Book excerpt: The purpose of the current study was to investigate the reliability, concurrent validity, and criterion validity of Minnesota Multiphasic Personality Inventory-2-Restructured Form (Ben-Porath & Tellegen, 2008/2011) scores in a sample of 811 chronic low back pain patients (346 males, 529 females) beginning treatment in a short-term interdisciplinary pain rehabilitation program. We calculated internal consistency coefficients, mean-item correlations, and standard errors of measurement for all substantive scales, as well as zero-order correlations with collateral medical record information and self-report testing. For criterion validity analyses, we examined associations with emotional distress and pain disability outcomes. Results indicated reliability and validity for most of the MMPI-2-RF substantive scales. Descriptive analyses across diagnostic groups indicated that the MMPI-2-RF Restructured Clinical Scales had substantially improved discriminant validity when compared to the MMPI-2 Clinical Scales. Higher scale scores from all the MMPI-2-RF substantive domains were associated with worse emotional distress outcomes, with measures of negative emotionality demonstrating the strongest associations with poorer post-treatment functioning. Consistent with the bio-psychosocial model of pain, higher scores on scales from the Emotional/Internalizing, Thought Dysfunction, and Interpersonal domains were associated with worse pain disability outcomes. Implications of these findings and limitations of this study are discussed.

Though multidisciplinary treatment programs have been established in attempt to address these multiple domains, not all chronic pain patients benefit from treatment. This study aimed to identify specific pre-treatment psychological and personality factors that predict poor treatment outcome as measured by functional impairment. The Minnesota Multiphasic Personality Inventory-2 Restructured Form's (MMPI-2-RF) Restructured Clinical scales (RC), Specific Problems scales, and Psychopathology-5 scales were examined in the prediction of perceived disability due to chronic pain, measured by the Pain Disability Index, in a sample of chronic pain patients (N=232) at intake into treatment, discharge, six months post-discharge, and 12 months post-discharge. Time-nested under person Hierarchical Linear Modeling was utilized in these analyses. Results indicate that individuals scoring higher on Somatic Complaints, Low Positive Emotions, Neurological Complaints, Cognitive Complaints, Helplessness/Hopelessness, Disaffiliativeness, Psychoticism-Revised, and Introversion-Revised had higher Pain Disability Index scores over treatment and post-treatment compared to individuals scoring lower on these scales. An interactive effect between Multiple Specific Fears (MSF) and Time was found in that individuals with higher MSF scores exhibit a rate of change in Pain Disability that differs from those scoring lower on MSF. Clinical implications and limitations are discussed.

Book excerpt: Profiles of the Minnesota Multiphasic Personality Inventory (MMPI) and Back Pain Classification Scale (BPCS) (Leavitt, 1979) were compared on 65 chronic low back pain patients to determine whether they would yield similar outcomes in predicting the presence of emotional factors contributing to pain complaints. MMPI profiles were classified according to Bradley's profile criteria (1978), predicting poor medical treatment outcome. A Chi Square analysis indicated that the two profiles were independent of each other. MMPI profiles of patients obtaining a High rating on the BPCS were compared with those obtaining a Low rating. A t test Analysis indicated there were no significant differences in mean MMPI scale scores of the High versus Low except on the Hy Scale, indicating that only the Hy Scale of the MMPI could predict a difference between High and Low scores on the BPCS at a probability of.0001. Additional research which emanated from this study is offered.
