implemented in one community mental health center for youth (n = 538; 57% male, 52% Latinx/Hispanic, M age = 9.80). Level of clinician implementation of the MFS was also examined, and higher levels were found to impact treatment outcomes. Limitations include systemic issues related to archival data, including use of a historical control. Clinical and theoretical implications of these findings are discussed, in addition to recommended next steps for researchers interested in dissemination and implementation of effective mental health treatment
