It can especially a problem at the university counseling center, where there is increased demand for a variety of psychotherapy services at campus centers which can be underfunded. At these centers, a dropout represents an opportunity cost with impacts at the client, therapist, and administrative level where it is most prohibitive. While several predictive factors for dropout have been identified from previous research studies, the current study aims to examine potential predictors using a very large (n=23,144 clients in the final presented model), representative data-set gathered through a practice research network, the Center for Collegiate Mental Health (CCMH), for an in-depth analysis of a specific treatment setting. Using multiple logistic regression, previously empirically validated variables are examined along with clinically derived variables specific to the college counseling demographic, and predictive models for dropout models are presented using these variables available at intake.

Therapist-related factors that serve to reduce this risk are essential to elucidate given the consequences of premature dropout. The therapeutic alliance is a commonly studied component of psychotherapy (Martin, Garske, & Davis, 2000) and may reduce the risk of dropout from treatment (Cooper et al., 2016; Principe, Marci, Glick, & Ablon, 2006; Tryon & Kane, 1995; Wnuk et al., 2013). Although less well-studied, validation and invalidation are also theorized to be important constructs in therapy outcomes (Linehan, 1997; Lynch, Chapman, Rosenthal, Kuo, & Linehan, 2006). However, these variables have received limited attention in regard to dropout research. I examined the relation between these therapist-relevant constructs (i.e., therapeutic alliance, validation, and invalidation) and dropout from treatment. Participants included clients in psychotherapy (n = 52; 79% female; 83% Caucasian) and therapists (n = 22; 68% female; 86% Caucasian) in three graduate training clinics. Clients, therapists, and outside observers rated therapist validation, invalidation, and the therapeutic alliance (alliance was only rated by clients and therapists) across four early sessions of therapy (i.e., sessions 3-7). Scores for each construct were averaged across the four sessions and examined as predictors of client dropout from treatment. Client ratings of therapist validation, invalidation, and the alliance all independently predicted dropout. Clients who reported a strong alliance and more therapist validation were less likely to dropout, while higher ratings of invalidation predicted a greater risk of dropout. Therapist and observer ratings of these constructs were unrelated to dropout. With regard to these therapist-relevant constructs, only the client’s view significantly predicted treatment dropout. Assessing and attending to client perceptions of alliance and validation/invalidation by the therapist early in treatment, through brief self-reports, can alert therapists to clients at greater risk and, thus, possibly prevent further dropout.

The most accurate dropout definitions included using a statistical modeling procedure to determine whether the client's rate of change at the final session was greater than average of change for all clients; clients who were still demonstrating gains greater than the average rate of change at the final session were considered to have terminated prematurely. A total of 177 couples (354 individuals) who sought therapy in the VA medical centers in Charleston, SC and San Diego, CA were examined. With a few exceptions, demographic variables generally did not predict dropout. A couple's relationship adjustment and response to conflict were significant predictors of dropout. The content of therapy sessions predicted dropout only when dropout was defined, at least in part, by client's rate of change at the final session, suggesting that such methods of defining premature termination are the most sensitive to the therapy process. Therapists' characteristics, including gender and level of experience, did not predict dropout across all six definitions of dropout.

Book excerpt: The primary goal of this study was to provide an updated meta-analytic review of the psychotherapy dropout literature in a methodologically rigorous fashion. In addition to analyzing research that had not yet been included in a meta-analytical review of premature termination, this study analyzed variables that were not examined in the most recent meta-analysis of premature termination (Wierzbicki & Pekarik, 1993). The meta-analysis was conducted using 73 studies published from 1990 to 2006. Study descriptor variables and empirical findings were coded, and effect size was calculated using Cohen's d (Cohen, 1988). Data were analyzed according to Shadish and Haddock (1994) and Hedges and Olkin (1985). Moderator analyses were conducted to examine the potential impact of study descriptor variables according to Hedges (1994). Therapeutic alliance, treatment expectations, patient motivation, patient self-efficacy, patient hostility, and patient impulsivity were found to be moderately strong predictors of premature termination. Patient psychological variables (symptom severity and diagnosis), therapy and therapist variables (treatment length, treatment type, and therapist training), were less strongly related to premature termination, consistent with previous reviews of psychotherapy dropout literature. Simple patient demographic variables (age, gender, marital status, and level of education) were found to be weak predictors of premature termination. An additional analysis calculated a 35.26% average dropout rate across 110 studies; average dropout rate varied significantly according to country of study, study design, and primary diagnosis. The current study did not find that dropout rate varied as a function of dropout definition. Limitations of the present meta-analysis as well as directions for future research were discussed.

