Book excerpt: In this study, a non-linear Finite Element (FE) model was created and analyzed to determine the pressure distribution between the residual limb and the prosthetic socket of a transfemoral amputee. This analysis was performed in an attempt to develop a process allowing healthcare providers and engineers to simulate the fit and comfort of transfemoral prosthetics to reduce the number of re-fittings needed for the amputees. The analysis considered the effects of interference due to insertion of the limb into the prosthesis, referred to as donning, and also the effects due to the body weight of the amputee. A non-linear finite element static implicit analysis method was utilized. This analysis implemented multiple finite element techniques, including geometric non-linearity due to large deflections, non-linear contacts due to friction between the contact surfaces of the residual limb and the socket, and non-linear hyper-elastic material properties for the residual limb's soft tissue. This non-linear static analysis was carried out in two time-steps. The first step involved solving the interference fit analysis to study the pre-stresses developed due to the effect of donning. The donning process results in soft tissue displacement to accommodate the internal geometry of the prosthesis. In the second load application time-step, an additional load of half the person's body weight was applied to the femur. The maximum normal stress (contact pressure) of 84 kPa was observed due to the combined effect of the donning procedure and body weight application, comparable to the studies performed by other researchers. The procedure developed through this work can be used by future researchers and prosthetic designers in understanding how to better design transfemoral prosthesis.

The lower limb prosthesis consists of a socket, which interfaces with the residual limb, a pylon, attachment hardware to secure the pylon to the socket, and a prosthetic foot. For the prosthesis to be effective, the socket must be comfortable, functional and aesthetically appealing, usually in that order. Lack of comfort and fit can cause movement problems and health issues. The residual limb of the amputee changes its volume throughout the day and in order to maintain comfort a socket must be able to adapt to these volume changes. Previous research has resulted in the development of concepts for inflatable prosthetic sockets capable of addressing this need. The concepts rely on laser sintering (LS) to manufacture the parts. This research focuses on the development of a finite element analysis (FEA) method to assist in the design of adaptive sockets. The FEA can be used to predict the pressure-deflection curve of a given socket design. The FEA method was verified by experiments using LS manufactured test specimens. Socket stress distribution is performed on three types of socket, polypropylene (5mm), polypropylene (3mm) and a standard laminate (3mm) sockets to determine the stresses path through the prosthetic socket during the gait cycle. It is found that heel strike phase is the critical phase. The stresses increase at the socket base while the maximum deflection is maximum at the patellar region of the socket. This work is achieved by finite element program, ANSYS. The development of design specifications of a below the knee prosthetic socket is presented in this study for successful ambulation and comfort. Seven different design models are created by changing the socket wall thickness and the material. Polypropylene socket gives the best results with regard to the allowable deflection for the regions of the pressure relief areas of the stump. The effect of varying the modulus of elasticity on the polypropylene socket is considered in this study. Three models are analyzed with different tensile creep moduli during different periods. It is found that the polypropylene socket will enlarge in size with time. An experimental study is conducted to compare the strength of five prosthetic sockets made of different materials. Compression, three-point flexural and tensile tests are implemented by the Testometric machine. The laminate sockets have larger compressive stiffness than polypropylene while polypropylene has larger flexural stiffness than the tested laminates except for socket No.5.

Book excerpt: For people living with limb amputation, the prosthetic socket - the interface between the residuum and prosthesis - is the most critical component. When a socket is uncomfortable, especially due to poor fit, the quality of life for a patient is greatly hindered. However, conventional design of sockets is largely artisan, with limited input of quantitative data. Current computer-aided and manufacturing (CAD/CAM) designs are still not clinically applicable solutions. Due to model identification procedures that employ non patient-specific and incomplete data sets, today's finite element (FE) models of the residuum are not predictive, leading to suboptimal socket designs. As such, there exists a need for a comprehensive biomechanical model of the residuum for the quantitative design and computational evaluation of patient-specific prosthetic sockets. This thesis presents a combined experimental-numerical approach to evaluate and validate a transtibial residuum biomechanical model. The central hypothesis of the work is that a single biomechanical model can predict the large non-linear response at various sites on a residuum under load. To evaluate this hypothesis, a non-linear, two-tissue model was formulated where tissue geometries were defined using MRI data of the residuum. The non-linear viscoelastic material parameters of the model were identified through inverse FEA-based optimization using in-vivo indentation experimental data at four locations. Using optimized model tissue parameters, the mean percentage error (mean absolute error/ maximum experimental force) between the experimental and simulation force-time curves at 14 other locations across the evaluated transtibial residuum was 7 ± 3%. Using this same modeling methodology and a single set of material constants to describe the bulk soft tissue biomechanical response of seven distinct transtibial residual limb models, the average percentage error for indentations at multiple locations across all seven limbs was 7 ± 1%. From these predictive models of residuum limbs, one rigid novel socket and two multimaterial transtibial sockets were designed, fabricated and evaluated through an entirely quantitative, automated and repeatable methodology. In a preliminary clinical investigation, the novel sockets were shown to reduce peak contact pressures at the tibia and fibular head regions on the residuum by significant amounts during standing compared to a conventional socket interface designed and fabricated by a trained prosthetist.

Prosthetic sockets enclose the residual limb and connect to other parts of the prosthesis to facilitate control and promote a more natural gait pattern by the user. One of the main challenges is to create stability between the residual femur and the prosthetic socket, thus improving efficiency and comfort whilst reducing gait deviations.Established designs such as the ischial containment socket (ICS); purport to reduce femoral abduction by containing the medial aspect of the ischium in counterforce to the greater trochanter and femoral shaft. This design relies on a proximal 'bony lock' to limit femoral abduction at mid stance within the prosthesis. A new design of socket named the compression/release socket (CRS), however, uses three or more 'compression' regions between the femoral bone and the socket walls to minimise femoral motion. The CRS design claims to improve the gait pattern of the user through enhanced distal femoral stability and socket control. Despite minimal evidence being available, the design is currently being introduced in clinics worldwide.The aim of this research is to compare the stabilisation achieved in two transfemoral prosthetic socket designs by identifying and measuring the effect on the user using appropriate gait parameters. Initially, a literature review was conducted to establish understanding of normal and prosthetic gait. Literature was then examined to establish historical prosthetic transfemoral socket concepts and the biomechanics underpinning currently accepted designs. Due to the difficulty in obtaining participants appropriately fitted with the CRS sockets, this project has been presented as a feasibility study. Two designs of sockets were then compared: ICS versus CRS design. The gait of one subject with transfemoral amputation was assessed whilst wearing both sockets in a gait laboratory. Important kinematic and spatial temporal parameters, identified in the literature review, were compared for both socket designs under a variety of challenging walking conditions (level, downhill, uphill and cross slope walking). Comparison was also made between the amputated and contralateral side for each socket design. Results indicate that, kinematic parameters did not demonstrate large variations, but that some variation was found between prostheses when measuring hip abduction, hip flexion, knee flexion and ankle plantar flexion in both the prosthetic and contralateral limbs. When using the CRS design, user range of motion for the hip joint appeared to be greater than that of the ICS. Step length and step width on both sides were closely matched in both socket designs. Preliminary study results indicate that alteration in the step length or width did not reduce the velocity of gait, but was more likely to result from change of strategy by the participant to ensure greater control over balance or stability. Further research with a greater number of participants is required to investigate the effect of socket design on stability and gait parameters in the future.