Book excerpt: Psychotherapy research has shifted from mainly focusing on the average effects of different treatments to concentrating more on questions related to the individual patient. When research attention shifts, it can give rise to the implementation of new statistical methods that, in turn, can illuminate new challenges that must be addressed. The aim of the thesis was to study how traditional methods for predicting certain psychotherapy outcomes have been conducted in the past, and how more advanced statistical methods might be used to enhance knowledge of how to predict these outcomes today. Three studies were performed: Paper I focused on how Multi Level Modeling (MLM) can be used to study certain aspects of the relationship between working alliance and treatment outcome. In Paper II, Latent Profile Analysis (LPA) and item-level analysis were used to give nuance to the relationship between psychological distress at baseline and change rate during treatment. Finally, in Paper III, Machine Learning (ML) was used to detect dropout patients in the early phase of treatment by exploring complex patterns of symptom distress during the early phase of treatment. The thesis showed how different goals of scientific exploration can be studied in the context of routine care with the use of these statistical frameworks and discussed some of the challenges and opportunities worth noting when entering this line of research. Psykoterapiforskning har på senare tid gått från att fokusera på genomsnittliga effekter av olika behandlingsinriktningar, till att inrikta sig mer mot den enskilda patienten. När fokus förflyttas på det här sättet kan det leda till att nya statistiska metoder behöver tillämpas vilket i sin tur kan leda till nya utmaningar för psykoterapiforskaren. Syftet med avhandlingen var att undersöka hur traditionella statistiska metoder har använts för att studera olika typer av psykoterapiutfall i rutinmässig vård, och hur mer avancerade statistiska metoder kan tillämpas för att öka kunskapen om hur dessa utfall kan prediceras. Tre studier genomfördes. Studie I fokuserade på hur flernivåanalys kan användas för att studera relationen mellan arbetsallians och behandlingsutfall. I studie II användes latent profilanalys för att studera sambandet mellan psykologiska besvär vid det första besöket och symptomförändring under behandlingens gång. Slutligen, I studie III, tillämpades maskininlärning för att upptäcka patienter med risk att hoppa av behandlingen i förtid. Sammanfattningsvis belystes i avhandlingen hur olika typer av vetenskapliga frågeställningar kan studeras i en klinisk kontext med hjälp av dessa statistiska ramverk samt några av de fördelar och begräsningar som är viktiga att notera när de tillämpas.

Book excerpt: Treatment dropout is a pervasive phenomenon that can preclude clients from benefiting fully from psychotherapy. Research efforts to understand the phenomenon in the preceding decades yielded few consistent results. The investigation of intrinsic client and therapist factors gave way to the more recent exploration of dynamic therapeutic process factors potentially influencing the dropout process. The availability of periodic treatment outcome measurement instruments has helped client-focused research explore the effects of treatment response as a process factor on aspects of psychotherapy. As an added benefit, real-time treatment response measures, such as the Outcome Questionnaire, offer the possibility of timely adjustment in clinical intervention to meet evolving client needs and enhance therapeutic treatment. This present study primarily sought to explore the relationship between psychotherapy dropout and treatment response patterns in terms of clients' psychosocial well-being as measured by the Outcome Questionnaire. The results suggest that treatment response patterns alone may not effectively predict dropout probabilities. Even so, the measure's sub-component assessing the client's subjective experience of symptom distress is shown to be more accurate in predicting dropout than the composite Outcome Questionnaire measure or any other component scale. This finding conceivably highlights the relative importance of symptom distress in the dropout process for the sampled clients. Those clients reporting higher levels of symptom distress appeared to be associated with greater probabilities of dropout termination. Additionally, prior research has recognized a likely mediated relationship between higher client educational attainment and lower dropout probabilities - a trend also observed in this study's sampled population. As one of its expressed intents, this study examined educational attainment's moderating effect on the relationship between aspects of client treatment response and dropout probabilities. While showing educational attainment to be a relevant factor in assessing dropout risks, the analysis results indicate that this client characteristic variable's interactional effect on the evaluated treatment response pattern feature is weak and statistically nonsignificant. The present study contributes to the research literature through providing some clarification to the importance of treatment response in the prediction of psychotherapy client dropout.
